Poisonous Spider in Tagbilaran Attacks Foreigner

I have just learned and seen a Norwegian friend of mine visiting here his fiancee on his school break now and what i observed was not good.

He was in the car of his GF's dads car and was bitten by a poisonous biting spider and went into shock after being injected with the spiders venom.

When he went to the hospital here in Tagbilaran the Doctor administrated a wrong medicine in him and it has caused him to have swollen veins and be broken out in 95% of his body.

He has times in when he suddenly stops breathing or is short of breath.

Please people be careful of spiders, snakes, and frogs who have poisonous venom.

Doctor's please be careful when administering medicines.

I have just received an update from my friends girlfriend he said he is not eating and is tired and feeling weak, she is worried about him and his experience here makes him scared to go back the Dr. or hospital.

I hope i can get the attention of the media and bring this to the publics attention.

please people be careful.

I am really worried about my friend now.

Please give advice he was given the medicine "Celestamine".

he was also the foreign judge for this years Miss Bohol International.

looya man pud nga deri pa jud sija napaaki... ug kinsa man pud tong doktoraha nga nagpataka lang ug prescribed ug medicine..... ipa tandok na la man to unta... supsupon ang venom... maajo pa....

What are your familiarities to such reactions and what are the symptoms to look at before it goes into a non totally re missive state in where it would be at the point of not being helped or saved?

The type of spider that bit him was a small black spider that jumps around.

Also what are symptoms to look for as far as a victim is concerned? does the spider die after attacking his prey?

Dear Priscilla,

I cannot make an exact prognosis as I do not know what species of spider attacked the individual nor do I know what previous medical condition, any medication that this patient was on prior to the incident of the spider bite.

We have to understand and take into consideration that drug interaction could play a role in the manifestation of side effects and symptoms, considering the notion of impeding with the drug metabolism in in the hepatocirculartory system or even extrahepatocellularly.  (meaning in the liver or outside the liver---as we have Liver enzymes known as CYTOCHROME P450 isoenzymes, which are responsible for the metabolism of drugs, toxins etc; any alteration in the operation of these isonenzymes, can either increase the effect of drugs/toxins (in the case of inhibtion of the hepatometabolism, or the antithesis (opposite of which, in regards to the induction or overinduction of said enzymes).

The patient that was bitten by the spider was injected with endotoxins from the spider, which will elicit an immediate humoral defense response (immune system response) from the body due to the fact that the toxins are foreign from the body.
What happens in this case is that the circulatory system will increase mast cell production, and with it the release of prostaglandin and bradykinin, as well as the release of mast cell's histamine. Histamine causes swelling, vasodilation,---which triggers for more wbc and macrophagial cellular systems to come to the area of infection and insidious onset to mitigate and contain the toxin.

Histamine can cause bronchoonstriction, which will lead to a difficulty in breathing, as it causes spastic contractions in the bronchioles, now if this man was an asthmatic patient, this would be an important contraindication. Difficulty of breathing would occur--and with that, a plethora of medical conditions correlated to that: dyspnea, nausea, vomitting, headhache, tremor, etc. etc.

The drug : "Celestamine" is nothing but an example of Anti-Histamine. Its a simple and a general subtype used here in the United States as well as in any country in the world with an established medical system. The PHilippines, being one of them. :)

The problem is not the drug, Priscilla, the problem is the pharmacokinetic and the pharmacodynamics that occurs when the drug is taken. If the patient was taking nitrates, if the patient was taking cimitedine or phenytoin or any other drugs that can inhibit or induce the CP450 isoenzymes, this would affect the metabolism of the said drug and other drugs that the patient was taking.

I don't know the patient and dont know his medical charts, but if he was an CHD patient, he would already be C/I (contraindicated) for the provision of "Celestamine" since it causes cardiotoxic efffects. Can cause tachyarrhthmia etc etc.

I don't think the problem stems with the Philippine Doctor's incompetence, on the contrary, Filipino doctors are one of the Best of the Best here in the 'states.

The problem would probably lies on the fact that the Filipino doctors don't have full access to the patient's medical history, drug list, or the lot. SO their ability to prrescribe medication according to contraindications and medical diseases is hindered. The patient, who, am sure, went to the hospital, already in a shocked state, was not in the state of mind to provide his complete medical history, nor does hhe have the list with him, considering the fact that this bug bite situation was totally an assymetrical circumstance. It was probably not in his plans to get bitten by a spider in his trip to the philippines, and therefore nullify any subconscious notion of bringing his entire medical files from his physicians back in Europe.

Right now, the best thing to do for this patient is to maintain in hospital care. So that the honored Nurses and Doctors in Bohol can monitor his situation, and provide the medical condition that is alloted in the Province of Bohol.

Hope it's not a black widow spider or perhaps the brown recluse. Very dangerous spider and I just see some episode on animal planet about people being bitten by spider. At least the two spider mention above only found in U.S.
Probably the spider that bite him maybe the Black house spider.It is poisonous but not lethal.Certain people bitten experience severe pain around the bite site, heavy sweating, muscular pains, vomiting, headaches and giddines.

hehe, mao bitaw, Ate. We can only make sure to give proper anti-histamines as well as fluid and electrolyte IVs.

