2. Duly accomplished Personal Data Sheet (CSC Form 212), which may be downloaded from the CSC Website (
www.csc.gov.ph), indicating all the required data information;
3. Authenticated Certificate of License;
4. Certified true copy of Training Certificates;
5. Certified True Copy of Certification of Sea Service;
6. Latest Clearances;
a. NBl Clearance
b. Police Clearance
c. Barangay Clearance
7. Birth/Marriage Certificate (PSA Authenticated);
8. Notarized certification (not employed nor connected with any MTls, MHEls, Review Center, Practical assessment centers).
Only those applications with complete documents will be considered.
