Dear Students of Medicine,

W will further go into detail in Immunological Pathways by focusing on the different kinds of Immune-Related Diseases and Syndromes. We shall explore the different kinds of immunological conditions, the mechanisms of actions, the symptoms of these conditions, and any pharmacologic-surgical modes of treatment available.

This will be extensive, so this will take some time to work on.

All the best.

Inspiration & Hope / Determination
May 20, 2010, 11:57:07 AM
In 1883, a creative engineer named John Roebling was inspired by an idea to build a spectacular bridge connecting New York with the Long Island. However bridge building experts throughout the world thought that this was an impossible feat and told Roebling to forget the idea. It just could not be done. It was not practical. It had never been done before.

Roebling could not ignore the vision he had in his mind of this bridge. He thought about it all the time and he knew deep in his heart that it could be done. He just had to share the dream with someone else. After much discussion and persuasion he managed to convince his son Washington, an up and coming engineer, that the bridge in fact could be built.

Working together for the first time, the father and son developed concepts of how it could be accomplished and how the obstacles could be overcome. With great excitement and inspiration, and the headiness of a wild challenge before them, they hired their crew and began to build their dream bridge.

The project started well, but when it was only a few months underway a tragic accident on the site took the life of John Roebling. Washington was injured and left with a certain amount of brain damage, which resulted in him not being able to walk or talk or even move.

"We told them so."
"Crazy men and their crazy dreams."
"It`s foolish to chase wild visions."

Everyone had a negative comment to make and felt that the project should be scrapped since the Roeblings were the only ones who knew how the bridge could be built. In spite of his handicap Washington was never discouraged and still had a burning desire to complete the bridge and his mind was still as sharp as ever.

He tried to inspire and pass on his enthusiasm to some of his friends, but they were too daunted by the task. As he lay on his bed in his hospital room, with the sunlight streaming through the windows, a gentle breeze blew the flimsy white curtains apart and he was able to see the sky and the tops of the trees outside for just a moment.

It seemed that there was a message for him not to give up. Suddenly an idea hit him. All he could do was move one finger and he decided to make the best use of it. By moving this, he slowly developed a code of communication with his wife.

He touched his wife's arm with that finger, indicating to her that he wanted her to call the engineers again. Then he used the same method of tapping her arm to tell the engineers what to do. It seemed foolish but the project was under way again.

For 13 years Washington tapped out his instructions with his finger on his wife's arm, until the bridge was finally completed. Today the spectacular Brooklyn Bridge stands in all its glory as a tribute to the triumph of one man's indomitable spirit and his determination not to be defeated by circumstances. It is also a tribute to the engineers and their team work, and to their faith in a man who was considered mad by half the world. It stands too as a tangible monument to the love and devotion of his wife who for 13 long years patiently decoded the messages of her husband and told the engineers what to do.

Perhaps this is one of the best examples of a never-say-die attitude that overcomes a terrible physical handicap and achieves an impossible goal.

Often when we face obstacles in our day-to-day life, our hurdles seem very small in comparison to what many others have to face. The Brooklyn Bridge shows us that dreams that seem impossible can be realised with determination and persistence, no matter what the odds are.

Even the most distant dream can be realized with determination and persistence.

A blind boy sat on the steps of a building with a hat by his feet.
He held up a sign which said: "I am blind, please help."
There were only a few coins in the hat.

A man was walking by.
He took a few coins from his pocket and dropped them into the hat.
He then took the sign, turned it around, and wrote some words.
He put the sign back so that everyone who walked by would see the new words.

Soon the hat began to fill up.
A lot more people were giving money to the blind boy.
That afternoon the man who had changed the sign came to see how things were.
The boy recognized his footsteps and asked, "Were you the one who changed my sign this morning?
What did you write?"

The man said, "I only wrote the truth. I said what you said but in a different way."
What he had written was: "Today is a beautiful day and I cannot see it."

Do you think the first sign and the second sign were saying the same thing?

Of course both signs told people the boy was blind.
But the first sign simply said the boy was blind.
The second sign told people they were so lucky that they were not blind.
Should we be surprised that the second sign was more effective?

