Science and Research / Re: What Is Dementia?
February 09, 2011, 04:53:21 AM
Treatment:

The goal of treatment is to control the symptoms of dementia. Treatment depends on the condition causing the dementia. Some people may need to stay in the hospital for a short time.

Stopping or changing medications that make confusion worse may improve brain function.

There is growing evidence that some kinds of mental exercises can help dementia.

Treating conditions that can lead to confusion often greatly improve mental functioning. Such conditions include:

    *

      Anemia
    *

      Decreased oxygen (hypoxia)
    *

      Depression
    *

      Heart failure
    *

      Infections
    *

      Nutritional disorders
    *

      Thyroid disorders

Medications may be needed to control behavior problems caused by a loss of judgement, increased impulsivity, and confusion. Possible medications include:

    *

      Antipsychotics (haloperidol, risperdal, olanzapine)
    *

      Mood stabilizers (fluoxetine, imipramine, citalopram)
    *

      Serotonin-affecting drugs (trazodone, buspirone)
    *

      Stimulants (methylphenidate)

Certain drugs may be used to slow the rate at which symptoms worsen. The benefit from these drugs is often small, and patients and their families may not always notice much of a change.

    *

Donepezil (Aricept), rivastigmine (Exelon), galantamine (Razadyne, formerly called Reminyl)
    *

      Memantine (Namenda)

A person's eyes and ears should be checked regularly. Hearing aids, glasses, or cataract surgery may be needed.

Psychotherapy or group therapy usually does not help because it may cause more confusion.

Science and Research / What Is Dementia?
February 09, 2011, 04:52:48 AM
Dementia is a loss of brain function that occurs with certain diseases. It affects memory, thinking, language, judgment, and behavior.

causes:
Most types of dementia are nonreversible (degenerative). Nonreversible means the changes in the brain that are causing the dementia cannot be stopped or turned back. Alzheimer's disease is the most common type of dementia.

Lewy body disease is a leading cause of dementia in elderly adults. People with this condition have abnormal protein structures in certain areas of the brain.

Dementia also can be due to many small strokes. This is called vascular dementia.

The following medical conditions also can lead to dementia:

    *

      Parkinson's disease
    *

      Multiple sclerosis
    *

      Huntington's disease
    *

      Pick's disease
    *

      Progressive supranuclear palsy
    *

Infections that can affect the brain, such as HIV/AIDS and Lyme disease

Some causes of dementia may be stopped or reversed if they are found soon enough, including:

    *

      Brain tumors
    *

Changes in blood sugar, sodium, and calcium levels (see: Dementia due to metabolic causes)
    *

      Low vitamin B12 levels
    *

      Normal pressure hydrocephalus
    *

Use of certain medications, including cimetadine and some cholesterol-lowering medications
    *

      Chronic alcohol abuse

Dementia usually occurs in older age. It is rare in people under age 60. The risk for dementia increases as a person gets older.

Symptoms

Dementia symptoms include difficulty with many areas of mental function, including:

    *

      Language
    *

      Memory
    *

      Perception
    *

      Emotional behavior or personality
    *

Cognitive skills (such as calculation, abstract thinking, or judgment)

Dementia usually first appears as forgetfulness.

Mild cognitive impairment is the stage between normal forgetfulness due to aging and the development of dementia. People with MCI have mild problems with thinking and memory that do not interfere with everyday activities. They are often aware of the forgetfulness. Not everyone with MCI develops dementia.

Symptoms of MCI include:

    *

      Forgetting recent events or conversations
    *

Difficulty performing more than one task at a time
    *

      Difficulty solving problems
    *

Taking longer to perform more difficult mental activities

The early symptoms of dementia can include:

    *

Language problems, such as trouble finding the name of familiar objects
    *

      Misplacing items
    *

      Getting lost on familiar routes
    *

Personality changes and loss of social skills
    *

Losing interest in things you previously enjoyed, flat mood
    *

Difficulty performing tasks that take some thought, but that used to come easily, such as balancing a checkbook, playing games (such as bridge), and learning new information or routines

