Philippine Passport Requirements

To renew your Philippine passport, you'll need to follow these steps and prepare the necessary documents:


Core Requirements:
1. **Confirmed Online Appointment**: Schedule an appointment through the DFA's online system.
2. **Accomplished Application Form**: Fill out the passport renewal application form.
3. **Personal Appearance**: You must appear in person at the DFA office.
4. **Current Passport**: Bring your current ePassport along with a photocopy of the data page.
5. **Original PSA Authenticated Documents**: These are required if there are changes in your personal information, such as:
   - Marriage Contract (if changing your name due to marriage)
   - Annotated Birth Certificate (if there are corrections)
   - Annotated Marriage Contract (to show annulmentorce)
   - Death Certificate of Spouse (if applicable)


### Additional Requirements:
- **For Lost Valid ePassport**:
  - Police Report in English (original and photocopy)
  - Affidavit of Loss in English (original and photocopy)
  - Penalty Fee: Php 350.00
  - Additional 15 business days clearing period


- **For Lost Expired ePassport**:
  - Similar to the requirements for a lost valid ePassport, but treated as a new application¹²³.