Moral of the Story: Be thankful for what you have. Be creative. Be innovative. Think differently and positively.

Invite others towards good with wisdom.
Live life with no excuse and love with no regrets.
When life gives you a 100 reasons to cry, show life that you have 1000 reasons to smile.
Face your past without regret.
Handle your present with confidence.
Prepare for the future without fear. Keep the faith and drop the fear.

Great men say, "Life has to be an incessant process of repair and reconstruction, of discarding evil and developing goodness…. In the journey of life, if you want to travel without fear, you must have the ticket of a good conscience."

The most beautiful thing is to see a person smiling…
And even more beautiful is, knowing that you are the reason behind it!!!

Guys,

I want you all to hear the story of Tony Melendez. A man who was born without arms and hands, and only feet. But with an amazing ability to play the guitar with his feet, and performed before the late Pope John Paul II.

Tony Melendez reminds us that our failings and shortcomings should not be a limiting factor, but a means to find our inner strength and tap into it. And help others in the process. His story inspires us....that the LORD always listens and hears our prayers.

MANILA, Philippines â€" Nearly a quarter-century after Imelda Marcos and her dictator husband fled the Philippines in disgrace â€" leaving a debt-ridden country but a lavish collection of shoes â€" the 80-year-old former first lady and two of their children are poised to revive the family's political fortunes.
At first sight, the outcome is surprising in an election that also looks set to award the son of the Marcoses' nemesis, "people power" President Corazon Aquino, the country's top office. Benigno "Noynoy" Aquino III campaigned heavily against corruption â€" endemic in the Philippines and allegedly practiced by the Marcos dictatorship on a massive scale.
But their family name still holds clout.
Imelda Marcos won a seat in the House of Representatives, where she also was elected in 1995, and her eldest daughter, Imee, also a former member of Congress, was elected governor in the family's northern bailiwick, Ilocos Norte province. Her son, former governor and current Congress member Ferdinand "Bongbong" Marcos Jr., likely won his Senate race, according to almost-complete results of Monday's vote.
With Marcos Jr.'s rise to the Senate, the Marcoses would claim their highest nationally elected post since their patriarch was ousted in a 1986 "people power" revolt.
"I thank the Lord, the Ilocanos, the Filipino people for the overwhelming mandate for the Marcoses in spite of all the odds," Imelda Marcos told The Associated Press in a telephone interview Wednesday. "The Filipino people can be assured of our selfless and endless service and love to all."
Marcos is forever remembered for her collection of eye-popping diamonds and 1,220 pairs of shoes discovered in the abandoned presidential palace after Ferdinand Marcos and his family were sent into U.S. exile, ending his 20-year dictatorship and leaving the country's economy faltering under huge debts.
He died in 1989, and his widow returned to the Philippines in 1991 with her children, twice ran unsuccessfully for president and won a seat in the House of Representatives in 1995.
She retained her supporters despite her reputation for extravagance, including shopping trips to the world's poshest boutiques and lavish beautification projects in an impoverished nation where a third of about 90 million Filipinos live on $1 a day.
Despite some 900 civil and criminal cases she has faced in Philippine courts since 1991 â€" ranging from tax evasion to embezzlement and corruption â€" she has emerged relatively unscathed and has never served prison time. All but a handful of the cases have been dismissed for lack of evidence and a few convictions were overturned on appeal.
The Marcoses and the Aquinos are the most prominent of the Philippines' wealthy political dynasties and are inextricably linked.
A court found that Aquino's father, an opposition leader, was assassinated in a military conspiracy during Marcos' rule. Aquino's mother then led the mass protests that swept away the strongman and restored democracy. Only after his mother died last year of cancer did Aquino, a quiet senator and former House member, decide to seek the presidency.
If Aquino wins, "I will pray for his success because his success will be for our country and the Filipino people," Imelda Marcos said.
She said she hoped Aquino will be successful in fulfilling his campaign promise to fight corruption, while she rejected as "lies" allegations that her husband engaged in massive kleptocracy, graft and human rights abuses.
"The Filipino people have not forgotten because even in this campaign they continuously resuscitate the lies about the Marcoses and they keep repeating that, but the Filipino people are getting to know more and more the truth," Marcos said.
Her husband and his associates allegedly amassed an estimated $5 billion to $10 billion in ill-gotten wealth while he was in power. The Presidential Commission on Good Government, created to recover the money, has found cash and assets totaling 85.1 billion pesos (around $1.9 billion).
Aquino, in an interview with AP last week, said as president he would set up a body to determine whether Marcos, a World War II soldier, should be given a hero's burial as the Marcos family has been demanding.
He also said he wants a truth commission formed to bring closure to questions about his father's assassination, including alleged links to Marcos.
Political analyst Ramon Casiple, executive director of the Institute for Political and Electoral Reform, said Imelda and Imee's victories were expected since they ran in their stronghold â€" known as the "solid north" for voters' loyalty to the Marcos family.
But he said many overlooked Ferdinand Jr.'s bid for the Senate as they focused on the presidential race and in the absence of a concerted opposition effort against him. Two leftist politicians who were among Marcos' noisiest critics ran alongside him in the same party, Casiple added.
"Many of the electorate were young. So the main factor was name recall and who would not know a Marcos," Casiple said.

