“You have heard that it was said, ‘Love your neighbor and hate your enemy.’ But I tell you: Love your enemies and pray for those who persecute you, that you may be sons of your Father in heaven. He causes his sun to rise on the evil and the good, and sends rain on the righteous and the unrighteous."

â€" Matthew 5:43-45

===

Jesus was stretching the imagination of the Jewish listeners. To love your enemies is to acknowledge that God loves them too. This is another way of saying that there is hope for anyone. No matter how evil or unrighteous someone may seem, the sun still shines upon them. We are challenged to live in the love of our heavenly Father, who wishes for none to perish and all to come to repentance.

How, then, can they call on the one they have not believed in? And how can they believe in the one of whom they have not heard? And how can they hear without someone preaching to them? And how can they preach unless they are sent? As it is written, “How beautiful are the feet of those who bring good news!”

â€" Romans 10:14-15

===

God has given us His creation to display His glory, but that is not enough â€" there needs to be an explanation of that creation. Paul is telling us preaching is the means of grace that helps us toward belief in the redemption of creation. Unless we are willing to tell others about Jesus, we are not giving them the chance to believe. We have good news that needs to be lived AND spoken to those around us. Let God send you today.

Poems and Poets / JM's God

JM’s God.
By Dr. Glenn Bubley, MD

Entering JM’s room I find him where I always do,
Pouring over his large print Bible as if its all brand new.
Before I can ask him how is today,
He looks me in the eye and asks if I’m ok. I marvel at how important my answer is to him
As he lies on his hospital bed, tubes in every limb.
If there is an ultimate justice of genuine worth,
Surely JM and his kind will inherit the earth.

In JM’s time he was a victim of segregation,
With no chance to rise above his station.
He working loading boxes with his back and hands,
Acceding for years to his bosses demands.
And now even with cancer he’s neither bitter nor angry,
His struggle comported with the utmost dignity.
This man of sorrow, acquainted with grief,
Only by his release will he find ultimate relief.

Although there may be a balm in Gilead,
On this ward, IV morphine is the best we have.
Although the pain of cancer gnaws at his bones
He’s apt to laugh more often than groan.
His smiles leave lines etched on his old black face
Reflecting an inner peace that cancer can’t erase.
His strength seems to be emanate from a glimpse of God,
A glance that may be as close as men are ever allowed.
Now hobbling through the valley of the shadow without moorings,
Will he soon “mount up on the wings of eagles” up soaring?

As for me, I have much more than my daily bread.
So I wonder what it is about the future I dread.
If I could embrace the mystery of his faith might I break free,
If I could quash my doubts could JM’s God touch me?
If I could genuinely consider the lilies of the field,
Might I find a faith that feels strong and real?
Then would vain-glory and self promotion,
Fall away in favor of more genuine emotions?
Would everything change if I could begin to ponder
If it really possible that He walked on water?
Did Moses really hear the Lord from the bush?
Does JM’s bible verses contain seeds of truth? Can a book learning cynic be taught by an unschooled man,
That some things are controlled by an unseen hand?

I had thought that JM’s God was only a useful superstition,
But his life bears witness that the Holy Ghost’s not just an apparition.
If he’s found a lasting faith and true hope that abides,
Then nothing else matters on this mortal side.
His pain and the cancer is just a brief bother,
Before he finally stands before his Father.
And on that last journey that we all take alone,
Will a redeemed JM stand before the throne?
And will he hear these words now that his race is run,
“My good and faithful servant, well done, well done.
Walk up right into this new Jerusalem, it’s not a dream
Here justice pours down like water, righteous a never-failing stream”

Sharing My Body

By Janet Greene, RN

Before I knew it,
I was sharing my body with a stranger.
No love binds us, but my whispers go unheard.
I cannot get away.

A cancer has crept in.
Motionless, I dance with this intruder.
My feet are numb.
I struggle for balance.

I am sharing my flesh with a partner
Who touches private parts in me
And leaves me nothing.

