Right now , I am at work. Recently, I finished a doctoral class on Organizational Leadership on Health Administration. I was very happy to get my grade result. I really need an editor or a reader who can be willing to read my doctoral papers. It is really imperative I find one soon. If there any of you here in TB who are willing to read my papers (i have to normally complete 2 papers a week; ranging from 5-10 pages in length each paper), I would be appreciative.

I am thinking about my good friend Albert, who I recently was able to catch up on the phone yesterday. He is having some issues involving his father who recently had a thyroidectomy done because they found papillar carcinoma on his thyroid. I was trying to encourage him and pray with him on the phone. I thought about him last night before going to bed and also all day today. It is hard because he is my very good friend and also a fellow Doctor. He is currently studying for his upcoming Medical Board Exams and it is hard for him to deal with this going on with his papa.

I know that prayer is a powerful tool. So I am asking all of you here in Tubag Bohol who are prayer warriors , please pray for the complete and speedy recovery of Albert's father as well as for spiritual encouragement on Albert. Palihug , mga higala. He needs our prayers.

So far, research is going good; sa una na overwhelmed ko sa dictum sa required, pero dili man diay 'impossible'. Nag busy ko sa una reading over some 30 reference papers, pero interesting kaayo ang result sa ahong findings, naay relationship gajyod sa child abuse, child maltreatment og developmental abberation, unadaptive personality og greater frequency og externalization of behavior.

Mao lagi, ayaw'g jyud i-abuse ang bata, kamo mga amahan og inahan. Kai og ka grabe ra pod injong discipline, ma affected gajod ang bata, dili lang sa ilang personality development, pero naa poy high relative risk og psychopathology.

Natural ra man na, Father, sa intern years sa pagkadoktor, it is the time to experience all things. One has to keep an open mind ; and remind oneself that "you cannot save everyone of your patients." ka tinuod baya na. The first time we loose our patient is the most painful and most heart-wrenching experience, but one has to understand that as a doctor, one cannot always save everyone.

When I lost my first patient, I really took it hard. We tried our best to resuscitate her, as in all 4 of us that were working on her were doing our best to revive her heart (patient had died due to STEMI (st-elevation myocardial infarction) secondary to a massive occlusion of the left descending coronary artery). When I left the room, I could not believe that she died; and within minutes, I was consumed by such a massive and overwhelming burden and pain. I had to explain to her husband and her kids that their mother "did not make it ". It was burden to see the kids cry and clench their dad. They kept on saying, "mommy! we want to see our mommy! please , doctor, can't you make her wake up again?" The youngest child (8 years old) was saying this to me.

I had to get out of there because i was rolling in tears. It was so very hard to see the kids cry. I had to talk to my attending about this.

Anyways, my attending told me, "Lorenzo, this is a natural pain you experience. Recognize it, don't deny it. You cannot save everyone, we are not God."

He was right.

I just came back from work and lo and behold it was raining as i was walking back from my car to my apartment. Na unsa man ning weather diri sa Chicago oy! First it is cold, then it is warm, then it is cold again, now it is raining!

Am now reviewing notes and organizing my upcoming paper. I am now brewing coffee to help me stay up tonight. I think it will be another long night of studying for me. Later on, I will have to brew Colombian coffee. It's gonna be a long night of reviews for me.

Today at work, I talked to an older RN in the floor and asked how she was doing. I call this particular RN a "Tita" because she tries to help me in the floor and always makes good conversation. Today, I asked her how she was doing and she always says, "Okay lang gehapon, Doy, Datung Pobre."

This particular nurse is from Bohol.

So in response to her response ingon pod ko, "Ah, ka dato-a nimo, Tita, uy! Kai datuhang pobre man. Ako , pobreng pobre lang gehapon".

Ning katawa gajod mi duha. Ning bahakhak sija gajod. As in. LOL.

The MD who I was rotating with said to me, "Doc, what are you saying to make these nurses laugh so hard?"

Ingon lang ko, "AWWW, you wouldn't understand....its a Filipino thing!"

He kept on saying, "Try me! Try me!"

Crazy laughs gajod. Ah, it kept the night funny at least. Made me laugh so hard ning hilak ko.

