I have chronic lower back pains. What to do?

I have back pains 24 hours/7 days a week. Any suggestions on how to ease pain?  :(

So do I, Melrose.  I was diagnosed of myalgia of the upper trapezius more than 10 years ago unya karon pati naman lower back nako.  I was in theraphy for many months until gihaguan nako sige'g therapy.  It's already a chronic pain so I learned to ignore na the pain but when I am so stressed, di jud ma ignore.   Ang gi ingon sa doctor, di daw pahilot but wa ko namati kay ma relieve man jud kon mahilot.

Let's see what Doc Dodong Bran can say about our condition.  :)

Mao ba gracie? I've been to different doctors, wa may makahatag nako og diagnosis uy. Ang ako lang, mahinganlan ang akong sakit sakit. One time a doctor said, basin scoliosis, another finding pud says straightening of the lumbar. Worse, giingnan ko sa usa ka doctor basin kuno Bamboo Spine. Gahilak ang beauty day. Maayo gani kay dili diay.

Basta sakit kaayo kung mag-spasm, dili ko kalihok, dili ko ka lakaw. Ambot ani. Sometimes, I feel so down. Magsige ra kog hilak.

I wanted to take a positive outlook. Dili na lang nako tagdon ang pain but sometimes, dili gyud madala og ignore.

Sige ra ko pahilot intawn. Pero until when?

Sige ko basa books about chronic pain, pero wala pa ko kabalo asa ko moduol, kinsa nga doctor diri sa Pilipinas. Maayo tingali sa gawas kay naay daghang doctor.  :(

Vids, sa CDO pa man ko adto. I had two doctors di lang ko ka remember unsa jud to tawag sa ilang specialization.  If I am not mistaken, basta mura'g osteopathic ug orthopedic to sila.  Mao daw to'y sakit sa accountant kanang dumot mag trabaho busa naka form na ang muscle like kong whole day ka gadungo-dungo ga balansi ug usa ka centavo.  Ang ingon sa doctor nako, dapat daw di ta permanente in one sitting position.  From time to time, mag stretch jud daw ta.  As to my pain, adto na time, gi inject gani akong liog dapit  ug unsa ba kaha to basta since then hadlok na ko kaayo kay tiaw gaka imagine nako duol kaayo sa akong spine kon mamali sila tuplok sa injection, nah unsaon na lang!  Sa akong therapy, aguy butangan ug warm bag na mura'g sand ang both abaga nako dayon gi traction akong liog plus kadtong ilang exercise and the ultra sound (daw gi plantsa with some sticky gel).

Ma feel ba nimo Vids na kon sakit na jud kaayo mura'g kasukaon ka?  Ana ako unya sometimes feverish pa.  Maayo man to naa pa ko sa Manila kay barato ra pamasahe sa mga buta sa Rotonda sa QC, dire tawon ordinary lang na mangmasahe, pinaka minus kay 60 dollars.

diri sa bohol, barato ra massage sa mga Spa. Kaya pa nako twice a week. Nagtherapy pod ko sa una  pero wa koy time kay office hours is up to 5pm, unya ang clinic 5:30 pod magsira. dili ko kaabot. naa pod koy pain killer sauna, girl. pero makadaot man sige inom tambal.

pareha ta girl, sakit ako lawas. mura kog hilantan. feeling nako naka-augment ang hormone imbalance sa pain. sa ako readings, sakit sa lawas baya pod nang naay hormone imbalance. pero lagi, inag adto nako sa doctor, ingnon ra man ko, "Bataa pa man ka, imong sakit, sakit sa tiguwang!"

I started having this pain Vids when I was still in my mid 20s.  One time pa gani didto koy assignment sa Cebu adto unya kay sugmat man jud as in sakit akong bat-ang, pag adto nako sa Chong Hua kay naa daw koy "slip disk" and I needed surgery asap.  Unsa ka asap pod ko ning layas sa doctor's office.  Tiaw butangan daw ug steel akong likod.  No way, Jose!  Well, usa sa ingon sa doctor na maka trigger kining tag-as ug heels akong shoes.  Well, I'll deal with the pain. hahahaha

Naa bay chiropractor diha sa ato, Vids?  Or basin naa Cebu kay sure ko naa sa CDO.  Pag try daw pa treat sa ila.