Ownership
By Matthew Hitron, MD

During my third year of medical school one of my patients died. It was a medical error that killed her, and I have been told to feel that the responsibility for it is shared, as a system and as a team. I didn’t feel that way then, and I don’t feel that way now. She died not only because of a screw up, but because no one really cared. I live with the fact that I could have stopped it. She needed someone to care, and I failed her in that. She was an elderly woman with many problems and a dementia that made her difficult to interview and examine. She was uncooperative and at times combative. She had no family, with a court appointed legal guardian and HCP. She arrived from her nursing home with mucus stains on her face and two necrotic toes. She swore at you, and even spat at you when you tried to speak with her. There are ways to break down the story into its components, and analyze the systems that failed; the cracks that she fell through. But it is smoke clouding the picture of a patient who was going to challenge all her care givers by requiring of them a true and exhausting commitment to her humanity; a humanity that was easily forgotten after a few seconds in the room. She was passed off from person to person, service to service, consult to consult; and I was complicit in this.

It was two weeks into my third year medicine clerkship. At times it felt like a show and I was painfully aware of the need to impress. I was not about to back down from any task, and was constantly negotiating the third year paradox of needing to learn everything while hiding the fact that you know nothing. My resident warned me she would be tough, but I had to prove I was good. She was admitted that night uneventfully, numerous consults were called, and the day was over.

The next day she was the last patient on rounds, kept NPO and on maintenance fluids while she sat in her room like that was the curative measure. Nothing happened. The team would wait for the consults to do something and the consults would wait for the team to tell them what to do. The removal of her dead toes was at least some sort of plan, but her strangely elevated INR, her ominous acidosis, and her altered mental status were just glossed over before lunch, with vitamin K, bicarb, and olanzapine given to make everybody feel better.

I came in the next morning, and as I flubbed my way through the note jotting down a K+ value of 2.0, it never occurred to me to be sure someone else knew about it. I remember thinking “wow; that sure is low…” if it were a test question I would have gotten it right. With all the nurses, consultants, and residents milling around her, someone must have seen it too and acted with purpose. No one did that morning. Instead of taking ownership, I was just another in a long line who passed the responsibility off, with no one left after me to pick it up.

Late that afternoon potassium was finally hung on her IV. The patient was alive, and I had dodged a bullet. I walked to the stairwell, stopped on a landing and leaned against the wall, “Take ownership” I said to myself, “you may be the only one left.” I remember feeling like I learned a huge lesson without a patient having to pay with her life.

I also remember moments later, and will never forget; the panic, the disbelief, and the grim realization that a patient had fallen through the cracks to her death, while I ran up the stairs to the sounds of the code alarm.

Science and Research / Re: What is Dementia?
March 08, 2011, 08:21:51 AM
What Kind of Care Does a Person with Dementia Need?

People with moderate and advanced dementia typically need round-the-clock care and supervision to prevent them from harming themselves or others. They also may need assistance with daily activities such as eating, bathing, and dressing. Meeting these needs takes patience, understanding, and careful thought by the person's caregivers.

A typical home environment can present many dangers and obstacles to a person with dementia, but simple changes can overcome many of these problems. For example, sharp knives, dangerous chemicals, tools, and other hazards should be removed or locked away. Other safety measures include installing bed and bathroom safety rails, removing locks from bedroom and bathroom doors, and lowering the hot water temperature to 120°F (48. 9°C) or less to reduce the risk of accidental scalding. People with dementia also should wear some form of identification at all times in case they wander away or become lost. Caregivers can help prevent unsupervised wandering by adding locks or alarms to outside doors.

People with dementia often develop behavior problems because of frustration with specific situations. Understanding and modifying or preventing the situations that trigger these behaviors may help to make life more pleasant for the person with dementia as well as his or her caregivers. For instance, the person may be confused or frustrated by the level of activity or noise in the surrounding environment. Reducing unnecessary activity and noise (such as limiting the number of visitors and turning off the television when it's not in use) may make it easier for the person to understand requests and perform simple tasks. Confusion also may be reduced by simplifying home decorations, removing clutter, keeping familiar objects nearby, and following a predictable routine throughout the day. Calendars and clocks also may help patients orient themselves.

People with dementia should be encouraged to continue their normal leisure activities as long as they are safe and do not cause frustration. Activities such as crafts, games, and music can provide important mental stimulation and improve mood. Some studies have suggested that participating in exercise and intellectually stimulating activities may slow the decline of cognitive function in some people.

