the doctor was somewhere else; probably in another hospital, or so i would like to believe.  perhaps he was tackling a life-and-death emergency, who knows.  i can take comfort though in the fact that none of us, maybe 6 or 7 bored patients-in-waiting, was an emergency case.  doctors understandably hardly have time on their hands, but it would be nice if some time estimates can be had so patients like me need not waste our own precious time. 

Well, you can always reschedule. :)

Speaking about patients waiting-- today while I was doing my clinical duties, I entered a room in which a patient was literally 'red' in the face. She was waiting literally 2 hours to be seen, unsaon man lagi, daghan kaayo pasyente pero only few attending M.D's were present and fewer interns (5, and i was included in that number). Suko lagi tong pasyente, pero, kind words and reassurance helped mellow her down.

I am empathetic to your situation, Isles. Hopefully next time your M.D won't be late again. ;)

Ayaw pod intawn i-encourage ning aksyon, Father, uy! Diri sa America, sue happy ang mga tawo diri. Pinaka sobrang sue happy....to the extent that doctors are now practicing defensive medicine to avoid being sued.

This is one of the many reasons why physicians spend 60% of our time filling papers and documents.

hahay! i should have gone to medical school in the Philippines because of the full scope of practice medical students have in the Philippines. Medical interns in the Philippines are not pressed in having to pay "Medical liability insurance" every year while doing clinical rotations. Diri sa America, anyone is liable.

This is the reason why Filipino doctors who graduated from Philippine Medical Schools are considered "all-around" Docs diri. Dili ma hadlok sila mo dive in sa situation.

Sa Pilipinas, daghang tawo walay kwarta dili ka afford maka check up. Sa America, bisan walay health insurance or walay kwarta, naay MEDASSIST nga mo bayad; naa pod daghang free hospitals/ free health clinics sa America. Daghan gehapon mo sue sa doctors kai naa poy pasyente nga ambisyoso maka kuhang kwarta dali.

Maayo pa ang mga Doctors sa Pilipinas kai walay situwasyon nga daghang liability. My own uncle told me that when he was still a medical student in Cebu, he would already perform circumcision on hundreds of children as per requirement of their rural medicine rotation. That is unheard of here in the United States.

bitaw, my impatience with waiting for the doctor was fleeting.  nangita na lang kog positive pagkahuman. kaayo na lang nga wa magdali ang doctor kay gitabang ko.

i had a worse wait some years back.  tibuok buntag hangtod mid-afternoon.  wa ko sapota ato kay wa koy laing appointment (unlike in my 3-hour wait lately).  nganong na-late ang doctor?  nagtabang intawon ug himatyonon sa i.c.u. from 4:00 a.m. to 3:00 p.m., pumping the patient's heart now and then.  by the time he came for our appointment, morag katumbahon kay way kaon.  i didn't ask him if his patient lived.  but i thanked my creator silently because i only needed to wait; i wasn't that suffering patient lying between life and death.

kabahin tuod ug kiha-kiha, morag mas european ta kay sa american.  put another way, morag mas filipino ang europe kay sa american kun kiha-kiha lang ug doctors ang hisgotan, hehe.  i know from a doctor friend in germany that hardly are doctors sued by patients there.  that's why they're sympathetic with american doctors.

world-renowned suers at that.  imagine, some years back, the hilton hotel in new york used to give its extra victuals of the day to the homeless.  naunsa nga ikiha na man ang hilton for giving stale food kuno.  an 'ambulance-chaser' of a lawyer tipped those homeless off on how to get money.  na, hilton decided to just throw its extra food instead.  everyone lost.  ::)

naa pod sa ato if one only knows where to go.  dswd is always a place to ask help from, honest.  the other is the philippine charity sweepstakes office, from the provincial to the national level.  of course, we have our rural health clinics whose services are free; manned by a doctor at the municipal level and a midwife at the barangay level.  (most of our nurses would rather go abroad.) 

himoon pa man gani ning rural health clinics ug outlets sa abortifacient contraceptives kun madayon ang rh bill.  (n.b. contraceptives are actually already available now in rural health units.)  so far, one can also have access to vaccinations and first aid medicines. 

kaso, ingrained na sa pinoy mentality nga kun gobyerno, wa gyoy ayo, mao nga di na lang moadto ug mangayo ug panabang.  but believe me, they work.

it's the feast day of the black nazarene (quiapo church).  manila expects terrible traffic (more terrible than usual) and the multitude of devotees is being warned against a possible terrorist threat.

i'm done reading, at least for the day, anything i could about the impeachment of chief justice corona.  the whys and wherefores in the run-up to the impeachment are indeed disturbing.  so far, many characters are falling flat on their faces, from congressmen to palace bureaucrats.  and so far, only the senate has managed to remain august vis-a-vis the impeachment.  or so i guess.

it's the doctor again today.  it's another doctor and the wait was longer... 9:30 a.m. to 1:45 p.m.  i was so hungry after a light breakfast and a delayed lunch. 

i got to talk with a very talkative nun.  she turned out to be a nurse accompanying a 90-year-old fellow nun.  their wait was longer, so who am i to complain?  to think that their congregation is the hospital's administrator.  they gave me rosary beads, glad to know that i'm a product of one of their schools. 

anyway, i have to go back to the hospital for more tests.  drat this sedentary lifestyle.