LAOAG CITYâ€"The landslide victory of the Marcoses in Monday’s elections reflects the Ilocanos’ respect to the late strongman Ferdinand E. Marcos, said priest historian Danilo Laeda on May 11, when Ilocanos gave an overwhelming support to the Marcos family in this year’s historic automated elections.

The 80-year-old former First Lady, Imelda Marcos, won as congressman of Ilocos Norte’s Second District with a total of 109,571 votes against rival candidate and former Marcos ally, lawyer Mariano Nalupta, who got a measly 27,359 votes.

Her daughter, Maria Imelda “Imee” Romualdez-Marcos, won as governor of the province with a total of 196,160 votes over her first cousin, incumbent Ilocos Norte Gov. Michael Marcos-Keon, who got only 86,005 votes.

Senatorial candidate Ferdinand Marcos Jr. is among the top 10 winning candidates based on latest election result as of May 11.

“Filipinos are passionate, and emotions run high,” said Laeda, which means people of Ilocos Norte still think of the late President Marcos as a source of pride amid his history of being a dictator and being deposed from the presidency through a people power revolt in 1986.

The “Marcos magic” that the priest believes to have played a great factor on the Marcoses winning in this year’s election shows that the family maintained its influence in the province having the necessary logistics, machinery and campaign funds.

Though the Parish Pastoral Council for Responsible Voting (PPCRV) in Ilocos Norte received reports of massive vote buying, Laeda said voters here have not yet reach a high level of maturity, considering that the old system of “corruption” is unabated or hard to eradicate.

Francis Bitagon, chairman of the PPCRV here, said voters openly admit that they receive money from candidates, ranging from P200 to more than P1,000, which is given to them mostly by local village officials or political party coordinators on a staggered basis. The term is popularly known locally as first wave, second wave and final wave, when the money was handed over to voters.

The PPCRV admitted that the “culture of corruption” is already embedded into the system, and sees no cure unless the younger generation is educated on electoral reform.

While giving thumbs-up to the Commission on Elections (Comelec) for the successful and peaceful conduct of automated polls, the PPCRV hopes that some glitches and long lines of voters due to voters’ registration verification will be properly addressed next time.

PPCRV volunteers said the automated polls  made their work easier.

Bible Study / Re: What Is the Recession For
May 18, 2010, 01:05:53 AM
Brothers and Sisters, even if the economic recession is at hand, we are reminded, again, that it is by Faith that we are given strength to overcome any obstacle, be it financial, health uncertainty, and familial woes. The Lord will never abandon us, never, ever. On the contrary, He will Deliver!

There are two beautiful and inspiring bible verses that help me moving forward and fighting on, in Jesus' Name. These two verses, which I will share with you all, is Philippians Chapter 4, Verse 19 and John Chapter 14, verse 1.

Philippians 4:19, "And my God will meet all your needs according to his glorious riches in Christ Jesus."

and,

John 14:1, "Do not let your heart be troubled;believe in God, believe also in Me."

Jesus tells us that we should believe in Him, and Trust in Him, for He promises us in in Hebrews 13:5 that The Lord God will never leave you nor forsake you.

