Taking thyroid cancer seriously
 
With the increasing incidence rate of thyroid cancer in Metro Cebu, the Eduardo J. Aboitiz Cancer Center (EJACC) of the Ramon Aboitiz Foundation Inc. (RAFI) urged the public to undergo early screening and prevention measures to fight the disease.

“Thyroid cancer is high on prognosis. It is preventable through various screening methods, such as physical examination and ultrasound of the neck, as well as some prevention measures, including regular exercise and maintenance of a well-balanced diet,” Ronald delos Reyes, EJACC program coordinator, said.

Based on EJACC’s Metro Cebu Population-based Cancer Registry, the incidence rate of thyroid cancer in 2003 to 2007 is high but the mortality rate is comparatively lower.

Within this period, about 573 cases of thyroid cancer were reported and 116 were recorded to have died of the disease. The figures also show that 97 cases have been attributed to persons in between the ages 15-30 while 350 cases are persons with ages 30-60. More women than men have been affected by this disease; in fact, 134 cases are male while 439 cases are female.

As a common type of cancer diagnosed in men and women, thyroid cancer ranked sixth in most common cancer cases in Metro Cebu.

According to the 2010 journal of the Philippine Cancer Facts and Estimates of the Philippine Cancer Society, the incidence rate had increased from 1980 to 2002, with an annual change of 0.4% in males and 1.6% among females.

Thyroid cancer is the most common cancer of women at ages 15-24 years. Among women, the incidence rate rises at age 30 and continues to rise with increasing age. Among men, the incidence rate begins to increase much later, starting at 60 years old.

The thyroid is a butterfly-shaped gland located in the front of the neck just below the Adam’s apple. It works like a tiny factory that uses iodine, mostly from the diet in foods like seafood and salt, to produce thyroid hormones that, in turn, help regulate the body’s growth and metabolism and other functions of the body.

The most common signs and symptoms of thyroid cancer include a lump or thyroid nodule, that can be felt in the neck; trouble in swallowing; throat or neck pain; swollen lymph nodes in the neck; cough; and vocal changes.

People exposed to high levels of radiation are much more likely than others to develop papillary or follicular thyroid cancer. Treatment with x-rays is one significant source of radiation exposure. Between the 1920s and the 1950s, doctors used high-dose x-rays to treat children who had enlarged tonsils, acne, and other problems affecting the head and neck. Later, scientists found that some people who had received this kind of treatment developed thyroid cancer.

People who are diagnosed with this kind of cancer may undergo surgery. Common surgeries include thyroidectory, lobectomy, and tracheostomy. Sometimes, even the removal of the thyroid itself is needed.

“Like many other cancers, thyroid cancer may be more easily cured with early detection. Earlier diagnosis removes thyroid cancer at a time when they are not likely to have spread beyond the thyroid gland,” delos Reyes said.

It was a cold nodule, they took it (major opera) and cleaned it off (ingon pa sa doctor) then they did a nuclear radiation after a almost 3 months kay nadugay man kog balik from RP. Bale 3 days pod sa hospital. Since I lost my voice in the process ila ko gitaan ug medical leave for 5 weeks mao niuli ko sa Philippines to recuperate. And that was the last time I saw my house and personal belonging because while in the Philippines, our most unwlcomed guess to date visited us: Hurricane Katrina and wiped everything I had. The year was 2005.

Oh, by the way... OK na ko karon. On synthroid med for life, to control or balance my hormones.

Thanks for sharing that Father. To explain to everyone here, Father had a surgery called 'thyroidectomy", which is a surgical removal of all, or parts of the thyroid gland. In this case, the thyroidectomy removed the Thyroid Cancer (which comes in three distinct forms: papillary, follicular, and medullary).

Normally, the thyroid gland is responsible for secreting thyroid hormones (T3, T4) otherwise known as thyroxines that are responsible for the regulation of body metabolism and hormonal balance. With the loss of the thyroid gland, the body no longer was able to secrete the Thyroid hormones (T3, T4, TSH), and thus, one must be on regular thyroid hormone replacement for life for the maintenance of metabolism.

Medicine has created synthetic thyroid hormones, otherwise known as Sinthroid or Xinthoroid, through the Human Genome project, which successfully mapped the some 100,000 human genes and their location on the chromosomal DNA. Because we now know the composition of said DNA and their location, we are now able to make "synthetic" versions of these hormones in vitro and apply it in vivo.

Another example of "synthetic" hormone replacement is the creation of "synthetic insulin". Patients who are unable to secrete insulin (Diabetes Mellitus type I or extreme form of Diabetes Mellitus type II) cannot survive without insulin. However, because of the successful Human Genome Project, Medical Scientists are able to create a "synthetic" form. Now, people that normally could not survive without said hormones can survive and live a normal life.

As we speak, there are Medical Scientists who are researching on the proteins that regulate cancer growth, linking specific mutated proteins that are responsible for neoplastic growth (the genesis of cancer). Medical scientists are actually working on creating synthetic inhibitors that can theoretically inhibit cancer growth, and possibly even reverse the effects. To some extent, we have succeeded in the creation of some drugs that can effectively inhibit further cancer growth through the inhibition of proto-oncogenes and the regeneration of deleted TNF (tumor necrosis factors).

Some time ago, I had a patient who was in advanced age. When I was taking her history, I asked her, "Do you take any medications?"

She answered back, "Oh, I take plenty of them, Doctor. You want to see them?"

As I was filling my notes, this very old woman took all of her capsules out of her bag and placed it on the table. I could not help but smile.

I turned and said to her, "You really take care of yourself , ha?"

Ingon siya nako (with a big smile), "You don't get to live up to 92 years without taking care of yourself, Doctor!"

bwahahaha. We both laughed. She was taking a lot of medication for her specific ailments, but I was amazed that her spirit was very resilient!