I was walking in the park and I saw three very beautiful women dressed in tight spandex attire jogging with no coats. I thought to myself, "Thank God there is no cold winter season and Thank God for the beautiful view" :)

Was at Starbucks Coffee store in the AM today. As I was getting my coffee from the counter, a man who was sitting in one of the high stools collapsed and fell backwards hitting the back part of his head on the edge of the wooden sofas. I triaged the man and stabilized him while I asked the workers to call the paramedics. The patient , whose name was Mike, held my hand when I laid him on the sofa and asked, "will I be alright? I don't have health insurance, you know."  I reiterated to him, "you will be alright, Mike, you will be fine." He turned to me and asked, "What is your name, sir?". I answered him, "My name is Albrando Lorenzo." Within 5 minutes the Paramedics / EMTs came and escorted Mike to the nearest hospital.

The starbucks workers gave me 4 free coffee passes for the "rapid help". Talk about being at the right place and at the right time.

I hope Mike is alright.

And with that, my day started.

Today's unpredictable incident allowed me to ponder and discern on the verse from Sacred Scripture:
"Beloved, do not merely look out for your own personal interests, but also for the interests of others." (Philippians 2:4)

It reminds me, and I hope that it also reminds all of us that it is good to give back to people and help those who are in need. Tho my assistance towards Mike today was , in my opinion, very minscule, I know that it was a reminder for me from GOD to focus on giving and doing things without expecting anything in return. That , if you do good and stay true to God, He will provide. Always....


May God Bless all of us, my dear brothers and sisters in the Faith. Have a Blessed Day!

I had to change my car battery today. It took me forever to loosen the bolts from the battery holder tho. I need to stop by the auto body shop tomorrow to make sure it tightened properly. Thanks God, I didn't freeze outside while I was changing the battery!

Good Morning,


The itme is 10:29 AM here in Chicago, Illinois, USA. I woke up to a dread sight: SNOW! And a lot of it. Looks like a foot of snow has been poured on top of Chicago's streets and I am getting ready to go out and shovel my car out of snow. Not a pretty sight, nope, not at all! Thank God I am not on-call today or working. We have a faculty meeting this coming 27th and then a "skype" faculty meeting with some members of Northwestern faculty on the 25th. There is a graduation ceremony on February, several of my former students are going to be graduating so i have to attend ..proud pod ko that naka pasar sila sa ilang boards!

The other day I had a call from one of my former students -- to inform me that he passed his Electrocardiography Technician Board Examination , and then I got a call form one of my other former students also passed his Advanced EKG Tech Board Exam as well. I was so proud of them! They called to thank me for the instruction and for giving them "tips" on the exam. hahaha! There really is no describing the feeling of knowing that you have helped someone succeed ... it is priceless!

It's time to go out now. There is a job that must be done---Shoveling Snow!

i watched a 45-minute fireworks display from our window.  luckily from our vantage point the chinese new year fireworks of the mall of asia in manila were quite near.  i wished our 10-year-old was with us to enjoy the show.

there are no sirens yet... good.  there are no fires in the vicinity after all, unlike last december 31 when a fire that broke out was just two blocks away.  and it looks like there won't be any smog in manila tomorrow morning.  now i know there's a difference between fireworks and firecrackers.

I am currently looking for the proper MEDICAL RESIDENCY program that can best fit my future goals and aspirations. I have laid down the following possible Residency specializations that I will target this coming year: General Surgery, Internal Medicine, Psychiatry.

I have talked to some MEDICAL RESIDENTS in the area of the possibility of "scramming" for possible residency spots here in the Greater Chicago Area. I might have to look into that when the time comes closer.

Right now, as RESIDENCY MATCHES are coming up, and my upcoming USMLE (United States Licensing Examination) , I am truly in a state of excitement and nervousness.

I hope and pray to God that my spirit remains calm during this time. Please pray for me, guys.

It's Friday, Jan. 27 my niece birthday "Shaira". Happy birthday Ate Shaira. And it's our first Pro-D Day too for 2012, so I decided to stay home og mag general cleaning. Laba diri, laba didto, luto og kuskos sa among bathtub, vacuum, silhig unya kuskos pod sa sawog usahay. Hayyyyy!