Many studies have found that driving is unsafe for people with dementia. They often get lost and they may have problems remembering or following rules of the road. They also may have difficulty processing information quickly and dealing with unexpected circumstances. Even a second of confusion while driving can lead to an accident. Driving with impaired cognitive functions can also endanger others. Some experts have suggested that regular screening for changes in cognition might help to reduce the number of driving accidents among elderly people, and some states now require that doctors report people with AD to their state motor vehicle department. However, in many cases, it is up to the person's family and friends to ensure that the person does not drive.

The emotional and physical burden of caring for someone with dementia can be overwhelming. Support groups can often help caregivers deal with these demands and they can also offer helpful information about the disease and its treatment. It is important that caregivers occasionally have time off from round-the-clock nursing demands. Some communities provide respite facilities or adult day care centers that will care for dementia patients for a period of time, giving the primary caregivers a break. Eventually, many patients with dementia require the services of a full-time nursing home.

What Are the Different Kinds of Dementia?

Dementing disorders can be classified many different ways. These classification schemes attempt to group disorders that have particular features in common, such as whether they are progressive or what parts of the brain are affected. Some frequently used classifications include the following:

    1. Cortical dementia: dementia where the brain damage primarily affects the brain's cortex, or outer layer. Cortical dementias tend to cause problems with memory, language, thinking, and social behavior.
    2. Subcortical dementia: dementia that affects parts of the brain below the cortex. Subcortical dementia tends to cause changes in emotions and movement in addition to problems with memory.
    3. Progressive dementia: dementia that gets worse over time, gradually interfering with more and more cognitive abilities.
    4. Primary dementia: dementia such as AD that does not result from any other disease.
    5. Secondary dementia: dementia that occurs as a result of a physical disease or injury.

Science and Research / What is Dementia?
March 08, 2011, 08:19:31 AM

Dementia describes a group of symptoms that are caused by changes in brain function. Dementia symptoms may include asking the same questions repeatedly; becoming lost in familiar places; being unable to follow directions; getting disoriented about time, people, and places; and neglecting personal safety, hygiene, and nutrition. People with dementia lose their abilities at different rates.

Dementia is caused by many conditions. Some conditions that cause dementia can be reversed, and others cannot. The two most common forms of dementia in older people are Alzheimer’s disease and multi-infarct dementia (sometimes called vascular dementia). These types of dementia are irreversible, which means they cannot be cured. (Symptoms associated with dementia can often be treated, however.)

Some conditions may mimic dementia, but are actually reversible conditions. Reversible conditions with symptoms of dementia can be caused by a high fever, dehydration, vitamin deficiency and poor nutrition, bad reactions to medicines, problems with the thyroid gland, or a minor head injury. Medical conditions like these can be serious and should be treated by a doctor as soon as possible.

Sometimes older people have emotional problems that can be mistaken for dementia. Feeling sad, lonely, worried, or bored may be more common for older people facing retirement or coping with the death of a spouse, relative, or friend. Adapting to these changes leaves some people feeling confused or forgetful. Emotional problems can be eased by supportive friends and family, or by professional help from a doctor or therapist.

Science and Research / Re: Schizophrenia
March 08, 2011, 08:17:12 AM
WHAT CAUSES SCHIZOPHRENIA?

There is no known single cause of schizophrenia. Many diseases, such as heart disease, result from an interplay of genetic, behavioral, and other factors; and this may be the case for schizophrenia as well. Scientists do not yet understand all of the factors necessary to produce schizophrenia, but all the tools of modern biomedical research are being used to search for genes, critical moments in brain development, and other factors that may lead to the illness.

Is Schizophrenia Inherited?

It has long been known that schizophrenia runs in families. People who have a close relative with schizophrenia are more likely to develop the disorder than are people who have no relatives with the illness. For example, a monozygotic (identical) twin of a person with schizophrenia has the highest risk â€" 40 to 50 percent â€" of developing the illness. A child whose parent has schizophrenia has about a 10 percent chance. By comparison, the risk of schizophrenia in the general population is about 1 percent.

Scientists are studying genetic factors in schizophrenia. It appears likely that multiple genes are involved in creating a predisposition to develop the disorder. In addition, factors such as prenatal difficulties like intrauterine starvation or viral infections, perinatal complications, and various nonspecific stressors, seem to influence the development of schizophrenia. However, it is not yet understood how the genetic predisposition is transmitted, and it cannot yet be accurately predicted whether a given person will or will not develop the disorder.