I felt better dear sa NUGABEST. kanang mechanical chiropractor nga bed. nakasuway ka ana? Naa siyay heat. product na sa Korea. Daghan na dri sa Tagbilaran. Libre ang mopila. Naa poy VIP room, bayad ka P50.00 per sesssion. Barato ra. Good thing kay naa pod na didto atbang sa St. Luke's Hospital, dool sa among new place.

Pero mao gihapon, sakit gud gihapon.

Sometimes, feeling gyud nako hopeless case ko.

Naa koy similar ing-ana, Vids.  It's like a huge pad that can cover my whole body.  Ako nang ibutang sa either chair or sofa.  Ganahan ko butang sa sofa aron mag higda lang dayon ko.  Mag massage ang legs, lower back, upper back with matching heating pad.  Pwede pod lower back or upper back lang ang i massage without heat pero mas chada naay heat.

Dili na hopeless imong case oi.  Unya mabasahan ni ni Doc Dodong Bran am sure he has better suggestions what to do.  Basta for sure, I know what you are feeling.  I thought abnormal ko for having this pain.  Pero one thing di jud ko mo take ug pain reliever kay di ko gusto ma anad ko.  Tylenol ra jud akong ma take kon di na kaya.

Hi Mel,

Kindly asking lang ko, how long have you had this back pain? Recently ni or has it been for a good while now?

Where are you having pain, what part of the back?
Is it in the upper posterior? Shoulder-scapular area? Spinal area? lower lumbar? Side?

Mel, there are alot of spinal nerves and nerve roots that travel through our spinal cord, and each of these nerves will connect to a specific target tissue (muscle, gland,) because this provides effector signals (from brain to tissue) as well as they are routes for affector pathways (from tissue/muscle to brain).

Back pain can be caused by a weakening of back muscles; trapezius, latissimus dorsi muscles, and or neuritis, which is an inflamation of a spinal nerve. Sometimes, the pain can also be due to improper movement/prolapse of the spinal disc.

Mel,

if this pain has been recently, (for last week or just recently), I suggest you do back exercises to strengthen your back, and don't apply too much pressure, or force on your back. Avoid strong massages if it hurts. As it could indicate nerve damage.

If this pain has been here for over 3 months, Mel, this is already called Chronic back pain.
If your family physician cannot diagnose properly, please kindly have him/her refer you to a orthopaedic specialist so they can make the proper diagnosis/prognosis.

Hi Ate Ging,

kining myalgia of the trapezius can be caused by either:
1) nerve receptor hyperactivity (nociceptor hyperactivity: pain receptors ni sila)
2) Stress
3) Pathology in the accessory and cervical nerves.

When a person is stressed, it heightens receptors for pain, Ate Ging.
In these cases, of chronic pain, one can strengthen their dorsal muscles by exercising them. And if it is really severe, a physician will prescribe pain medication, but abuse of that can and will lead to dependency.

If hiluting helps relieve the pain, then continue with it, as massaging does help vasodilation and blood flow to the tissue. But if the pain is burning and aching and really unconditional, please ask to see a back pain specialist.

Because in the lower back, ate ging, chronic pain in that region is also symptomatic for endocrinal pathology. Kidney, colonic, and liver abnormality. We are told that even asymptomatic patients should have their levels checked at least once a year to make sure. Consant pain on the side of the lumbar area, also is symptomatic of bile cyrstallization, 'te. This manifests in gall stones. And it does lead to lower lateral back pain.


Always see the doctor, once a month jud. Bisan way symptoms, always ask for check up (complete). Better prevention that treatment of an ailment.