February 1, 2012


I just finished a 24 hour on-call shift today. Was in the hospital since 7:00 AM yesterday, and just got discharged from on-call shift at 7:00 PM right now. I was supposed to be done by 7:00 AM  this morning, however, because 1 medical intern was not able to come in today, I was asked to stay in for another half shift. What was supposed to be an "extra 4-5 hours" was extended to another 12 hours. So , my "24-hour on-call shift" became a 36 hour on-call shift.

I slept only 5 hours during the on-call shift because of the admissions we had in the Psychiatry Department. Since it was the beginning of the month, we had a lot of Suicidal patients, schizophrenic patients, schizoid patients, bipolar patients, major depressive patients, and even catatonic patients admitted. Ay ka libog kaayo.

I did not like having to run around from room to room, doing most of the admission reports when there were supposed to be 5 medical interns present and 3 attending M.Ds. Because of some very lazy medical intern who was in our group, some of us were forced to do most of the work in patient history , prognosis and ordering lab tests.

I was so very annoyed at how some people , who are expected to see patients and fulfill their clinical rotation requirements , expect others to do their share of work. At work, I am not so very vocal because I like to maintain a very professional , professional demeanor, but when certain medical interns do patient reports as required , it is very unnerving when 2 others are too busy laughing and making jokes. I don't understand how it takes 2 hours to finish only 4 patient files. Jonathan, Bruce and I were able to finish 12 each in that time, however, certain interns thought it more important to joke around.

Perhaps the reason for my irritability was because of my lack of sleep. I feel that my circadian rhythm has been out of place for the past week; the toll of these excruciating rotations is putting a toll on me. By chosing to stay an extra 12 hours, I thought to help out the undermanned staff in the Psychiatry Department, however, it is very unnerving when medical interns who didn't do 24-hour on calls were unable to do their job properly. It is so very inconsiderate of them. The should be reminded of the importance of "TEAM WORK".

I am happy I don't have to go in tomorrow. Friday , tho, I have another 24-on call shift. I pray to God it doesn't become another 36-hour shift.

Oh, truly, these are the "slave" years....   :'(

This week has been a very busy week. At 2 AM in the morning today, I went to the physician sleeping room to lay down and get some sleep. I went to bed at 2:20 am, and i was woken up at 3:30 am because some patient who was admitted the previous night was experiencing acute psychosis. It was a code gray. I cannot believe that there was no one else they could call, they had to wake me up after only starting to sleep!

Hospital staff should show some respect for people who are trying to sleep, and if it is an emergency, should knock on the door instead of barging in and turning on the light and yelling to get up.

If it is a psychotic patient, sedate lang sija with Haloperidol. Don't make such a big deal about it. Because of that incident, my "5 hour sleep" wasn't even sleep. It was 5 hours of trying to sleep. YATAPS!

I just went to attend one of our last department meetings today. I had a talk with a good friend of mine today, hoping he is doing fine. I do miss my mother and father, wish that I could be home to see them again. I also miss my two little brothers, Don and Andre.

This is the last week for my Psychiatry Clinical Rotation, thanks be God I am done with that elective rotation.

Would I consider applying for MEDICAL RESIDENCY for Psychiatry?

Yes, I wouldn't mind. One thing that I like about it is the complex nature of the specialty, it truly is like playing a puzzle in trying to identifying the patient disorder, understanding the best mode of treatment / therapy , and all at the same time keeping a check on their medical history , drug interaction.

we had a little grill party at home.  bag-o pa kong nahuman ug panghipos ug tiwas kay gipapahuway na lang nakog una ang mga nitabang kay momata pa ra ba silag sayo.  naglurat na sad akong mata pareha gabii nga hapit na mi alas dos nga niabot sa balay gikan sa airport kay nitagbo ni banana republic.  i still enjoy this habit of counting the spoons, forks, and knives and other cutlery after a party.  i enjoy it more when nothing is misplaced.

Just got back from work , i had to fill in for Professor Esther , who was attending Jury Duty today. The class that I filled in was Anatomy & Physiology I , most of the students in that class were Nursing and Physical Therapy students. Class started at 5:00 PM and ended at 9:30 AM. I was 6 minutes late today because of traffic. I lectured on Chapter 2 , which dealt with body chemistry, pH, covalent bond , ionic bond, hydrogen bond, carbon reactions, and body electrolytes.

I took roll call at about 5:10 PM, and the facial expression of the students was priceless. One asked me , "Excuse me, sir, aren't you too young to be be lecturing?".