Several regions of the human genome are being investigated to identify genes that may confer susceptibility for schizophrenia. The strongest evidence to date leads to chromosomes 13 and 6 but remains unconfirmed. Identification of specific genes involved in the development of schizophrenia will provide important clues into what goes wrong in the brain to produce and sustain the illness and will guide the development of new and better treatments. To learn more about the genetic basis for schizophrenia, the NIMH has established a Schizophrenia Genetics Initiative (see Web site at http://www-grb.nimh.nih.gov/gi.html) that is gathering data from a large number of families of people with the illness.

There are various types of Schizophrenia, but Paranoid Schizophrenia is the most common. The patient has delusions that people are deliberately trying to persecute her, she may well consider herself of exalted birth, or maybe that she's been sent on a special mission by the Government, although she's not always sure what that mission might be.

Jealousy is another unpleasant symptom, together with hearing voices that are either of a threatening tone, or which threaten her directly. Sometimes she smells and tastes things which aren't real.

The onset of this disease is usually between the ages of 15 and 35, and while there's no cure, it can be controlled by conventional medications such as Thorazine, Prolixin, Haldol and Stelazine. These drugs became available in the 1950s.

In the last decade, more advanced medications appeared, such as Abilify, Zyprexa, Seroquel and Geodon. However, doctors recommend that if you've been placed on a course of the older drugs, you should stick to them and not try to change.

http://ezinearticles.com/?Paranoid-Schizophrenia,-the-Krays,-and-Freuds-Confusion&id=3625466

correction: kining older drugs such as haldol (haloperidol) are not being prescribed anymore because of the negative side effects.

i have patients who were diagnosed with manic depression / schizotypal disorders and we were consulted to ween them off the older typical anti-depressants (such as haldol/haloperidol). these older drugs (typical antidepressants) can cause EPSE (extra pyramidal side effects) such as tarditive dyskinesia.

here in chicago hospital systems, we give psychotic patients atypical anti-depressants; such as fluoxetine, and aripiprazole (also known as abilify). sometimes we also give them anxiolytics such as carbomezapene and diazepam.

i have to understand the side of mr. Lorenzo, its like the story of the blind men who assess the elephant. Becuase Bay Lorenz can only give what he observed then lets respect that. However, as TAGA BUKID, in my almost 30 years of existence, I can personally say that the REAL VALENCIA is hidden beneath the beautiful facade of the town with its newly constructed buildings. The network of roads are still poor, poor performance of students in Provincial, regional and national examinations, mahal ang taxes ug mamaligya sa merkado and etc. These are the problems nga angay unta tagaag atensyon.

Bai Jun, salamat sa imohang pagka open mindedness sa other people's point of view on things. Para nako ug sa ubang taw nga wa kahibalo sa situwasyon (ang day to day living sa local residents dinha sa Valencia, especially ang mga residente nga below poverty rate). In my view lang , i did not see destitute people during my many walks around Sawang, Canmanico, etc. You're rigt, Jun, that those who visit Valencia are unaware of the destitute , and I will not deny that there are destitute, just voicing that in my view lang, i did not see destitution, but rather a signs of progress in Valencia.

I sympathize with you and others who are clamoring for the recognition of the current tax rates in the town, as well as the lack of sports development as you said earlier. If this is a case , perhaps you and other concerned citizens can bring this up to the current Mayor during meetings. Perhaps, concerned residents can voice this concern to their elected barangay captains and then have them bring this up to the mayor.

Thank you, Bai Jun for sharing your point of view, as well as seeing my point of view as well.

I lived in Jagna from age 3 to 7 while my mom finished her AB English at the Colegio de la Medalla Milagrosa. My earliest memory of childhood is of Jagna--and the thoughts that it brings is always warm, happy, idyllic. Two years ago, I brought my three kids there on our way back from CDO. I retraced the paths I've trodden as a child and was surprised that 40 years later, I could still remember the layout and the landmarks of the town. I was even more astonished to realize that what were etched so huge and so far in my memory were actually just small proportions of my imagined sizes and distances. At one point, my kids asked in bewilderment, "What's in this place that you're misty-eyed, Dad?"

Hehehe the fond memories of a man's childhood. I'm sure your children will always remember the town as the town that their father is so fond of. :)

Did you also visit my hometown of Valencia, Tito Ben?
