HURRY! BUSINESS PERMITS AND LICENSES RENEWAL,JAN. 2-20, 2008 (FROM THE TAGBILARAN CITY PERMIT AND LICENSE DIVISION UNDER THE OFFICE OF THE CITY MAYOR)

   Avoid rush and penalty!

   Renew your Mayor’s Business Permit before JANUARY 20, 2008

1.   All businesses are to be renewed within the first twenty (20) days of January. During this period, the City Government of Tagbilaran has a ONE – STOP SHOP wherein all offices with direct participation in the processing of the application for renewal of business permits are housed in one place to help facilitate the smooth flow of documents. The One–Stop Shop shall be in operation for the first twenty (20) working days of January, unless extended.
2.   To avoid delay in the processing of business permit, the following requirements are to be complied with:


a.)   Previous Mayor’s Business Permit
b.)   Barangay Clearance      -   From barangay where business
establishment or activity is located
         c.)   Community Tax      -   Currently issued from the City
                     Treasure’s Office (photocopy)
         d.)   Tax Clearance         -   From the City Treasurer’s Office
                     showing that the operator has paid all                      tax obligations in the City
         e.)   PIN            -   From the City Assessor’s Office
f.)   Affidavit of Annual Gross Income (Form) – From the City Treasurer’s
Office
Other Requirements (Case to Case Basis):

         a.)   Lease of Contract      -   All who are renting spaces for their
                     business
b.)   Photocopy of the Mayor’s Permit as “Real Estate Lesor” – from the
owner of the building or land
         c.)   License to Operate      -   From BFAD, in case the license
                     expired (owner of drugstores, drug
                     testing centers)
-   From Camp Crame, in case the
license expired (owner of insurance
agencies)

REQUIREMENTS FOR EMPLOYEES OF BUSINESS ESTABLISHMENTS

1. Barangay Clearance         -   Where business establishment is located
2. Community Tax         -   From Tagbilaran City (photocopy)
3. Police Clearance         -   From Tagbilaran City
4. Health Certificate         -   From City Health Office

FOR REGISTRATION/ RENEWAL OF MOTORIZEDTRICYCLE/MOTORELAS



REQUIREMENTS: (OPERATOR)

1.   Certification of Registration of the Motorcycle (CR) - Original and two (2) photocopies
2.   Official Receipt      -   From Tagbilaran City
3.   Barangay Clearance   -   From Tagbilaran City
4.   Community Tax   -   From Tagbilaran City
5.   Tax Clearance      -   From City Treasurer’s Office
6.   Picture (2X2)      -   recent (3 pieces)


IN CASE THE OPERATOR IS ALSO A DRIVER OF THE TRICYCLE ADDITIONAL REQUIREMENTS:

1.   Police Clearance
2.   Health Clearance

REQUIREMENTS (DRIVER: EXTRA OR REGULAR)

1. Barangay Clearance      -   From Tagbilaran City
2. Community Tax      -   From Tagbilaran City
3. Tax Clearance      -   From the City Treasurer’s Office
4. Health Clearance      -   From City Health Office
5. Police Clearance      -   From Tagbilaran City
6. Professional Driver’s License
7. Picture (2X2)         -   Recent (3pieces)

OTHER REQUIREMENTS:

C) License to Operate
D) Photo (2x2)         -    Two (2) pieces – of all operators, regular drivers and ‘extra’ drivers of can for hire (at the Integrated Bus Terminal or at Tagbilaran City Port)
