The Daily Splurge
« on: June 05, 2010, 08:07:04 PM »
Good morning,
Today is June 5th, 2010 and i just woke up from a rather limited sleep to drive my younger brother to take his SAT exams. Now that I am back to my desk, i cannot help but organize my notes on endocrinology and gastroenterology. For as much as I have conference calls with colleagues and meet up to discuss a clinical vignette, one thing continues to hound my mind. It is the simple fact that tho we are able to discuss the mechanisms of action, modes of therapeutic treatment, one thing is clear: the genetic causes of these diseases continue to haunt.

All therapeutic modes target only the symptoms, i have discussed with another colleague in the necessity to find the genetic basis and utilize/implement a gene therapy. For if we do not think of this, we will continue to treat the same disease, with no victory in sight.

Examples of which include:  Cushing's, Conn's, Addison's diseases.

I could not help but laugh and chuckle at the brilliance of Dr. J. Barone. Truly this man is a genius of a physician-surgeon. When dealing with patient diagnosis , i am amazed at : Pheochromocytoma.

Dr. Barone, your 5 P's Rule comes out quite handy.
-Pressure (elevated)
-Pain (headache)
-Perspiration
-Palpitations (tachycardia)
-Pallor

Now only if that patient was just nice. I must admit, i give it to the nurses to have such tolerance. :)

Evening Thoughts (June 3rd Entry):

Why is it that when I am asking for LFTs, you are unreceptive? If I give an order, I expect it to be completed so that proper diagnosis can be made. If I order something specifically for the Al Phos levels, or if I say I want the GGT levels, I mean it. Do you understand that this is indicative and sensitive for various liver diseases. And you know what, it was evident that the patient had hepatocellular carcinoma, as verified by the raised Al Phos levels.

Kusug ra ka mo katawa, thick headed ra ka kaayo. Learn to take orders and follow them.
For sure I am not going to be the type to stand and not say anything. I guarantee you that.
Don't think that I will be a push-over type like Dr. Bermingham. No way.

Truly,
B.Lorenzo

After picking my younger brother from his SAT exam, we had a long brotherly talk, one that i found myself reminiscing of my own high school years; seeing him at 17 years of age and hearing his questions, I could not help but refer back to my own youth when I myself was asking the very same questions he asked. He asked about how will it be like being away from home for college, did i ever get over leaving my high school friends etcetera etcetera.

So before I could even answer him, I had to think and contemplate and recover my own thoughts, memories, personal feelings. And with a deep breath, and a smile on my face, we talked. I told him that one has to be ready for the surprises of life, for sure, there is no sense of stability in life, there will always be some kind of storm and rock thrown at you throughout your journey. I told him that. But despite these challenges, one has to stay in the course, the path that one takes will never be easy, nor paved, but it takes strength of will and support of friends and family that makes the climb all the while and worth it. Though i only keep in contact with few high school friends, i told him that as one progresses in life, friends come and go, new friends and colleagues will be acquired in college, and later in graduate school and even in the professional field. Very close friends are hard to come by, and they are rare jewels that make life all the more pleasant. So , Don, my dear little brother, with your eyes searching for what lies ahead, know that with every action and every motive and every goal in life, there will be a positive and negative consequence. It is just a fact of life.

As a young man, growing in his late teens and approaching his 2nd decade of life, its the simple beauty of life, moving away from his childish past, and becoming more of a handsome young man that I see in his face, body, and eyes, I am very confident in his progression.

Nothing in life is sure, and cemented, my beloved brother, but one thing that we can always count on is this:-- That through all life's difficulties and hardships, the LORD, is and will always be there for you. Through the best of times, the worst of times, the lonely hours etc.

I cannot help but smile in knowing that my baby brother has become and grown into a fine young man.
A good, God-fearing young man.

I am so proud of you, Don Rico. Your Kuya adores you ever so much.

I had a conference call today with some colleagues, we are almost done our group project presentation on medical licenses and a mini lecture on malpractice law and how it affects medicaid processing. Sometimes I cannot seem to find enough time accomplishing tasks at hand while at the same time working on clinical preparations. I cannot wait for the vacation to Bohol in the month to come.

Signed,

Dear Blog,

Is there not enough time to go over clinical terms? The old medical dictionary that my old friend Christina gave to me back in summer 2008 has come useful in preparing my brother for his upcoming courses in biology and anatomy/physiology. I thought it best to introduce to him the basic medical vocabulary and acquaint him to anatomical terminology for his class this coming Fall 2010. He will begin his 4th year of high school this year while my younger brother starts grade 6. Where has the time gone?

