My mom was born in Bohol and her family move in Butuan since she was little she was born in 1956  but she don't have a record in NSO office. What did she do? She live in Butuan now.

Second her married lisence is spelled wrong in Middle name. Spelled (Bagenti) instead of Bahenting. Is that would be a problem when she get a passport?

Fr Chicogon is right, the whole process will take almost a year and some amount. Please ask some advice from your local registrar´s office.
