Employers from the private and government sectors are advised to release the original copy of the Member Data Record (MDR) issued by PhilHealth to their employees.
This is to hasten the compliance of the employed PhilHealth member in filing claim requirements in hospitals and to avoid instances of delay in the processing of their reimbursements.
The MDR is a document required in hospitals beginning September 1 this year for the purpose of availing PhilHealth benefits based on PhilHealth Circular No. 7, s-2007 (as amended by PhilHealth Circular No. 12, s-2007). PhilHealth-accredited hospitals require the MDR for counter-checking of the member or his dependent’s information as written in the PhilHealth Claim Form 1.
All PhilHealth members under the Employed Sector, Individually Paying Program, Non-Paying Program, and Overseas Workers Program are advised to keep the original copy of the MDR, together with the PhilHealth number card (PNC) issued to them upon registration. Employers may keep a photocopy of their employees’’ MDRs for record purposes.
Furthermore, employers are also advised to register their employees with PhilHealth within 30 calendar days from the start of employment. Employers must let their newly-hired employees fill out the M1a registration form and indicate the names in the duly accomplished Er2 form before they are given the MDR and PNC.
When an MDR gets lost or destroyed, the PhilHealth member or a representative can ask for another copy from the membership counter at the PhilHealth Members Assistance Center. Those who make a request for more than four people at one time, shall let their representative bring a letter with the complete names and PhilHealth identification numbers (PIN) of the members being represented, requesting such a reprint.