Good afternoon,

Today has been forward. Upon waking up i was reminded of my term papers that are due by next week, so i had to get started on those. Systemic reviews and more questions to look into in regards to adrenals. I think later tonight in order to fully cover the systems at hand, i shall make a video on said material and post it in youtube.com and then link it here in tubagbohol.com. This way, i cover the material, and share it as well.

I am going to meet up with colleagues for lunch and discuss work. Then, back to the grind. Carpe Diem!

To God be the Glory!

My day started at 8 am and continued till 4 pm. Work day. I am currently sitting down with a 20 oz cup of hazel nut coffee with French vanilla cream. Currently finishing up muscular disorders, then onwards later to finish neurological treatments and then later tonight to go over with colleagues on treatment protocol for musculoskeletal disorders. Cheers to a good day.

Dear Blog,

I've been getting ready and preparing all necessary materials for my month-long vacation to the Philippines. I still need to bet more books for donation to the local library and finish all necessary materials to be included in the balikbayan box (I hope it won't be overweight).
I still need to get clothes ready and get enough summer clothes for my stay in the Philippines. I still cannot believe that it has been 12 years since I last visited the Philippines.

I should be arriving in Manila International by July 12 (US TIME) and then in Cebu, then to Bohol by Super Ferry. I am indeed excited.

More to be updated..

June 30th, 2010.

I have made arrangements to meet up with my Tita to meet up with me at Cebu International. However, I want to ask her to take me to the Shrine of the Santo Nino before voyaging by boat to Bohol. I hope it will not take too long to visit, but I have a deep inward yearning to visit the Church to the Blessed Santo Nino de Cebu.

So I hear that Cebu has truly changed from friends who visited the province in 2005.
I am very pleased to hear that the region has developed considerably in the last 10 years. I do hope that the continued growth in Mandaue and surrounding cities is uninterrupted, of course, thanks to the works of the DILG Units in the region.

July 4, 2010
11:45 PM,

Today we celebrated the 4th of July here in New Jersey. I took my brothers to my aunt and uncle's Fourth of July Barbecue Party. It was good to see my Uncle Steve and Auntie Beth again after a 3 week absence. I met an interesting friend there, Patrick, who I had a long conversation on music history (involving metal bands, 80's hits etc), and a varied eclectic topics that we lightly touched. I was also able to see Uncle Steve's parents again, who joined us in the barbecue with other friends. I was also introduced to two other physicians there--a couple: one was a radiologist and another was an ER doc. The sights and smell of the event was typically American: fireworks, burgers, hot dogs, coleslaw, rock music, italian dishes, baba ganush, sauerkraut, pasta, warm bread, cake, cookies, beer, soda, and wine to boot.

It was quite a gathering. Happy 4th of July.  :)

I must add that as of this weekend, I purchased some aroma-therapeutic candles from Yankee Candles. I experimented in purchasing the Orange Dream Candle, and lo and behold, it was beyond expectation. I highly recommend this for all aroma-candle fanatics. It's truly worth the amount.

Cheers.

Yesterday I went to the South Jersey Gun Club and fired some rounds with my Uncle Steve. 'twas a nice day to do it since it windy and sunny. We tried the 50 yd range then switched over to the 300 yd range. The moral of the story is: Soviet rifles are indeed rugged, and carries a punch. (My goodness the recoil was a knocker. Or as I told my uncle, "this what i call self-induced bursitis") lol. Can't wait to go out again with him.

Today I finished packing. Received a package that I need to bring back to the Philippines for a relative. I'm planning to visit my Uncle and Aunt later this evening--before my journey back to the Philippines. Earlier this morning I decided to drive around and feel the warm summer breeze of Southern New Jersey. It was too hot outside, unfortunately, and decided to keep my windows down--and blasted my AC on high. There's nothing like driving on Jersey roads in the summer..

 :)

Driving out of my neighborhood subdivision

The 322 West--onward to Blackhorse Pike.

Today I was thinking of my dear little sister who is taking a summer course and on her 2nd semester of Medicine (finishing up her  1st year). I went to her room today and felt a nostalgic longing of my little sister. I miss her voice, her presence in the house, and just being able to take her and my siblings out on weekends when I'm off. When she is free--its taking a break from studies. I just wish that I could see her again soon. Mingaw pood ko ni Inday Angeli. I miss you and Love you, Inday. Praying for you everyday...

I cannot sleep nor can I force myself to sleep. My flight departs in 7 hours. The land of my birth--I shall touch thy sacred, precious shores again.

To my dearest Island of Bohol, home of the most industrious people in the Philippines,--I shall see thee again.