Hi, bran! i think i need to tell you this. nobody understands me. people think i just have low pain tolerance. but no! they don't know what i'm going through. when i feel pain, i get so depressed.

Maybe you can help me.

The doctor's had different opinions on my condition.

One said I might have juvenile arthritis. But she just laughed at me saying that arthritis are for old people. I felt so down since I was in pain and the doctor was just laughing at me.

Then I went to a physical therapist. He said, I have muscle spasms caused by mild scoliosis. So I had physical therapy for a while. I wasn't relieved. Years after, the same doctor said, it's not scoliosis but just pain caused by bad positioning so he gave me a corset, some sort of a back brace. It didn't work.

I went to a mayotherapist in Manila. He just gave me one major "realignment" of my bones by stretching my body.  I felt a little better. But after several months, I had major pain all over again.

Then, I went to  see an ortho in Cebu. He was thinking that I might have ankylosing spondylitis. But it turned out wrong. No further consultation followed since I was out of the country for some time.

I had acupuncture. I have body massage. I visit NugaBest, that mechanical chiropractor bed with heat. None so far have truly helped me ease my pain.

Then I had several x-rays. One film showed a broken tail-bone. The doctor said, it's not alarming. But I thought otherwise. But since he's the doctor, I said nothing. As I read through several articles on pain, some say old fractures can still cause pain although it has healed over time. As I understand it, the brain may still send signals telling the body that it is painful. I don't know anything further than that. 

I feel pain all over- from my neck down to my legs and ankles. But I think it starts somewhere on my lower back/hips. I first experienced pain in 1997 on my lower back with a numbing, tingling, throbbing pain. I feel my right hip (somewhere on the pelvis) locks when I don't sit/lay down properly. I become stiff and immovable when it happens. Now, I feel my neck locks, too.

Now, it develops into some other pain. I have tender points on my inner legs and thighs, even going up to my rib cages. Sometimes, I feel pain on my arms. The throbbing pain is now going up to my upper back, my shoulders and neck causing me headaches.

Aside from the pain, I feel tired all the time. I was also checked for thyroid problems. Nothing is wrong.

 I did my own research and I came across fybromylgia. I looked into the checklist of symptoms. I found out that most of my symptoms are there such as pain, fatigue, sleep disturbance, morning stiffness, numbness or tingling of the extremities (specially on my fingertips), painful menstrual periods, and many others.

There's no known cause for fybro but some articles say old injuries can cause pain. hmmm? It rings a bell...

My research is not thorough, of course. I came across several other syndromes similar to fybro. Some articles also link hormone imbalance to fybro. I think have hormone imbalance since I don't get to have regular periods.

I need a doctor who understands things like these. What kind of doctor ba dapat ang istoryahon?

Just reading your history, it seems that you are experiencing what we call neuropathic and nocicpetive pain.

It is interesting that you mentioned that it lapses and comes. I hve a friend, who is also a medical student as I, he complained of back pain as well; at first when he explained to me his symptoms, it rings back to what you are having. I thought he was just hypochondriac, but one time when we were walking, he was screaming because of his back pain. I had him go to the hospital to have a osteo consultation, wala man kono tissue damage.

This winter break, he went back and had a consultation, still wala. Then he went for acupuncture and says he hasn't had it. I don't know.

The kind of pain you are describing is a globalized neuropathic and nociceptive. This indicates damage to the nerves, since you are feeling it in your lower extremities, and the fact that you have felt this since 1997 is alarming.

Please ask for a referral to a back pain specialist; to consider non-invasive or even invasive surgical procedures that can solve this.

I don't want to be biased, Mel, but I am very leerie when it comes to chiropractic solutions when it comes to chronic pain. It can even lead to further nerve damage. Just from basic anatomy ug physiology na.

Have you been prescribed pain medications?
Considering the severity of your condition. Ask your family doc about it.

But my recommendation lang is to see a lower back/pain specialist.
and Neurologist.

Mel, that kind of pain is symptomatic for possible LDD (lumbar disc disease), the most common form is called Sciatica. 

