INSTRUCTIONS:


- BRING ONE (1) PHOTOCOPY OF DOCUMENTARY REQUIREMENTS (PROPERLY TABBED AND FILED IN A WHITE LONG 5)
- BRING ORIGINAL COPIES OF DOCUMENTARY REQUIREMENTS (FOR REFERENCE)
- VISIT THE NEAREST BJMP MIMAROPA JAIL UNIT/REGIONAL OFFICE FOR THE INITIAL EVALUATION OF FOLDERS AND INITIAL MEDICAL EVALUATION
- TAKE NOTE OF THE SCHEDULE OF BJMP-MIMAROPA JAIL UNIT/REGIONAL OFFICE FOR THE INITIAL EVALUATION OF FOLDERS AND INITIAL MEDICAL EVALUATION


Medical Tests Needed (prepare original and 1 photocopy):
1.    Ishihara or Type Test for Color Blindness Minimum of 17 plates
2.    HBsAg - if no hepa vaccine (anti-HBs - if w/ hepa vaccine)
3.    Visual Acuity Test
4.    ECG
5.    Xray


ATTIRE: WHITE SHIRT, MAONG PANTS, RUBBER SHOES
MALE: NO EARRINGS
FEMALE: LONG HAIR MUST BE TIED NEATLY IN A BUN


Applicants whose names did not appear in this announcement will be accepted as WALK-IN applicants.