When the going gets tough, brothers and sisters, our faith should shine in the Promises of Christ Jesus. AND His greatest promise to us, friends, is that He will never leave us. Till the end of the world, still, He will be there for us.

~

God Bless All of Us.
All of Us.
Amen.

Savior, please take my hand   .
I work so hard, I live so fast.
This life begins, then it ends.
And then I do the best that I can,
but I don't know how long I'll last.

I try to be so tough,
but I'm just not strong enough.
I can't do this alone, God I need you
to hold on to me.
I try to be good enough,
but I'm nothing without your love.
Savior, please keep saving me.

Savior, please help me stand.
I fall so hard, I fade so fast.
Will you begin right where I end?
And be the God of all I am because you're all I have.

Hallelujah!
Everything you are to me
is everything I'll ever need.
and i am learning to believe
cause you're the one who's saving me.

Technically, Yes, that is right. The mnemonic is DABDA (denial, anger, bargaining, depression, acceptance).

It's interesting to know tho that not all dying patients go through all of the stages of dying; some skip certain phases, some alter stages, and some die while being stuck in one particular phase. These stages are essential in helping one cope with death, and understanding the effects of the dying process.

There are some patients that go through depression, and then denial of the event, until the very end. The 5 stages of dying (Kubler-Ross Model) applies in particular to terminal patients and does not apply for patients who die of acute conditions or due to complications from a minor surgery etc.

thanks... maybe because i am very realistic and i dont let my emotions get in my way of thinking. i lost 2 people that i love because of cancer. my fully recovered grandpa suddenly died. a very good friend of mine was ambushed. my grandma was sleeping besides me and i didn't even know she was already dead for i dont know how long....these made me stronger and have accepted that life is short and if my time will come, it will come..

There are many stages of dying:

a. anger
b. denial
c. bargaining
d. depression
e. acceptance

Some people go through these stages, some do not, one thing is clear tho...when one accepts it, either during the dying phase, or before it, it allows a mental clarity that can allow proper judgement on what course of action is to be done.

These certain circumstances, have , indeed, made you stronger...to the benefit of your own judgement. Again, keep on being strong.

Sweet radiance of the glow of your light brown eyes
that pierces not only my eyes, but the beating heart of my soul,
Soft kisses that are mine , are given to you
Soft touch of my hand, feel the gentle rub of fingertips

Sweet talks and sensation of passion

Feel the ecstasy within my heart

For you i shall wait,

my view in this issue is that it shouldn't be banned for personal reasons. if this will happen to me, i would rather die immediately than burden my family with financial problems and heartaches. i have discussed this issue with my husband and we have the same point of view. in the case of cancer, stages 1 and 2, treatment will be sought but if its already in stages 3 and 4, never mind!

A respectable point of view. Tho i must say, Anne, i have heard cases of patients who were in stage 4. The patient did make a complete recovery after a full bone marrow transplant.

The point of financial cost is understandable and you do have a point. Thank you for sharing your view. Mo respect ko ana.

lol.

Virgil, your post made me reminisce a conversation I had with some friends who were muslims and jews. Jews (strict orthodox) eat only kosher; while muslims (conservative) eat only halal. I asked them what was the difference once, and it is the manner in how they kill the animal and offering in prayer for the animal to YWEH or to Allah. The way they kill the animal is by cutting the carotid arteries, which allows proper disposal of the blood and the quickest way possible (therefore the animal suffers only briefly). It is the manner in which the animal is killed and prepared for consumption that differentiates. Actually the way some butcheries kill bovine species is considered animal cruelty.

Perhaps we can start another topic on this. What say you, ladies and gents?

