My vacation was memorable for two things:

1. Nakat-on kog Inilonggo ug misaad nga di nako hikalimtan ang maong pinulongan.
2. Nakahukom ko nga dili ug dili gajud magpatuli.

Decades have passed and I'm proud to say that I have kept my promises...

the doctor was somewhere else; probably in another hospital, or so i would like to believe.  perhaps he was tackling a life-and-death emergency, who knows.  i can take comfort though in the fact that none of us, maybe 6 or 7 bored patients-in-waiting, was an emergency case.  doctors understandably hardly have time on their hands, but it would be nice if some time estimates can be had so patients like me need not waste our own precious time.

Well, you can always reschedule. :)

Speaking about patients waiting-- today while I was doing my clinical duties, I entered a room in which a patient was literally 'red' in the face. She was waiting literally 2 hours to be seen, unsaon man lagi, daghan kaayo pasyente pero only few attending M.D's were present and fewer interns (5, and i was included in that number). Suko lagi tong pasyente, pero, kind words and reassurance helped mellow her down.

I am empathetic to your situation, Isles. Hopefully next time your M.D won't be late again. ;)

6  When Jesus saw him and knew he had been ill for a long time, he asked him, “Would you like to get well?”
7 “I can’t, sir,” the sick man said, “for I have no one to put me into the pool when the water bubbles up. Someone else always gets there ahead of me.”

8 Jesus told him, “Stand up, pick up your mat, and walk!”

9 Instantly, the man was healed! He rolled up his sleeping mat and began walking!

January 04, 2012, 01:08:26 PM
Laparoscopic cholecystectomy is a procedure in which the gallbladder is removed by laparoscopic techniques. Laparoscopic surgery also referred to as minimally invasive surgery describes the performance of surgical procedures with the assistance of a video camera and several thin instruments.

During a laparoscopic surgical procedure, small incisions of up to half an inch are made and plastic tubes called ports are placed through these incisions. The camera and the instruments are then introduced through the ports which allow access to the inside of the patient. The camera transmits an image of the organs inside the abdomen onto a television monitor. The surgeon is not able to see directly into the patient without the traditional large incision. The video camera becomes a surgeon’s eyes in laparoscopy surgery, since the surgeon uses the image from the video camera positioned inside the patient’s body to perform the procedure.

Benefits of minimally invasive or laparoscopic procedures include less post operative discomfort since the incisions are much smaller, quicker recovery times, shorter hospital stays, earlier return to full activities and much smaller scars. Furthermore, there may be less internal scarring when the procedures are performed in a minimally invasive fashion compared to standard explorative surgery.

January 04, 2012, 12:53:02 PM
Craniotomy is a cut that opens the cranium. During this surgical procedure, a section of the skull, called a bone flap, is removed to access the brain underneath. The bone flap is usually replaced after the procedure with tiny plates and screws.

A craniotomy may be small or large depending on the problem. It may be performed during surgery for various neurological diseases, injuries, or conditions such as brain tumors, hematomas (blood clots), aneurysms or AVMs, and skull fractures. Other reasons for a craniotomy may include foreign objects (bullets), swelling of the brain, or infection. Depending on the reason for the craniotomy, this surgery requires a hospital stay that ranges from a few days to a few weeks.

Here in the U.S. a test will be done to determine if the causative agent is a virus or bacteria
and if it's bacteria a sensitivity test will determine the drug  to be used.
In case of  viral infection then the medication will be only  for the symptoms and not any antibiotic.
At present there is no drug in the market without any side effect, so the best defense is to keep
the immune system at it's best.

Reaching for an apple, some oatmeal, or an herb before you have to start taking an antibiotic is smart. Antibiotics are now the leading cause of adverse drug reactions in the United States and the cause of oft en deadly strains of resistant bacteria. In addition, antibiotics are commonly overprescribed. Even though, for example, most sore throats are viral and not bacterial in nature (therefore, resistant to antibiotics), 73 percent of viral sore throats are oft en treated with an antibiotic prescription, according to a yearlong study by the U.S. Food and Drug Administration Center for Drug Evaluation and Research.

While super pills may help with super infections caused by bacteria, drugs such as penicillin and sulfur drugs can lower your resistance even as they help you fi ght back. In the process of killing those bad bugs, antibiotics deplete a wide spectrum of nutrients that are the foundation for immunity, including vitamin C, iron, zinc, and most of the B vitamins, plus bone-building vitamin K, calcium, and magnesium. In addition, antibiotics wipe out your good bacteria, so they are best used only when truly necessaryâ€"and even bett er, avoid them altogether with super immunity nutrition.

Good article!

What is a Cardiac Tamponade?

Answer: This is a pathological accumulation of fluid (serous or blood) within the pericardial cavity. The fluid compresses the heart and restricts the venous filling during diastole and reduces cardiac output.

