Histological slide of a patient with diabetic glomerulosclerosis w/ nephrotic syndrome:

#18912
Nephrotic syndrome

Nephrotic syndrome is a group of symptoms including protein in the urine (more than 3.5 grams per day), low blood protein levels, high cholesterol levels, high triglyceride levels, and swelling.

Nephrotic syndrome is caused by various disorders that damage the kidneys, particularly the basement membrane of the glomerulus. This immediately causes abnormal excretion of protein in the urine.

The most common cause in children is minimal change disease, while membranous glomerulonephritis is the most common cause in adults.

This condition can also occur as a result of infection (such as strep throat, hepatitis, or mononucleosis), use of certain drugs, cancer, genetic disorders, immune disorders, or diseases that affect multiple body systems including diabetes, systemic lupus erythematosus, multiple myeloma, and amyloidosis.

It can accompany kidney disorders such as glomerulonephritis, focal and segmental glomerulosclerosis, and mesangiocapillary glomerulonephritis.

Nephrotic syndrome can affect all age groups. In children, it is most common from age 2 to 6. This disorder occurs slightly more often in males than females.

What determines GFR?

-Except for an unusual situation when plasma protein concentration changes dramatically or renal obstruction develops, the main factor determining GFR is glomerular capillary pressure.

-An increase in capillary pressure increases GFR, and a decrease in capillary pressure decreases GFR.
#18939
Hi guys. Let's talk about Renal Physiology and the clinical significance of it all. :)

CLINICAL ESTIMATION OF GFR AND CONCEPT OF CLEARANCE

GFR= Glomerular Filtration Rate
CLEARANCE = Refers to a theoretical volume of plasma from which a substance is removed over a period of time.

-Estimates of GFR are used clinically as an index of renal function and to assess the severity and course of renal disease

-A fall in GFR means the disease is progressing , whereas an increase in GFR suggests a recovery

-In many cases a fall in GFR may be the first and only clinical sign of renal dysfunction

-Estimation of GFR rely on the concept of clearance

Your Help Cometh From the Lord

So I share my story to encourage someone today. Your help does not come from what you have or don’t have nor what you know or don’t know. Your help comes from the Lord, the Lord which made Heaven and earth (scripture verse: Psalm 121:2). God said in His word that He will supply all of your needs according to His riches in glory and we know we serve a God who cannot lie.

It is by the grace and merciful hand of God that my family and I had the funds to pay rent each week with NO INCOME. Even if I wanted to, I could not take the credit for it because the little money I did make was no way near the money needed each week to pay the hotel fees. So I know it’s only God who has been sustaining my family and I.

So as I encourage myself, I encourage you. You may not know where the next dollar, meal, drink, or etc is coming from. You may want to panic and give up. You may be tempted to take matters into your own hands. However, I encourage you to stand on the word of God and His promises because your help comes from the Lord!

If God can send ravens to feed Elijah, then surely God can use anything to be a blessing to you. Remember, your help comes from the Lord (bible verse: Psalm 121:2). Obey God and leave the when, how and where to Him. Your Heavenly father is a present help in times of trouble and He will not leave you hanging.

Inspiration & Hope / My Help Comes From the Lord
October 02, 2011, 12:29:25 PM
How many times have you taken it upon yourself to get things done when you knew God was telling you to stand still and/or wait on something?

I’ll be the first to raise my hand in saying I’ve done so. Right now in my life, I’m in this place where every week my family and I are faced with the challenges of paying hotel rent. I’m in the process of looking for employment…but I’ve tried to take matters into my own hands, thinking that I’ll “help God” because I can’t figure out what He’s doing.

Today, the very thing I placed my trust in to make some extra money was cancelled out. My first reaction was to panic and get upset. But you know what, after I prayed I am reminded that my help does not comes from what I, Naquan can do, but from what God can do (and we know that God can do all things).

Why do I share this with you? Because I know I’m not the only person who’s been tempted to step outside of faith and take matters into their own hands. Now, I’m not saying that you’re not to do anything to make a living…however, in the situation that God has ME in, it’s a faith walk and sometimes the faith walk is not the most easiest walk.

What’s next?

As you walk with Christ, know that the enemy will accuses you day and night, but Jesus is interceding on your behalf. Whenever you fall short of the glory of God, repent â€"ask God for forgiveness, receive it in Jesus Name and know that God has forgiven you. God tells us in His word that He is faithful and just to forgive us of our sins, to cleanse us from all unrighteousness and remember our sins no more. Confess, repent and receive your forgiveness in Jesus Name. After you asked the Lord for his forgiveness, don’t continue to dwell on your mistakes. If God remembers your sins no more after you repent, why should you?

When the enemy comes and condemns you, the best weapon you have to defeat him is the Word of God. I like to use Romans 8:1, however there are many others to use such as Romans 8:34, which reads: “who then will condemn us? No oneâ€"for Christ Jesus died for us and was raised to life for us, and he is sitting in the place of honor at God’s right hand, pleading for us.” God knows that we come short and sin; however, when you repent and ask for forgiveness, your sins are no longer attached to you. Remember, wherever the Spirit of the Lord is, there is liberty and freedom.