To my family (immediate and extended), whom I will see soon. I cannot contain my smile nor my joy that is abounding.

To the Sacred Holy Infant Jesus--The Blessed Santo Nino--I dedicate everything to you. All for you.

To my beloved kababayans, and bayanis,

I am in the Philippines. I arrived in Manila International Airport at 10:35 PM (Philippine Time).
I am now in the Philippine Domestic Airport terminal 7 waiting for my flight to Tagbilaran City.
My flight leaves at 5:05 PM and I am scheduled to arrive in Tagbilaran at 7:00 AM.

It is surreal. I am finally here in my beloved land of my birth: The Republic of the Philippines.

My heart is so happy. I opened my eyes for the first time in 12 years and saw the lights of Manila City and breathed in the air of the Philippines.

I am Home.

To God be All Glory!
Thanks be to God!

On my flight from Detroit, Michigan to our stop over to Nagoya, Japan--then to our connecting flight to Manila, Philippines--I befriended two other bayanis who helped me in the Manila International Terminal. Thanks to Sharon and Herbert. Thanks for the well wishes, Sharon, and I hope you have a great time in your 3 week vacation in Pampangas. To Herbert, I hope you have closure in burying your father this week--and make it home safely by the end of this week. Thank you for making my flight to the Philippines easier, positive, and filled with laughs and jokes.

God, in his ever loving heart, hears our prayers and thoughts. He sure did make my flight journey fast, and stress-free. I am sure it was with the help of Angel Raphael who kindly heard my little prayer.

My eyes are looking south--to the direction of the shores of Bohol. To my beautiful Chocolate Hills. To my beautiful Valencia...

Truly there is no place like home.  :)

Thanks be to God.

Dear sister Raquel,

Thank you for your kind and helpful advice. I will sure do just that when I arrive in Bohol. 20+ hours on a plane sure does something to the body.

Love and Thanks,

Culture shocked indeed!

I arrived in Tagbilaran City at 7 AM by the 13th of July. My uncle gerard, the head of security at Tagbilaran Airport, saw me as i got of the airplane. I shouted, "'cle Gerard!" He was shocked! We locked in embrace and laughed. After which I saw my Mommy Wella (younger sister of my father, Engr. Rico Lucino), who is the DILG Director in the region. I will then get in contact with the rest of the group as soon as i have time.

I spent most of my day visiting Valencia (back and forth) and Tagbilaran. I have both sides of my family in Tagbilaran and in Valencia.

On the 15th of July I visited the cemetary in Tagbilaran to visit my Uncle Roy, Mama Luz and Papa Titing. The 15th of July was the 15th death anniversary of my Mama Luz. I was able to sweep the grounds of their gravesite, got some flowers and candles for them. My uncles and aunts and cousins came with me for the visitation. I miss them. I was in tears. I was at ease. I could feel peace. Uncle Bayoy was there.

Today on the 16th, I visited Valencia again to visit my Lolo Ciano and repair my truck. Changed tires, and preon for the AC. I also was able to visit my lolo Nating (the late Mrs. Fortunata Tan Salvo) and my other relatives.

I went to Barangay Anas, Valencia. Salvos. Salise. Valencia.

I went to the church of the SENOR SANTO NINO DE VALENCIA. PIT SENYOR!
PIT SENYOR! PIT SENYOR! :)

I am home. It has been so long. Its crazy how alot of people remember me but i do not know them; so many relatives, so many titas, titos, cousins, etc. I am truly at home here.

Bohol. Ang Yuta Na Natawhan.

The past 2 weeks have been filled with so much memory. We had a double celebration on the 23rd; my birthday and my Mommy Wella's. Today is the day that my uncle, Dr. Renee E. Lucino, will arrive to Bohol. He is currently based in the UK. So much to do. I feel as if 1 month in Bohol is not enough. Hoping all is good.

Best,
Lorenzo

Today is Sandugo. I cannot wait. I plan on attending the Sabong today. :)

The places, sounds, and people here in Bohol, Philippines is incomparable. Truly, I love this beautiful Island.

Yesterday I went to Manga, Tagbilaran City to buy fresh fish, and supplies for kinilaw and sinugba. I ordered fresh seaweed for salad and i must say that the sights and sounds of the fish market was overwhelmingly fond. It is inopportune to have an American accent here, they recognize my accent and even with my binisaya, they still manage to target me as an OFW. lol.

That said, i still love the place. Love it.

I visited more relatives, and cannot believe how much I have missed in 12 years of my absence since my last visit in 1998.