"Absolutely not. Would you prefer to lecture?" The class burst into laughter, broke the ice. The 4 hour lecture that i gave was at par, i would say. I gave them some high yield clinical information in addition to the material that was prescribed. There were about 60 students in that class.

Just finished the 5th paper for my Psychometrics & Statistics class. Taking this class has been an asset to me in having a better appreciation for how medical researchers compile data, analyze data via multiple t-tests, ANOVA, Chi-Square tests and other processes related to this very interesting subset of medicine.

So far, my professor, Dr. Leonard, who is a very rigid and hard-line type of instructor was able to convey to us the importance of analyzing instances how to read Post-Hoc analysis when observing the effects of 3-5 independent variables on dependent variables.

Given, I was a bit whizzed at it because it was a "while" since i used statistical analysis. We were not taught this in medical school, so I had to remember some theoretical concepts (which meant opening my old statistical text books from undergrad) lol.

Since this is my first "statistical" class for my Ph.D requirement, I have to get used to it. Next term, I have elected to take Multivariate Statistics II with Dr. Leonard. At first, I did not like this man, but now, I've grown acquainted to his teaching style. Forward !

I talked to one of my colleagues, Dr. Del Rosario, about the possibility of doing clinical research at Rush Memorial Hospital , a research-teaching hospital in downtown Chicago, on Diabetes Mellitus Type I and II. Apparently, there are positions available on the summer for a 2 month-long research internship. I am still debating if I should proceed and submit my candidacy.

all alone am i in the house.  everyone's out and i didn't feel like going out.  not in this sunless weather.  i'd rather listen to some good, old songs and think of the guitar chords.  yeah, the word is 'think', because i gave up my guitar for the banana republic's friend some years back.  his friend, who seemed to be emotionally falling apart, had borrowed my guitar, and the banana republic felt that he needed it more than i did.  now i can't help but think of that guitar because that was the banana republic's valentine gift to me 15 years ago! 

did i regret the gesture?  i don't think so.  but i did tell the banana republic that i didn't want to see that guy again.  so far, so good.

all alone am i in the house.  everyone's out and i didn't feel like going out.  not in this sunless weather.  i'd rather listen to some good, old songs and think of the guitar chords.  yeah, the word is 'think', because i gave up my guitar for the banana republic's friend some years back.  his friend, who seemed to be emotionally falling apart, had borrowed my guitar, and the banana republic felt that he needed it more than i did.  now i can't help but think of that guitar because that was the banana republic's valentine gift to me 15 years ago! 

did i regret the gesture?  i don't think so.  but i did tell the banana republic that i didn't want to see that guy again.  so far, so good.

I am to instruct a new Medical Terminology class that begins on the 16th of this month till the 20 of March. This is a prerequisite class for my students who are mostly nursing, pharmacy, and physical therapy majors in college. I am happy that the schedule is Monday, and Wednesday from 9:00 AM till 1:00 PM. This is good because it gives me time to work with schedule to attend hospital duty on Tuesday,Thurs,Friday from morning till whenever and on Monday and Wednesday anytime after 2:00 PM. Busy napod ang schedule. I have been told that the class that I am to instruct will have 52 students.

My feelings on this? They expect me to cover 15 chapters in this class, i just hope that the students will be able ready to speed through this material; as this course is to be only 12 weeks long. Anyways, i just hope that i don't have any 'bogoybogoy' students. :P

Thought for the day: I just recently finished talking to my younger brothers on the phone. Both of my younger brothers had an issue concerning respect and work. Apparently, my younger brother is being a bit too hard on our youngest brother, Andre. I had to remind Don Rico that kind words, patience, and positive reassurance is the best way to encourage and facilitate growth and understanding of one's younger sibling. I had to remind him, Don Rico, that when he was Andre's age, I encouraged him in studies and in actions through reassurance and through loving embrace, not through excessive scolding. The latter does not build character , but only levels it down. I had to remind him that kindness and positive encouragement supercedes scolding and discipline. To correct the situation, i had a "talk" with both of them on speaker phone and to remind the importance of family, in supporting one another and not on putting one another down.

Sometimes its easier for us to put or correct someone through negative and immature means, which however, does not build a person up, but on the contrary, only puts them down and hurts one's credibility and own advice. Better to see the underlying situation first, then efficiently and properly advocate change through positive encouragement first. Sometimes, we forget that the one receiving also has feelings. We tend to forget this from time to time.

I reminded both Andre and Don Rico of a verse from Proverbs 17, which states, " A friend loves at all times, and a brother is born for adversity."

We need to be there for each other, support each other, as living example of the saying, "Blood is thicker than water."

I do hope that my younger brothers exercise prudence , patience and lovingkindness with one another.