As the dementia becomes worse, symptoms are more obvious and interfere with the ability to take care of yourself. The symptoms may include:

    *

      Forgetting details about current events
    *

Forgetting events in your own life history, losing awareness of who you are
    *

Change in sleep patterns, often waking up at night
    *

      More difficulty reading or writing
    *

Poor judgment and loss of ability to recognize danger
    *

Using the wrong word, not pronouncing words correctly, speaking in confusing sentences
    *

      Withdrawing from social contact
    *

Having hallucinations, arguments, striking out, and violent behavior
    *

      Having delusions, depression, agitation
    *

Difficulty doing basic tasks, such as preparing meals, choosing proper clothing, or driving

People with severe dementia can no longer:

    *

      Understand language
    *

      Recognize family members
    *

Perform basic activities of daily living, such as eating, dressing, and bathing

Other symptoms that may occur with dementia:

    *

      Incontinence
    *

      Swallowing problems

Signs and tests

Dementia can often be diagnosed with a history and physical exam by a skilled doctor or nurse. A health care provider will take a history, do a physical exam (including a neurological exam), and perform some tests of mental function called a mental status examination.

The health care provider may order tests to help determine whether other problems could be causing dementia or making it worse. These conditions include:

    *

      Thyroid disease
    *

      Vitamin deficiency
    *

      Brain tumor
    *

      Intoxication from medications
    *

      Chronic infection
    *

      Anemia
    *

      Severe depression

The following tests and procedures may be done:

    *

      B12 level
    *

      Blood ammonia levels
    *

      Blood chemistry (chem-20)
    *

      Blood gas analysis
    *

      Cerebrospinal fluid (CSF) analysis
    *

Drug or alcohol levels (toxicology screen)
    *

Tests for exposure to metals such as lead or arsenic
    *

      Electroencephalograph (EEG)
    *

      Glucose test
    *

      Head CT
    *

      Liver function tests
    *

      Mental status test
    *

      MRI of head
    *

      Serum calcium
    *

      Serum electrolytes
    *

      Thyroid function tests
    *

      Thyroid stimulating hormone level
    *

      Urinalysis

Science and Research / The Dangers of Loneliness
February 09, 2011, 02:54:55 AM
Psychologist John Cacioppo of the University of Chicago has been tracking the effects of loneliness. He performed a series of novel studies and reported that loneliness works in some surprising ways to compromise health.

   * Perhaps most astonishing, in a survey he conducted, doctors themselves confided that they provide better or more complete medical care to patients who have supportive families and are not socially isolated.
   * Living alone increases the risk of suicide for young and old alike.
   * Lonely individuals report higher levels of perceived stress even when exposed to the same stressors as non-lonely people, and even when they are relaxing.
   * The social interaction lonely people do have are not as positive as those of other people, hence the relationships they have do not buffer them from stress as relationships normally do.
   * Loneliness raises levels of circulating stress hormones and levels of blood pressure. It undermines regulation of the circulatory system so that the heart muscle works harder and the blood vessels are subject to damage by blood flow turbulence.
   * Loneliness destroys the quality and efficiency of sleep, so that it is less restorative, both physically and psychologically. They wake up more at night and spend less time in bed actually sleeping than do the nonlonely.

Loneliness, Cacioppo concludes, sets in motion a variety of "slowly unfolding pathophysiological processes." The net result is that the lonely experience higher levels of cumulative wear and tear.

In other words, we are built for social contact. There are seriousâ€"life-threateningâ€"consequences when we don't get enough. We can't stay on track mentally. And we are compromised physically. Social skills are crucial for your health.

Crabby Old Man

What do you see nurses? .......What do you see?
What are you thinking......when you're looking at me?
A crabby old man, ....not very wise,
Uncertain of habit ........with faraway eyes?

Who dribbles his food.......and makes no reply.
When you say in a loud voice....."I do wish you'd try!"
Who seems not to notice ....the things that you do.
And forever is losing .............. A sock or shoe?

Who, resisting or not...........lets you do as you will,
With bathing and feeding ...... The long day to fill?
Is that what you're thinking? Is that what you see?
Then open your eyes, nurse......you're not looking at me.