Quote from: islander on May 13, 2010, 11:03:49 PM

lorenzo,

the way your mind works is truly awesome.  you don’t only miss the mark, you’re also cross-eyed.

our topic is the context of dreaming.  you zero in on one aspect like existentialism to prove what?  that you know something that any idiot can easily cut and paste from internet sources?  don’t steer the discussion to that which you can cut and paste (these are obvious because the syntax and grammar are different from your own lapses).

dear lorenz, you even give wise advice to me and others “to differentiate between nihilism and existentialism”!  goodness gracious.  here’s my own advice:  if you’re going to be a doctor, better start disciplining the way you think or, heaven forbid, you might treat a patient for syphilis when in fact he only has hemorrhoids.

okay, here’s something for your “outside the box” (shoe box?  manny pacquiao box?), cut and pasted as well:

Most existentialists have argued that we aren't born with a fixed human nature which forces us to act in certain ways and prevents us from acting in other ways. Instead, what we usually see as our "natures" is actually a product of our choices — sometimes even choices we don't realize we are making.

Thus, Darwinian evolution provides some scientific credibility in more than one way to the existentialist position that humans make themselves and remake themselves during their struggle to survive in their day-to-day lives. Not all existentialists are necessarily "staunch Darwinians," however. Although it would be a rare existentialist who rejected the truth of evolutionary theory, there are those who don't regard it as having any real bearing on their philosophy.  (emphasis mine)

i’m sorry to say that even this claim is unimpressive as it is striking more as an affectation of the intellect, like muddling the waters to see the bottom or wearing high heels with the miniskirt to give the illusion of long legs.   besides, darwin’s theory remains that, a theory.

with all your religious posts here at tb, you now seem to lean on the side of agnosticism and atheism by calling on existentialism and nihilism.  how is that?

as for wine, my husband is the oenophile, not me.  to be honest, i hate his collection of vintage reds, some dom perignons, including a smattering of liquors like hennessys and single malt whiskies side by side with his cigars in humidors, the total cost of which to my simple mind is the cost of a car.  i must admit though that on rare occasions I go for simple good, old bailey’s and a mateus.

awed at your mish-mash but glad that reading you brings out the nihilist in me,

Hahaha, such vivid spirit you have my dear lady,

The context of dreaming never was mitigated by my posts, rather I showed respect to our dear Hubag's preference to dream, and i must say, admit really, that i admire the nonconformity.

Dearest, i merely found the beauty of wolf's post and provided my view of how it blends existentialism to the concept of the survival of the fittest, in reference to wolf's using of brute force as being a tool for the weak and the strong. Actually, i would credit Wolf for that beautiful post.

In regards to Existentialism in its relation to Darwinian Theory of Evolution, I rather find it interesting how it coalesces. I must also add that one's faith does not mean it should limit one to read about different concepts and philosophies. Afterall, that is the beauty of discernment. :)
Again, it goes back to the notion of choice. hehehe! (subject matter in mind, haha).

``

One of my favorite Pinot Noir brands is Cono Sur (Chilean); the taste of red is rather unique to the California or French based due to the soil in Chile as well as the types of casks they use, the casks they use are old whiskey cherry wood casks. You can actually taste a hint of whiskey and the aroma of cherrywood via nasal discernment. One of my favorites.

Respecting your nihilism, but still keeping my youthful existentialist spirit,

Love you,
;)

ps. One can use differential diagnosis to rule out hemorrhoids from syphilis, but a discerned physician/intern will know that there are some STDs that have clinical symptoms of hemorroids, especially if the site of infection was in the anal canal, then can be easily mistaken for hemorrhoids. Since diagnosis does not rely solely on physical diagnosis (IPPA- inspection, palpation, percussion, auscultation), but rather lab diagnosis (PCR, gram stain, Lowenstein-Jensen, Maconkey's, DF, IF etc). And when the results are accumulated, would the proper choice to be made. In either case, RX for syphilis is : A) penicillin or 2nd -3rd generation Cephalosporins.  RX for hemorroids is non-invasive hemorrhoidectomy.

alternative to Syphilis RX: if patient is refractory to penicillin (due to allergy), then we would treat the patient with erythromycine + an adjuvant probenicid.

and that, precisely, is what i mean by being off the mark, which makes me suspect that maybe, just maybe, you didn’t really get bai hubag’s drift.

you presumed hubag’s inability to reason, his disinclination to reason, and topped that pudding off with the cream of declaration that to dream is to live one’s fantasy, as if that is all there is to dreaming.

first, we’ve got to give it to hubag who might not be able to write that way if his reasoning were not acute.