If the poison is isolated, then the order for proper anti-toxin can be made.

My only question is whether the hospital has resource required to order such, considering its location and financial predisposition.

In this case, I would recommend the family transport the patient to a large hospital system, per se. University of the Philippines Medical Hospital, or Santo Tomas Hospital Systems, or even Cebu's Cebu Doctor's Hospital.

The Philippines is a leading medical care provider in the region, and am sure and absolutely confident that this patient is in good hands.

20,000 Filipino Doctors practice here in the United States. The 2nd largest FMGs (foreign medical graduates) in the 'USA. Second only to the Indian-Pakistani Physicians, which number to over 40,000.

Filipino Doctors are considered one of the Best of the Bests here.
And they were trained in the best medical schools in the World.

Update,

Without mentioning the Dr's name who had prescribed and instructed the medicine to my friend the other day.

I had talked him into going to get a check up with the dr.and he agreed to do it this afternoon.

According to the written instructions from that particular Dr. he/she had written take 2 four times a day!.... Oh MY God!

The Medicine Intern Dr. said that the Dr. had made an error and had been overdosing my friend by four pills.


She was ale to prescribe new medicines and explained tht the four pills over was the cause of his skin burning and bumping up of the veins.

The face,neck, arms legs and back of his head down his spine swollen puffed up and rashed out.

The overdose is what caused over 50% of his symptoms including major dehydration, watery diarrhea, weakness, and dizziness.

She was thankfull that he was smart enough to stop the first medicine on his own b4 the prescribed 5 days was up.

The stupidity of this whole thing is because my friend is obese. So Dr. told him "Your fat I want you to double up from the normal dose."

I also have been told the same things by dr's here. i have found myself in bed the following day not being able to get my head off the pillow let alone knowing my own name.

The medicine required by the body is the same regardless of bodysize. according to his new Dr. today.

Oh my what an incompetent doctor. I myself don't trust the doctor there. Last time I went visit my family I got sick. Got high fever after visiting the known expensive hospital in the city I won't mention the name. I must have pick up some bugs. I got scared of going there again but my family insist I should go so they can check me up and at least give me prescription antibiotic. You won't believe how long the line is. Ended up spending half day waiting while my body shiver. One thing I notice since I am following a foreigner patient the doctor spend long time talking to this people. When it's my turn the doctor ask me few basic question regarding my health. Well guess what when I sign up the form I wrote lot's of symptoms in there some were not true I am just making it up. I wanna make sure how incompetent is the doctor is.
The lady doctor seems nice and she became more nicer when she know I live in U.S and then I start throwing lot's of medical term on her the one I read and learn before.I complain about I had otitis media and so on. To my horror she believe all my story. She sure believe all the crap I am talking to her. She start writing prescription medication on me but before that I actually stop her and said. How about check my blood pressure first and my vital sign and can you at least look on my ears. She check my ears and guess what she use the little dinky flashlight.  She never had the ear examination tools. She proceeded to write prescription antibiotic on me then and sure enough it's the most expensive antibiotic since she know I can afford them so she refuse to give me the generic one. These doctor were suppose to be the best doctor in the city but I guess not.

The moral of the story: Always cross reference and ask for a 2 to 3 tiered medical advice.

Tho I do have to admit, pharmacologically speaking, a larger a patient is, the larger the dose, due to the adipose tissue hindrance. Metabolism can be hindered and effect can be delayed due to the size of the individual, depends on the drug's lipid and blood solubility, which determines onset of action. Or for the physician that prescribed the drug, he or she was referrin to the MAC potential for the said anti-histaminase.

Instead of doubling the drug, he should have given an extra 'umpft' instead of a double dose. But whatever, that was his or her own professional rationale.

Just want to know if your friend is now in stable condition.

I'm saddened to read of your poor experience with some physicians, Ate Raquel.
But I can assure you that I have met some Boholano-bred Physicians here, and some of them are in Psychiatry, Family Practice, or Internal Medicine specialities.

I think the limited ability of the hospitals in Bohol is due to the lacking in funding, and the increased patient demand. The resources are in 'tight' check. As you provided us with your personal example of how long the line was.

From what I've heard from my relatives, the hospitals in Bohol are limited due to the funding situation. The Government in Bohol Needs to put medical facilities in the priority list. And I do hope Mayor Lim and Governor Aumentado read this.

Well the church needs to implement family planning and be more up to date and practical about how we are living in danger now due to over populating and NO RESOURCES. Like Duh!

Keep pushing out those babies and eventually we will lose the whole planet!

The most pisonous spider I ever know is the 'kagwason'. It is darkbownish that thrives in the sheets of decaying bananas.

Anyways my friend is doing better with all the right meds and the amounts and is headed back to Norway to attend classes in a couple of weeks.