I'll tell you who I am ....... As I sit here so still,
As I do at your bidding, .....as I eat at your will.
I'm a small child of Ten......with a father and mother,
Brothers and sisters ......who love one another

A young boy of Sixteen ...........with wings on his feet
Dreaming that soon now. .........a lover he'll meet.
A groom soon at Twenty .........my heart gives a leap.
Remembering, the vows........that I promised to keep.

At Twenty-Five, now .......... I have young of my own.
Who need me to guide ...... And a secure happy home.
A man of Thirty ......... My young now grown fast,
Bound to each other ......... With ties that should last.

At Forty, my young sons ........have grown and are gone,
But my woman's beside me........to see ! I don't mourn.
At Fifty, once more, ......... Babies play 'round my knee,
Again, we know children ....... My loved one and me.

Dark days are upon me .......... My wife is now dead.
I look at the future ...........I shudder with dread.
For my young are all rearing .......young of their own.
And I think of the years...... And the love that I've known.

I'm now an old man.........and nature is cruel.
'Tis jest to make old age .......look like a fool.
The body, it crumbles..........grace and vigor, depart.
There is now a stone........where I once had a heart.

But inside this old carcass ...... A young guy still dwells,

Quite touching actually.

The story goes:

When an old man died in the geriatric ward of a small hospital near Tampa, Florida, it was believed that he had nothing left of any value.

Later, when the nurses were going through his meager possessions, they found this poem. Its quality and content so impressed the staff that copies were made and distributed to every nurse in the hospital.

One nurse took her copy to Missouri. The old man's sole bequest to posterity has since appeared in the Christmas edition of the News Magazine of the St. Louis Association for Mental Health. Note that, while this poem says “nurses,” we all â€" every one of us â€" must put ourselves into this spot.

During my brief stay in Cleveland, I attended a clinical internship on Podiatric Medicine at the Cleveland Clinic & Cleveland Foot Clinic under the auspices of the Ohio College of Podiatric Medicine, the city life was quite quaint and amiable. There was an abundance of parks, social night clubs, museums, cafes, and a plethora of shopping districts. I did like it there. The people in Cleveland are also quite open and helpful.  :)

as crowded as it may be, i love this place.

in my childhood, my parents and grandparents used to take us grandkids here. i remember the stories of how my lola washing clothes in the bottom of the springs below, near where the river and the ocean meet.

we used to bring the whole family here, and my grandfather would bring a freshly caught giant nokos, sea bass, and would sinugba it. while all of us grandkinds would swim and bathe, lolo would cook and prepare the food. ah, kalami kaayo.

the feel of this place is nostalgia to the extreme for me. there are so many familiar faces that come here. that is why so many visitors, people keep on coming back to badjiang. all the time.

i have been to Garcia Hernandez' Roxas Park. It cannot compare to badjiang.

It is obvious bitaw that Badjiang is becoming very popular destination spot for many travelers and tourists. When I visited Badjiang when I was there this summer, okay man ang pool, gi remodel nila ang steps, and put in new cementing for the pool's edge. Ang problema ani ay the pool's size, tho large to begin with, cannot accomodate the huge numbers of visitors daily.

When i was there, bai dave, i was talking to some of the workers and they told me that people from ubay, tubigon, loon, jagna, dimiao, tagbilaran, anda, people from cebu, negros, and alot of foreigners come to the pool everyday.

People love to come here because of the fresh cold spring water from the badjiang falls, which flows directly to the pool. Plus, beyond the pool and springs is the beautiful Beach view. In order to accommodate further visitors and to ensure satisfaction, staff and municipal government should consider the following initiatives:

1. increase the size of the pool
2. build another pool equal if not larger in size as the original

This is so beautiful. Of all the Churches in Bohol, the most beautiful in interior design is that in Baclayon.

I talked to the priest and grounds crew in that church during a visit and was told that the church in Baclayon is a favorite for marriage spots.