second, dreaming in the context of hubag’s write-up is, in hyperbolic terms just so I could get my point across, in the league of world conquests.  conquerors dream of conquests and dream up and visualize strategies to win wars.  they don’t go to wars because of their brawns alone, much less because of their reason.  they go to wars because of their what-ifs.

to dream thus is also to invent, to devise.  great poets, philosophers, and scientists dreamed.  and changed the world.  no one can question their ability to reason beyond ordinary mortals.

the dreamers that bai hubag refers to actually have their reason intact.  this, one can derive by reading between his lines, which entails the reader to go beyond one’s self.  as for his statement about dreaming his life away, again, context, as it is obviously said tongue-in-cheek.

after your presumptions (or should I say conclusions?) that hubag is far from reason, you go on to medical school to explain what extreme dreaming is and call on european enlightenment (because it was the age of reason?).  i as a reader felt like I bungee jumped without the ropes.

oh, martin luther’s “i have a dream…” is far from schizophrenic.  and we know his role in defining u.s. civil rights.

allow me please to go further, to your reply numbered 62 in this same thread.  you mention wolf’s post as existentialist, and then relate this to darwin’s theory of evolution.  what?  one has nothing to do with the other, so i was confused.  what, indeed, is existential about survival of the fittest when existentialism is all about denying that the universe has any built-in meaning or purpose and therefore individuals have to take responsibility in shaping their own destinies?

what i’m trying to say here is that reading anything, like writing anything, needs contextualization and the proper grasp of concepts that one brandishes.  we have to go beyond the literal to be freed from the traps of the nuances of language.

i have nothing against youth’s knowledge and understanding.  these are gifts to the world.  but there is such a thing as mental indigestion that only the passing of years can cure. even child musical prodigies practice themselves to greatness.   thankfully, the mistakes youth makes are corrected through the years.  often.

at 40, you may look back to when you were 20, and laugh.  knowledge needs to be placed in the context of life as it is lived.  and, like good wine, it takes time.

love you too.

Isles,

the definition of existentialism was perfected by  Søren Kierkegaard (father of existentialism) who said that it is the individual that is soley responsible for giving meaning to their life by defeating existential obstacles. Wolf's input about using force as a means to defeat an existential obstacle (ie, angst, despair, anger, boredom, mental or physical osbstacles). I think that existentialism can blend in with notion of survival of the fittest, as it can give a source of legitimacy for surviving obstacles, a sense of purpose so to say.

It is essential that you and others differentiate existentialism from nihilism as they both are distinct philosophies.
It is only recently when F. Nietsche linked both in existential nihilism that both philosophies have been unfairly linked together; when in reality, their philosophical tenets are different.

"Existentialists believe that one can create value and meaning, whilst nihilists will deny this."

In regards to Wolf's comment, I draw connection to his comment with the works of Austin Cline's "Evolutionary Theory and Existentialist Philosophy". It links both existentialism with the theory of evolution. Something outside the box.

~

On a side note (outside the frame of this discussion), I must say that i do agree with you that aged wine is indeed exquisite. I do prefer a nice bottle of Pinot Noir, aged 6 years of more; or chenin blanc, aged 9 years or over. What is your favorite?

d. dissociation

Usually, this disorder is undiagnosed in men.

Those who are more deadly and dangerous psychos are those that pretend to be normal..they are like wolf in sheeps clothing ready to devour ther next victim..

This is very true; an example of this is Jeffrey Dahmer, who was a homosexual serial killer; he was actually an introverted individual that displayed an anti-social disorder with schizoid-like mood. In fact, in medical forensic psychiatry, they have booklets that describe the characteristics of these individuals, and always make an example of Dahmer. Individuals like these like to appear normal in quick gatherings and in brief encounters. But believe me, if you had him sit down with you for counseling, it would be obvious that he had some kind of dramatic/emotional / eccentric mood that would implicate some kind of disorder.

Individuals like these live in fascades. It is as you said, Virgil, an example of wolves in sheep's clothing.

During an extern rotation at a local hospital psychiatric unit, two of my colleagues and I had to treat 2 patients that were diagnosed with Schizophrenia (catatonic and dissociative type). Both  of the patients were charged with some kind of assault years prior.