To treat this condition a pericardiocentesis is performed with a needle at the left infrasternal angle through the cardiac notch of the left lung to remove the fluid.

Quote from: A Layman on December 22, 2011, 06:57:08 PM
That's exactly right Doc. All individuals have a childhood dream. Often, one's dream is prompted by family conditions and environmental influence. In example, when a child is raised from a poor family and hostile environment, he is prompted to dream a wealthy man someday to alleviate the situation he experiences. He may strive to seek a higher level of learning to hone himself to achieve his goal or any available means of his choice. However, in the process, there is no room for convenience and complacency because one has to struggle the hardest way to achieve that dream.

I agree with that statement, A Layman, "When a child is raised from a poor family and hostile environment, he is propted to dream a wealthy man someday to alleviate the situation he experiences." This is so true in that we all have a sense of existentialist self-improvement. I believe that all humans want to better themselves either it be in their day to day lives or through their progeny. An example of a day to day self improvement would be a carpenter who works 50 hours a week and gets paid 20 dollars an hour. So technically, he makes $1000 a week, which equates to $4000 a month. However, after taxes, he makes $3000 a month. Then he has to pay his house mortgage, electricity, gas, internet, health insurance, food expenses, children's expenses. So that leaves him with about $500 a month on savings. Will this man be content with only $500 a month on savings? No. We all are aware of the circumstances that come at us in life, and it is only prudent to save. So this man elects to work 60 hours a week; an additional 40 hours a month, which means an additional $800 saved. By increasing work hours, he can save ~ $1000-$1200 a month. In one year, he can theoretically save up to $10,000 - $12,000.

In regards to long term aspiration; every father or mother would like their children to do better for themselves than they. It is preprogrammed in the human brain as an offspring's success means genetic longevity.

Mao ba? Maswerte ka sa imong amahan Doc! Pero as I guess mas swerte sya to have you as his child. Civil Engineers are destined to build roads and bridges for human communications to flow smoothly, erect buildings where humans are provided with oportunities to work efficiently and provide a decent living for their families, and houses to provide them shelter. Pero mas superb ang missions ng mga doctors like you.

A Layman,

Civil Engineering is indeed a noble profession. An ancient one, which attests to the growth of human civilizations through the ages; from bronze age, to iron age, to classic age, to enlightenment, to modern. I find great similarity between Civil Engineering and Medicine. Both professions are specialized and tasked in the maintenance of pathways and the prevention of accumulation of 'traffic'. So that there is easy flow throughout the nation/body. The only remarkable difference is that medicine deals with the human body (maintenance of microscopic aspects of the body that affects the organ/ organismal level); whereas in engineering deals with the national body (or provincial/state body)  and is responsible for the maintenance of roads, bridges, docks etc. So I would say that Civil Engineering and Medicine is at par with each other. One is responsible for the maintenance of civilization, the other is tasked with the maintenance of the human person that lives within said civilization.

On another point, it is I who is lucky to have a father like my dad. Without whom and without whose guidance, I would be nothing. :)

Good Morning Tubag Bohol!

I hope that everyone is off to a great start this week (the first week of 2012, might i add)! The time here is 9:23 AM Central Standard Time. I'm off to starting my first day of Psychiatry Clinical Rotation at the hospital. Since its the first day, I'm probably going to be assaulted with clinical lab data to figure out for myself and then be expected to reiterate the information to attending/ chief of psychiatry. Oh the life of a medical intern!

I have to accept the fact that I am going into that direction as you are. Although my physical condition may not manifest such a human transformation, I heard from around children calling me 'Lolo' 'coz of the snow froast at my top but the furnace deep inside of me is still burning as a young carabao. So, I partly affirm such condition with an open mind. LOL! And I still hope to add some more in my record book! Well, life must go on 'igan' but as soon as you feel  that sign, be cool and stay young as you are! Merry Christmas to Everyone in TB!

A belated Merry Christmas to you, A Layman!  Thanks for this post, which I was able to read just now. The culmination and crowning glory of any man is experience, and grey hairs , which attest to his battles with life's tests. Thanks for sharing your experiences with us.

1 Therefore, holy brothers and sisters, who share in the heavenly calling, fix your thoughts on Jesus, whom we acknowledge as our apostle and high priest. 2 He was faithful to the one who appointed him, just as Moses was faithful in all God’s house. 3 Jesus has been found worthy of greater honor than Moses, just as the builder of a house has greater honor than the house itself. 4 For every house is built by someone, but God is the builder of everything. 5 “Moses was faithful as a servant in all God’s house,” bearing witness to what would be spoken by God in the future. 6 But Christ is faithful as the Son over God’s house. And we are his house, if indeed we hold firmly to our confidence and the hope in which we glory.

Hebrews 3: 1-6