Mel, you said you had Xrays, X-rays are good but they are not efficient. That is old style method of diagnosis, already.

Ask your physician to have MRI (magnetic resonance imaging), and CT scan.

My physical therapist gave me pain killers before. But he said, it's not good to take regular pain killers since it can damage the kidneys. Other than that, I am not taking any thing.

Another doctor told me to get MRI but I have not went through it.

The reason why MRI and CT scan is used as preferred to x-ray is because CT scans and MRI can develop 3D imaging, and can go indepth as compared to the limited xray.

In CT scan we can find any damage to the spinal disc, if there is a disc damage, it will cause pain not only in the contralateral and ipsilateral side of trauma, but also in areas of the lower extremity. all of which you described.

this kind of phathology can be treated by anti-inflammatories, and if is very severe, exploratory surgery can be done to see what the cause is. either it be a lesion of the associated region.

I am asking you Mel to see a neurologist as well; as they specialize in nerve damage and can assess your symptoms better than I.

Yes, that is true, pain killers will affect the Pentose 6 Phosphate pathway, in liver and kidney. But these are the risks we take if the pain is really severe.

When you go to Manila, Mel, go see:

1. Neurologist
2. Orthopaedic specialist.

ask them about possible treatment and drug therapy that can alleviate your pain for the time being.

Have you heared that some people have uneven legs? Ang  usa taas, ang usa mobo gamay? Just a milimeter of difference cause imbalance that puts pressure on your spinal column, hence back pains.

I can't stand physicians that are like that.

OT: laughing at patients is considered medical mal practice, over here in the United States.

That is basic medical ethics.

many physicians/surgeons have been sued for Milions for doing just that.

As a physician, one is to provide help and care, and understanding of the patient, and not laugh.

I started having this pain Vids when I was still in my mid 20s.  One time pa gani didto koy assignment sa Cebu adto unya kay sugmat man jud as in sakit akong bat-ang, pag adto nako sa Chong Hua kay naa daw koy "slip disk" and I needed surgery asap.  Unsa ka asap pod ko ning layas sa doctor's office.  Tiaw butangan daw ug steel akong likod.  No way, Jose!  Well, usa sa ingon sa doctor na maka trigger kining tag-as ug heels akong shoes.  Well, I'll deal with the pain. hahahaha

Naa bay chiropractor diha sa ato, Vids?  Or basin naa Cebu kay sure ko naa sa CDO.  Pag try daw pa treat sa ila.

If it was a slipped disc, then the treatment for that (complete effective treatment) is surgery. Now we have what is called no-invasive laproscopic surgery; Ive seen this in the OR where the physician doesn't pierce the skin with a surgical knife but merely makes a minor incision, where a 1.5 cm tube goes inside the affected region, and repairs the affected spinal disc.

If severe, and if there is a tumoric growth on the associated spinal nerve, which we can observe through proper MRI and CT scanning, we will then utilize what is called Explorative Surgery. The surgeon will have you in prone position, and make a lateral incision with scalpel, peel back the cutaneous adipose tissue, separate the origin of the Lattisimus Dorsi muscles that cover over the spinal cord. And remove the tumor and or place biomedical replacements for the affected/defective spinal disc. We can do this now vairly easily.

After surgery, everything is repaired, sutured, and the patient is put under fluids and provided with a heavy dose of Percocets or Oxycontin. Which are nothing but pain killers.

In 2 weeks, the tissue will heal, and patient removed from pain killer medication at appropriate time. Damage rectified.

In dealing with LDD's, the final solution is only through surgery.

I talked to my surgical neuroanatomy professor, he is also a licensed Neurosurgeon btw, and asked him about your condition.

Recommendation kono nija is that you see a Neurologist, and ask to get an MRI or CT Scan, both if possible.