It has a very distinctly Baroque Spanish style.  :)

Historical note: Valencia was once known as Panangatan, after the Panangatan River. Historically speaking, Valencia was part of Dimiao before partition. Until, in the late 19th century when the people of Panangatan wrote to the Spanish gobernador and recollecta in Cebu to be granted independence from Dimiao. The recollect priest and government officials acquiesced to the request of the people of Panangatan and granted the area autonomy.

Panangantan was ultimately renamed to VALENCIA, the hometown of the recollect priest who came to survey the new town.

Since then, Valencia has surpassed / eclipsed its old mother town, Dimiao, in economic activity, wealth, and population size. Valencia's population (30,000+) is double that of Dimiao's (15,000+).

Not so bad, considering the population of Valencia is over 30,000. Roughly 10% of the town lives below poverty, but about 90% are above poverty levels.

Not bad at all considering the Poverty rate in Bohol Province, as a whole is 42.3% ,which is the highest in Region 7, higher than the national average rate of 37.5%.

Definitely the current administration in Valencia has more work to do, to reduce the poverty in our town. Below 5% would be ideal, definitely a standard the all the towns in Bohol should strive for. Progress forward, Valencia!

I'm old and I'm bitter, with nothing to fear
So I hope I offend you by bending your ear.
It's my one joy in life - you can like it or not
No answer in edgeways? I don't give a jot
I've railed and I've raved since my dotage began
It's my privilage 'cos I'm a grumpy old man

And as I grow older; it's life I suppose:
But more and more things just get right up my nose
Like young mums with their kids and their stupid wee dollies
Who chat; blocking the aisles with their damned shopping trolleys
I barge my way past; just as rough as I can
So the bitches will know I'm a grumpy old man

And those tear-arse young drivers who must overtake
Then go at speeds lower than I want to make
No tail-gating for me and I don't use my horn
But I heartily wish that they'd never been born
And I see hades open to eat car or van
That did it, because I'm a grumpy old man

And I hate all those worthies who avert their eyes
Because I've forgotten to zip up my flies
I excuse myself saying 'It's quite plain to see
' I'll have far less bother the next time I pee
'In the human race maybe I'm just 'Also ran'
'But I don't give a toss - I'm a gumpy old man'

There's food down my shirt-front and some in my beard
Cos I eat off my knee and I don't think it's weird
When I lounge in my armchair while watching the telly
And my food drips unheeded all over my belly
It's a trait of the aged from here to Japan;
A perk just for being a Grumpy old man

I go apoplectic to hear people say
'Awesome' and 'Wicked' and 'Have a nice day'
The poor English language is brought to it's knees
And falls prostrate and screaming with phrases like these
They should be wrapped up neatly and flushed down the pan
But nobody heeds me: a grumpy old man

At my death just cremate me; I'm hoping to save
Somebody the labour of digging my grave
Set my ashes in concrete and on the urn write
' Here's a rebel whose aim was to put the world right
' He failed but what better memorial than
'A farewell salute to a grumpy old man'

ah yes, homer's works were a particular favorite of mine. Iliad, for example, is one of my favorite ancient Greek writings.

a college professor, professor shapiro, regularly made reference to this--during his class on European Classics, which relegated some ancient greco-roman philosophies with that of the French Revolution.

how i loved that class...

Maayo bitaw ni, effective health care programs, and health educational disbursements are essential in preventative measures per se in the rising cases of pancreatic cancer, diabetes mellitus, and cardiovasculare-related diseases. In my visit to the hospitals (Gallares, Tagbilaran Community, and various Valencia Clinics), it was made known to me that these disease types are common / pragmatic in Bohol.

It would be observably beneficial if such knowledge of regular check ups (colonoscopies, rectal examinations, and general check up) are faciliated via leaflets and availability through municipal health agencies.

Good Job, Dimiao.

In all fairness lang ha, when a man has not visited his hometown for almost 12 years...and has not seen his family, relatives, old friends, and has the ability to make new friends in his town,..well the ecstatic feeling he may feel , a sense of elation and love is natural.

It is not a syndrome, nor should it be laughed at or ridiculed at, but should , as a feeling and as an experience, should be shared with to those who find similarity to such an experience; per se a man who visits Dimiao, or a woman who visits Ubay, Candijay , Balilihan or whatever town.