I plan on going on a 2nd Bohol drive this weekend. Then manage to stop by for the derby/sabong/sunday school. :)

Early this week,

I was introduced to some young physicians whom my uncles know of. Young women my age who asked of my background. It was good to connect to individuals of the same background. Medicine. Veterinary Medicine. Nursing. Nice.

I was able to befriend a Dr. Omo and Dr. Opo of Ramiro Community Hospital and Tagbilaran Hospital here in this fond little city. I also found out that Dr. Omo was the medical director of my Uncle, Dr. R. Lucino, who is a Surgeon-Internist based in the UK. Interesting how everything intertwines.

Dr. Omo fondly asked me, "Are you related to a Dr. Renee Lucino?"

I replied, "Yes, of course. He is my uncle."

We both shared laughs and a fond discussion.

I am leaving for Cebu City this early afternoon. I should arrive there by 2 pm Philippine Standard Time.

I will be meeting with some of my relatives in my father's side there.

And of course, to check out SM and Ayala. :)

I should be meeting up with some good friends at Cebu Doctors ; some of whom are medical interns.

Next stop: The Queen City of the South.

 ;)

I am glad you were able to visit my dear Manga, Dong!  Manga Public Market is one of the best market places in Tagbilaran where you can buy fresh seafood, veges, meat and kakanin!  Did you buy syakoy there, too?

I am currently in Cebu City. I arrived here on Tuesday 1:40pm and have spent the time exploring the sights and sounds of this beautiful and booming metropolis. Am staying at the Crown Regency Hotel, which I must recommend to all incoming foreign tourists and or OFWs. I spent the good time with my cousin Jing and my baby cousin Daniella touring and swimming in the pools, visiting the cafe shops, and the malls: SM Cebu City and Ayala Mall. Am loving it here.

I should be arriving in Bohol this Friday.

I was able to visit the Basilica Minor to the Blessed Santo Nino De Cebu. My Pilgrimage to the Blessed Child Jesus is complete. My prayers and wish to visit and see Him was granted. Pit Senyor!

Hoping for another good week. Regards to all.

Dong, Dr. Omo is my family's favorite doctor.  He is one kind-hearted doctor.

From meeting him, and from our brief conversation, I will agree. He is caring for my grandfather, as the Internist in Tagbilaran Community Hospital. I went with my Lolo to update his medication regimen as well as the status of his G-TUBE, which I personally changed without needing the help of a surgeon and a surgeon's charge.

The tube I brought home from the states and the service for free. The hospital was going to charge us 7k pesos for a baloon g-tube and 5k pesos for the surgeon to install it. I installed it myself without having to pay such fees.

That said i will have to say that the CRs (bathrooms) in Tagbilaran Community Hospital and Ramiro Hospital have to to be updated. It is unsanitary and can propagate nosocomial infection spreads. I say this as a Medical Intern.

I addressed this to the medical administration there. I hope they can address the situation.

Note:

I want to build a house here in Cebu in the future. This island / city is in the best location.

Cebu has truly modernized and progressed. Love this place.

Dear Pare,

It is my great pleasure to read your daily life activities.  Very interesting, indeed! I was touched..  12 years (on the 13th of July 2010) the most awaited day to breath the missed  Bohol air again. The overwhelming joys meeting your longed old and new relatives in Valencia and Tagbilaran. I could just imagine how you feel being here sharing laughters, attending family gatherings (eat, eat and eating again..our Pinoy favorite traits)  ;D  Full booked  ug busog gjod.  Hope you`ve get enough time to make EB with some of our TB friends who are at the same time here for holidays. Fr. chicogon is desperately calling you in your given tel no. in Tagbilaran. He will be leaving on the 10th of August 2010.
So far, I wish you for the best of your 4 weeks holidays and stay in good health!

lavoooooo!

Today is my last day here in Cebu this week. Yesterday I had breakfast with my cousin Jing at Hebrew's Cafe and then picked up my little cousin Daniella (who is from the UK) and took her to SM City Cebu as well as swimming around. Daniella really amazed me with her British Accent and her spirited nature. Wherever we went to the mall she would always hold her kuya Bran's hand and thten i realized she did not have a bracelet. I got her a lucky jade bracelet in one of the botique stores here in SM City Cebu.
She made me smile how she said that my voice and the way i looked was similar to that of her daddy (my uncle), Dr. R. Lucino.
I realized how i want to be a father in the not too distant future. I would not mind having daughter(s); i think i would probably spoil them with a father's love and protection. (just as a side note!). I am leaving for Bohol today at 2 pm. Am excited to go back to the island.

Let it be known that i miss and love my beloved mother, father, brothers and sister. I miss them dearly.

Love and kisses to my loved ones!!!
With all my all,
B.L