Conviction

John 16:8-11 states the Holy Spirit will reprove the world of sin, and of righteousness, and of judgment:

   - Of sin, because they believe not on me;
   - Of righteousness, because I go to my Father, and ye see me no more;
   - Of judgment, because the prince of this world is judged.

Let’s break this down. The Holy Spirit does not convict believers of their sins. The Holy Spirit does however convict non believers of their sins because they do not believe in Jesus Christ. Although the Holy Spirit does not convict believers of their sins, it does however convict believers of righteousness.

The Holy Spirit has an assignment and part of that assignment is to guide the believer and bring glory to the Lord by revealing things to them. The Holy Spirit is the believer’s comforter and counselor and it’s to remind you of all the things God teaches us in His word. As humans, we at times stray away or fall short of the glory of God. God knows this which is why He sent the Holy Spirit to dwell within you. As the Holy Spirit leads you and guides you onto a path as righteousness, when you fall short or stray away from that path, you are convicted. The conviction happens to help you back onto the path of righteousness (right standing before the Lord). It does not happen to criticize and accuse you of your sins. Revelations 12:10 tells us that the enemy is the accuser as he goes before the Lord and accuses day and night.
#18958
Condemnation

Condemnation is a tool used by the enemy to blame, citizen and belittle you. Condemnation emphasizes on the sins you’ve done and seeks to make you feel as though you’re a failure to God. In John 10:10, the Bible states that the enemy comes to steal, kill and destroy. What better way to go about doing this than to condemn people of their short comings and convince them to be failures? If the enemy can condemn people and make them believe their failures, they may withdraw from God and slip back into a world of sin. Although this is the objective of the enemy; I have good news, there’s no condemnation to those who are in Jesus Christ (Romans 8:1). John 3:17 states “God sent not his Son into the world to condemn the world; but that the world through him might be saved.” God is not a condemning God nor is He a God who condemns you of your shortcomings.
#18959
Have you sinned or fell short from the glory of God and felt:

    - Small
    - Unworthy
    - Ashamed
    - Worthless
    - Belittled
    - Insufficient
    - Incapable of pleasing God
    - Non deserving of God’s presents or purpose for your life because of your down falls

Do your short comings replay over and over in your head? What you’re feeling is known as condemnation and I assure you many others have felt this too.

All of us exist because of God , all the universe exists because of His Will. So He is in all things, and all things are because of  Him. Everything was created because of  His purpose through which He alone deigns it. The Supremacy of God touches all of us , who are insignificant specks compared to the Power of Divine Providence, in this fact: That He came down from Heaven, took form of man, chose to be born by a humble maiden, suffered for us even unto the point of death. By dying for us he cleansed our sins, and shows to us how much such a Powerful and Magnificent Creator can love those whom He has created.

The U.S. sent its special envoy for Afghanistan and Pakistan to the region as politicians in Islamabad rejected U.S. allegations their country is aiding guerrilla attacks in Afghanistan.

Marc Grossman also will visit India, China and Central Asia in preparation for a conference on Afghanistan in Turkey in November, the State Department said in Washington, without providing a detailed itinerary.

A combined Afghan and coalition security force captured a leader of the militant Islamic Haqqani network last month, according to NATO. The capture of Haji Mali Khan could provide valuable information about the Haqqani network’s operations in Afghanistan and perhaps also about its links to Pakistan’s military and intelligence services, said a U.S. intelligence official.

According to NATO, Khan was detained during a security operation in Jani Khel district, Paktiya province, on Sept. 27. He is “the senior Haqqani commander in Afghanistan” and the uncle of Siraj and Badruddin Haqqani, the NATO statement said. He was responsible for managing bases and oversight of operations in both Afghanistan and Pakistan, the statement said.

The Haqqani network was formed in the 1980s with support from Pakistan, the U.S. and Saudi Arabia to battle the Soviet occupation of Afghanistan. It’s now based in Pakistan’s North Waziristan and considered the most dangerous ally of al-Qaeda and the Afghan Taliban, blamed for attacks on the American embassy in Kabul, the Inter-Continental hotel there and other U.S., NATO and Afghan government targets.
Strained Relations

Already strained relations between Washington and Islamabad might be complicated by new intelligence from Khan on Pakistani support for Haqqani attacks, the intelligence official said, speaking on the condition of anonymity because intelligence matters are classified.

Admiral Mike Mullen, who retired Sept. 29 as chairman of the Joint Chiefs of Staff, said last week the Haqqani Taliban faction “acts as a veritable arm” of Pakistan’s Inter-Services Intelligence Directorate. Pakistani government leaders said the claims “are without substance and derogatory.”
Alienating Pakistan

“Public recrimination of an ally is not the answer,” Pakistani Foreign Minister Hina Rabbani Khar said in an interview with CNN’s “Fareed Zakaria GPS” program, scheduled for broadcast today. “As foreign minister, my concern is you alienating the people of Pakistan. You cannot afford to do that.”