For myself, as one who was unable to visit Bohol, Philippines since 1998, this experience--to be able to be in Valencia for the limited time I was there..a mere 30 some days, was filled with joy and familial bonding.

How others translates his love and adoration for being home--is unfair, especially when it is negatively reiterated in such a way that distorts the very meaning and experience.

Dili pood maayo da. Insensitive pood.

Thanks.

Dimiao - Bohol / Re: Pictures of Dimiao, Bohol
February 06, 2011, 12:53:57 PM
Dimiao , as a town, its rich history, its colloquialisms, and its people, holds a special place in my heart. As a child, during my grandfather's truck routes, we would pass by Dimiao on our way towards Loay, Lila and Baclayon, and Tagbilaran. I remember a time when I was but a little child, my lolo would sometimes stop by the mercado in Dimiao to purchase some hot Dimiao-non Pandesal , with a cold Coca Cola bottle. The two of us would sit on the stools by the bakery shops in the mercado, chewing the hot pandesal, while sipping on ice-cold coca cola. The kind where the soda's effervescent  gaseous emulsion flares on one's nostrils. Refreshing, truly.

Dimiao is a special town for me. It is the sister town of Valencia, and the home town of my lolo's grandmother. There are Tadlips in Dimiao, of whom my great-great-grandmother (my grandfather's grandmother) belonged to; she was a Dimiao-non.

Yawan-on Nga Buhat

LUCENA CITY, Quezon, Philippines—Some of the injured passengers of a Manila-bound bus from Bicol that plunged into a ravine in Pagbilao town complained of losing their belongings to thieves.

Jocelyn de Leon recalled that when she regained composure she saw that a man who rushed to the accident site was opening her bag.

“I immediately grabbed my bag. We’re all needing help but some of them wanted to steal our belongings,” she said. She suffered a cut in the forehead.

Mga way atay ug batikon.

It all depends on one's personal relationship and faith in Jesus Christ as Savior and Redeemer. In the end, Salvation and Damnation--the decision itself, depends on The King of Kings, The Lord of Lords.

Bible Study / Re: The Judgment of the Dead
February 06, 2011, 11:58:16 AM
31When the Son of Man comes in his glory with all of his angels, he will sit on his royal throne. 32The people of all nations will be brought before him, and he will separate them, as shepherds separate their sheep from their goats.

33He will place the sheep on his right and the goats on his left. 34Then the king will say to those on his right, "My father has blessed you! Come and receive the kingdom that was prepared for you before the world was created. 35When I was hungry, you gave me something to eat, and when I was thirsty, you gave me something to drink. When I was a stranger, you welcomed me, 36and when I was naked, you gave me clothes to wear. When I was sick, you took care of me, and when I was in jail, you visited me."

37Then the ones who pleased the Lord will ask, "When did we give you something to eat or drink? 38When did we welcome you as a stranger or give you clothes to wear 39or visit you while you were sick or in jail?"

40The king will answer, "Whenever you did it for any of my people, no matter how unimportant they seemed, you did it for me."

41Then the king will say to those on his left, "Get away from me! You are under God's curse. Go into the everlasting fire prepared for the devil and his angels! 42I was hungry, but you did not give me anything to eat, and I was thirsty, but you did not give me anything to drink. 43I was a stranger, but you did not welcome me, and I was naked, but you did not give me any clothes to wear. I was sick and in jail, but you did not take care of me."

44Then the people will ask, "Lord, when did we fail to help you when you were hungry or thirsty or a stranger or naked or sick or in jail?"

45The king will say to them, "Whenever you failed to help any of my people, no matter how unimportant they seemed, you failed to do it for me."

46Then Jesus said, "Those people will be punished forever. But the ones who pleased God will have eternal life."

Kaliwat Bol-anon / Re: Lucino Family of Tagbilaran, Bohol
February 05, 2011, 02:59:53 PM
Discovering The Lucino Roots of Panaon Island, Southern Leyte.