Today I attended a meeting from 4 pm till 6 pm to discuss clinical cases, we had to get dressed up to present medical cases to fellow M.Ds. I presented cases revolving neurological disorders. I really don't know why they wanted us to dress up in suits. I would've preferred wearing scrubs and a white coat.

we went for a long hour drive today to the mountain, really it beautiful over there..post the view soon.

OK lang, saonz dili naman ko healthy... diabetic since 1995 or maybe even earlier. Sukad ni singwewen ko nisamot naman daghan gibati hahaha (has nothing to do with basketball, 1995 pako niundang ana). Morag second home naman gani nako ang hospital, bwahahaha!!!

Hmmm... may I add, today we went to Saigon Pho, a Vietnamese restaurant... akong ge treat daan ang 2 ka mothers for Mother's Day - mama sa akong house keeper ug my Meemaw, the old lady who took me in after the floods of Hurricane Katrina in 2005 - kay adto man sila with their families this coming Sunday. I invited the whole family (6 of us) early this evening... and food, as always, was terrific.

One student of mine said to me, "Doc, you're like my son's age!"

Last night, we had a patient come in for gun shot wound , came to the E.R. I was part of the team that was on-call last night. Bata pa intawn ang pasyente, i don't know why he was already engaging in gang activity, which led to his death. Bata pa intawn. This is so stupid why kids join gangs.

Go to school, improve yourself...and not depreciate your life by going to gangs and proliferating crime!

This particular patient had a gun shot wound to the occiput of his skull the size of a 1-dollar coin. The bullet had torn a whole in his posterior cranium at the level of the occiput, brazed the meningeal layer. Considering the damage it had done, the bullet had torn through the dural mater, causing a profound subdural hematoma. The patient was dead within 15 minutes.

Such an absolute waste of life. Total waste of life.


DON'T JOIN GANGS! GO TO SCHOOL. LIVE.

One thing to remember, if a patient is suffering from chronic grand mal seizures, immediately treat the patient with IV lorazepam. If this does not work, try phenobarbital, if that does not work, immediately take an MRI/CT scan on the patient.

One of my patients had been suffering from chronic grand mal seizures, college-age female student. The patient came in the night prior while seizuring. Under the auspiece of the attendign MD, we had provided IV lorazepam. She stabilized.

Some couple of hours later, the same patient was seizuring again. Ka kulbaan ko kai we had given her IV lorazepam, it did not work. I ordered to give IV diazepam, did not work. I had ordered to give IV phenobarbital, still, not working. Patient coded on me. She went into VENTRICULAR TACHYCARDIA. At this time, I was really kulbaan. So, i took charge and immediately continued with ACLS procedure. Defibrillate the patient. Thank goodness we were able to bring her back.

Later on, after CT scan was done, we found out she had a minor subdural hematoma , that was causing and proliferating the grand mal seiures. The patient was taken to the OR , and the surgeons operated to correct the aneurysm. Had we not found this, she would have died.

Life, truly, is too fragile.

The time is 1:32 PM, I have duty again today. Mo larga na pod ko balik na pod sa trabaho by 5:00 pm. Lord God, my God, please imbue in me calmness and the spirit of trust in you today. Carry me when I am weak, solidify me when I am feeling faint , and please, Lord God my God, do not leave my side when I need you most. Lord God , Jesus Christ, sanctify me. In you , alone, I place my trust.

I cannot stress the importance of getting along and trusting the nurses in the floors. I have learned so much from the nursing staff here who help guide the new medical interns ; so many of them may have "rough" exterior at first, but if one gives them respect, and develops a working relationship with them, the nurses become "life savers" for doctors. Plus, I have developed a fondness for their warm salutations...

So, to all the nurses ... much respect for you guys!

Today we walked with our puppy, Maxine, throughout the Chicago lakeside up all the way towards Evanston. The view was beautiful today, the weather was awesome ! Summer na gajod diri. Ning luto pod si babycakes og adobong manok (Bacolod-style) , aww ka nami gid , for dessert, we had vanilla ice cream served with fresh kiwi, strawberries, mango and oranges.

Oh you like Chinese food eh? He he he ! What did you have that was good? Chicken lo mein?