Mel, I know you are a very educated woman, what Lorenzo is telling sounds good but really costly. Do you believe in faith healers? Kanang  doktor laway? I have myself treated with a pain in my elbow (parehas ba sa tennis arm), ang mananambal ra ang naka cure. Si nong Demi taga Kasbo, Guidnulman. He keep on massaging the painful part and after half an hour it was gone.
He will also tell if your complains can be treated by a common medical person.
I gave him (1,000) Phil pesos, out of joy and gratitude which he declined because it was too much.
Try if you are interested.

There is no treatment for any disease aside from hard medicine.
In instances of quak doctors and the lot, those are all psychological.

Considering she is suffering a chronic pain, it is suggestive of global neuritis as the symptoms dictates it. Recurrent rubbing pain in said appendages indicates that there is some kind of pathology in the neuroanatomical structure, which only through MRI and CT scan can elucidate. Either it be a lesion, brusing, etc.

It is very unwise to forgo proper medical treatment for herbal homeopathic.

If I wishes to have a CT and MRI I have to wait for 3 months here in Germany. This is just to give you an idea how to access to this equipments.

Germany has a socialized medical system, Manay. That is why you wait 3 months. In the United States, you can have a CT or MRI within hours after request from a family practicioner and or neurologist.

The same in the Philippines, which does not have a socialized medicine.

Again, herbal medicine will not cure or alleviate something that is neuroanatomically linked. Rubbing the back etc will not help, her, as she had already mechanical chiropractics utilized on her, on the contrary, continuing to do so would even as so much as bruise and further damage lesions in the spinothalamic and spinocerebellar pathways, which can cause contralateral as well as ipsilateral hemiparesis.

That is paralysis (same side or opposite sided, or bilateral globalized).

In dealing with neurologic or spino-neurologic conditions, which she is experiencing, it is prudent to have proper medical treatment.

I think it is safe to place trust on the recommendation of a Neurosurgeon over a quak doctor.

Manay, I can understand if people go to quak doctors for neck pains, foot pains, arm pains, even spiritual pains (at the discretion of the patient, of course).

Ive heard such things before, doing prayers and rubbing etc etc.

Sure, it might even help in the vasodilation of muscles, etc. But when it comes to something neurologically related, directly, as in the case of the spinal cord, there is no excuse or replacement for proper medical treatment.

My conscience cannot and will not allow me to let that stand. In cases of patients that have such symptoms, it usually is indicative of either some kind of lesion or mass growth in the pain sensors in the associated hypothalamus, or some kind of inflammation of afferent sensory receptors in her posterolateral region.

We don't know. We can only infer and make a proper diagnosis after results of MRI and CT scan, which will allow the physician to give her a proper and hopefully positive prognosis.

I highly respect your opinion, but places like Bohol you could see that they just trust the Divine Providence. Had I allowed a nuerologist to opereate on my elbow, there is a high probability of things which might go awry like stiff fingers.  I don´t want to take the risks. I told him that I will have it done in Bohol and he was laughing, I showed him after my vacation, he was not laughing anymore but confused, ha ha ha.

Sure, Manay, thats good that the quak doctors fixed you up :)

Again, the arm is not the neurological system. The reason why I am forwarding the recommendation from Dr. Frank Cooper, who is a NYU-trained neurosurgoen and professor Surgical neuronatomy, is because Mel will be going to Manila.

Where she will have access to MRI or CT scan machines.

dong, thanks for the recommendation. nahayagan pod ko. i'm actually doing my research now. i'm reading this book on pain, titled "Your Pain is Real." It's by an NYU Neorologist and pain-management expert.

since i have been to different practitioners (and nobody has given my pain a name), i finally wrote down a pain diary to describe my pain in detail... what area, when it occurs and its frequency. i plan to give it to the doctor soon.

days ago, i accidentally met a low back pain doctor here in tagbilaran. he might be able to help. try lang. i want him to look into my diary and read the book i'm reading.

dong, i also learned from a site that fractured coccyx has relations to pain, chronic fatigue and fibromyalgia... i read through the site and the patient there was me. i'll post the url soon if i stumble into the site again. i printed it out though. i'll bring it to the doctor, too.