The story begins in Leyte. The Lucino Dynasty does not hail originally from Bohol, but are actually native to the lush green of Leyte Province. During my self-awaking experience in the summer of 2010, I was able to visit , for the first time in my life, the native land of my paternal grandmother and paternal grandfather. Both of my father's parents were from Southern Leyte: Padre Burgos and Pintuyan. The trip to Southern Leyte was coordinated via permission from my father and my aunt (both of whom are in the USA) as well as with the help of my Auntie Wella, Uncles Roger and Ray, cousin Jing and a family member, Aurora.

The journey to visit Southern Leyte required our travel by barge from the port in Ubay, which took us to the port in Maasin, Southern Leyte. From Maasin, we drove our Isuzu Crosswind through the towns of Sogod, Bato, Padre Burgos and then ultimately to Panaon Island, which took us directly to the home region of my late grandfather, Anhing Papa Titing.

That night , after arriving to Maasin, my aunts and uncles coordinated a family reunion with the Ebuenga Family, the family that my paternal grandmother came from. For the first time in my life, I was able to meet my grand uncles, distant aunts and uncles, and cousins. All of whom shared with me the memories, beautiful and tearful, of my late grandmother and my late grandfather, of my father's childhood and the experiences that they had regarding the old days. The eyes of my grand uncle, my grandmother's younger brother, kept on focusing at me during the dinner table. He reiterated time and time again, "Rico ka kaayo! Dong, Rico kaayo!" Apparently I bore strict resemblance to my father, whose nickname is "RICO".

The next day, we woke up early in the morning and drove to Panaon Island. The ancestral land of the Lucino Dynasty. The major town in Panaon Island is Panaon, and Pintuyan Town. There is a barangay in Pintuyan Town called "Catbawan". This town is populated with Lucinos, hundreds of my fellow clan mates. Then there is also another family there whom I have blood relation, the Estrella clan.

I felt so much at home in Pintuyan. Tho I never visited the place before, it felt as if i was there before, as if i walked the streets, smelled the fires at night, and saw the sunrises and sunsets here hundreds of times. There was a strong, powerful sense of 'belonging' there. That following day I also met with the local town officials in Pintuyan. The current mayor of the town is an Estrella, a relative of ours. The previous mayor was Mayor Lucino, and before him, was Mayor Cipriano Lucino (my grandfather's 1st degree cousin). I was able to see the newly built hospital , the newly restored St. Michael  Church, the rebuilt (and enlarged) Pintuyan Elementary and High School, and the growing docks. All of which were infrastructural projects by my Lolo Lolong (Mayor Cipriano Lucino). I was impressed.

After all was said and done, I made my way, with assistance from my Tita Magda, the daughter of my late Lolo Piles (Felix Lucino, the older brother of my grandfather), to visit the cemetary in Pintuyan. There I was able to visit the grave of my great-grandfather, the late Mr. Bernardo Lucino. Ning bisita intawn ijang Apo sa Sungay nija. :)
As Tita Magda assisted me in prayer and in lighting the candles for his memory, a strong gush of cool wind blew and stroked my face. Ever so softly.

Tita Magda said to me, "Na lipay imuhang lolo nga ning bisita ka nija, Dong. Lipay to sija nga wa siya kalimtan sa iyang apo sa sungay..."
She was crying when she said this to me. And tears came down my eyes...as well.

It was hard for me to leave Leyte in that trip; as I so wanted to stay longer. Someday, God-willing, I will be able to visit and explore Leyte further.

The pictures I will post are of Panaon Island and Liloan Bridge. This was taken as I was leaving to go back to Bohol. Early in the morning.

This picture was taken during my visit to Bohol, Philippines this July 2010. A picture of my late grandfather, Mr. Rustico Aray Estrella Lucino.
From what my Uncle Ray and Uncle Roger told me, Papa Titing was 25 when this picture was taken. About my age.

Papa Titing was not a Bol-anon, but 100% Leyteno. He did migrate to Bohol by way of Cebu and eventually made Bohol his home.

Businessman Mr. Rustico Aray Estrella de Lucino. Originally from Barangay Catbawan of Pintuyan, Southern Leyte.