Pakistanis “are the ones who have sacrificed the most,” Khar said. “We are the ones who are fighting it on the ground on the daily basis.”

“We need to be seen as a worthy partner,” she said on the CNN program.

Pakistani Prime Minister Yousuf Raza Gilani hosted political parties, including Islamic opposition groups, in Islamabad last week in a show of unity after U.S. charges that Pakistan-based insurgents struck American targets in the Afghan capital, Kabul, last month.

The Pakistani politicians called on Gilani to seek talks with militants in the country’s tribal regions near the border with Afghanistan. The militants have been targeted by army offensives for more than two years.
Failed Negotiations

Earlier negotiations to convince militants to end violence have failed. The army began campaigns in South Waziristan and the Swat Valley in 2009 to weaken the Pakistani Taliban. The group had aligned itself with al-Qaeda and carried out attacks and suicide bombings, killing thousands of Pakistani civilians and members of the security forces.

Grossman also will visit Kabul after the Afghan government said yesterday it may suspend efforts to work with Pakistan on a process to end the war in Afghanistan because no progress has been made.

Afghanistan may work more closely with the U.S., Europe and India instead of trying to negotiate with Taliban groups based in Pakistan, President Hamid Karzai said in a statement issued by his office.

The Afghan statement said Karzai met with government and security officials to assess Afghan policy after a suicide bomber killed Burhanuddin Rabbani, the head of the government’s High Peace Council, on Sept. 20.

“In spite of three years of negotiations and efforts to make peace and good relations with Pakistan, the Pakistani government has not taken any steps to eliminate terrorist sanctuaries or prevent Taliban military training and armament on its soil,” the statement from Karzai’s office said.

Talks involving Pakistan, Afghanistan and the U.S. have been “useful,” Nuland said Sept. 30 in Washington. Grossman will discuss the value of the process and “see where we go” with officials in Kabul and Islamabad, she said.

Is she your aunt, Bran? Congratulations to her on her new appointment!  ;)

Yes, Fr. Roel. Mommy Wella is my aunt. She's my dad's younger sister. :)

Prior to being DILG Provincial Director of Bohol she was the Head of Commercial Development in Mandaue City, Cebu Province.

When I visited Bohol-Cebu-Leyte last year in 2010, I was given a tour of the DILG Regional Office in Metro Cebu City. Kuyaw kaayo ilang processes and quite vibrant ilang initiatives sa local government building. Most importantly, very humbling to see that women are very capable in heading governmental positions in the Philippines. It beats the stigma that men are supposed to be on the top. :)

Sa una sa bata pa ko, mo adto gayod mi sa Badiang with my Lolo and Lola. Kami tanan nga apo mo uban mi sa among Lola og Lolo. My earliest memory of Badiang is when Lolo Ciano took myself, Joseph, Jan-Jan, Christian, Angel, and Forlyn to the falls and wash all of us with shampoo and soap. Ug mo gawas mi sa tubig, hapakon gayod mi ni Lolo. Afterwards, Lolo would give each of us a kiss on the forehead. Maka lipay og maka hinomdom ko adtong panahona. :)

Poems and Poets / An Ode of the Birth of our Savior
October 01, 2011, 12:59:29 AM
by Robert Herrick

In numbers, and but these few,
I sing Thy birth, Oh, Jesu!
Thou pretty Baby, born here,
With sup'rabundant scorn here:
Who for Thy princely port here,
Hadst for Thy place
Of birth, a base
Out-stable for Thy court here.

Instead of neat inclosures
Of interwoven osiers,
Instead of fragrant posies,
Of daffodils and roses,
Thy cradle, kingly Stranger,
As Gospel tells,
Was nothing else,
But, here, a homely manger.

But we with silks (not cruels),
With sundry precious jewels,
And lily-work will dress Thee
Of clouts; we'll make a chamber,
Sweet Babe, for Thee,
Of ivory,
And plastered round with amber.

The Jews they did disdain Thee,
But we will entertain Thee
With glories to await here
Upon Thy princely state here,
And more for love, than pity.
From year to year
We'll make Thee, here,
A free-born of our city.

22Do not rob the poor because he is poor,
         Or crush the afflicted at the gate;

23For the LORD will plead their case
And take the life of those who rob them.

Inspiration & Hope / The Boulder In Our Path
October 01, 2011, 12:49:22 AM
In ancient times, a king had a boulder placed on a roadway. Then he hid himself and watched to see if anyone would remove the huge rock. Some of the king's wealthiest merchants and courtiers came by and simply walked around it.

Many loudly blamed the king for not keeping the roads clear, but none did anything about getting the big stone out of the way. Then a peasant came along carrying a load of vegetables. On approaching the boulder, the peasant laid down his burden and tried to move the stone to the side of the road. After much pushing and straining, he finally succeeded. As the peasant picked up his load of vegetables, he noticed a purse lying in the road where the boulder had been. The purse contained many gold coins and a note from the king indicating that the gold was for the person who removed the boulder from the roadway. The peasant learned what many others never understand.
Every obstacle presents an opportunity to improve one's condition.
