E.coli outbreak in Europe caused by new toxic strain
« on: June 02, 2011, 08:34:49 PM »
By Tan Ee Lyn – 19 mins ago

HONG KONG (Reuters) – The E. coli epidemic in Europe is caused by a new, highly infectious and toxic strain of bacteria that carries genes giving it resistance to a few classes of antibiotics, Chinese scientists who analyzed the organism said.

The scientists at the Beijing Genomics Institute, who are collaborating with Germany's University Medical Center Hamburg-Eppendorf, completed sequencing the genome of the bacterium in three days after receiving its DNA samples.

"This E. coli is a new strain of bacteria that is highly infectious and toxic," said the scientists at the Beijing Genomics Institute in Shenzhen city in southern China.

They said in a press release on Thursday the bacterium was closely related to another E. coli strain, called EAEC 55989, which was previously isolated in central Africa and known to cause serious diarrhea.

Authorities are still hunting for the source of the new bacteria, which is believed to have contaminated raw vegetables. The E.coli outbreak has so far killed at least 17 people and made more than 1,500 others ill in eight European countries.

University Medical Center Hamburg-Eppendorf, one of the largest hospitals in Hamburg, treated many of the infected patients from north Germany and found they did not respond to some of the antibiotics used, the Chinese scientists said.

"The analysis further showed that this deadly bacterium carries several antibiotic resistant genes, including resistance to aminoglycoside, macrolides and Beta-lactam antibiotics: all of which makes antibiotic treatment extremely difficult," the scientists said.

This new strain of also bore the hallmarks of other E. coli strains that are known to cause symptoms such as bloody diarrhea, or hemorrhagic colitis, and hemolytic-uremic syndrome, which affects the kidneys.

E.coli can be passed from person to person but experts say there was no evidence this was happening in any significant numbers in this outbreak.

Health experts are recommending strict hygiene measures such as hand washing and thorough cleaning and cooking of food. -- http://news.yahoo.com/

Oregon scientists stunned by German E. coli outbreak
Published: Thursday, June 02, 2011, 1:21 PM  Updated: Thursday, June 02, 2011, 2:07 PM
By Lynne Terry, The Oregonian

Food safety experts in Oregon are riveted by the E. coli outbreak that's swept Germany in one of the largest outbreaks of its kind.

So far, at least 1,600 people have been sickened and 18 people have died.

"It's a stunningly high death rate," said William Keene, senior epidemiologist with Oregon Public Health. "It's a huge and terrible outbreak."

The outbreak was traced to a mutant strain of E. coli, according to the World Health Organization.

"This is a unique strain that has never been isolated from patients before," WHO food safety expert Hilde Kruse told The Associated Press. She said the new strain has "various characteristics that make it more virulent and toxin-producing" than the many E. coli strains people naturally carry in their intestines.

Keene said scientists are surprised by the skyrocketing number of severe cases in the form of kidney failure, up to about 500 so far. The outbreak has spread to 10 European countries but nearly everyone sickened has traveled to Germany or lives there. The Centers for Disease Control and Prevention is looking into two possible cases in the United States involving people who recently were in Hamburg.

Germany's E. coli outbreak spreads to 11 more countries
Jun 4, 2011  |
BY HELEN LIVINGSTONE AND PAT REBER
MCCLATCHY TRIBUNE

BERLIN -- The E. coli outbreak concentrated in northern Germany has spread to 11 other countries, including the U.S., the World Health Organization said Friday, amid increasing concern about the new strain's virulence.

The German government said a "European solution" will be found to compensate vegetable growers and traders who have suffered losses as consumers avoid raw vegetables and salads.

By Thursday, 1,122 cases of enterohemorrhagic Escherichia coli had been reported, while 502 cases of hemolytic uremic syndrome (HUS), a form of kidney failure caused by this E. coli strain, had been reported, the WHO said.

In all but two of the cases the patients lived in or had recently visited northern Germany.

But the number of cases of HUS in the country had increased to 520, German authorities said Friday.

So far, the outbreak has killed 18 people in Germany and one in Sweden. Most of the victims have been women.

Health officials said four people in the U.S. might be linked to the outbreak.

All four were in northern Germany in May and officials are confident that they were infected with E. coli in that country. Three of them -- two women and a man -- are hospitalized with a kidney complication that has become a hallmark of the outbreak.

Besides Germany and the U.S., the countries affected are: Austria, Czech Republic, Denmark, France, the Netherlands, Norway, Sweden, Spain, Switzerland and Britain.

Though Spanish cucumbers were cleared of blame for spreading the bug, described by Chinese researchers as a "new super-toxic" bacterium, a German Consumer Ministry spokesman repeated that consumers should avoid eating raw cucumbers, tomatoes and salad.

The difficulties doctors experience in treating the rare infection was raising concerns Friday.

"The severity of this strain has resulted in great pressure on the health services," said Winifred Kern of the University Hospital in Freiburg.

In northern Germany, hospitals were "experiencing a problem with availability of intensive care beds due to the high number of cases with life threatening complications," he said.

Doctors also questioned Europe's preparedness to deal with future outbreaks.

"This most recent E. coli epidemic, of a strain previously unseen in an outbreak, shows us yet again that new bacteria and infections are just around the corner," said Giuseppe Cornaglia, president of the European Society of Clinical Microbiology and Infectious Diseases. "The pan-European nature of this E. coli outbreak ... reinforces the need for concerted cooperation across borders to tackle not only this outbreak, but also future ones." -- http://www.freep.com/

we can't be too careful.  we may be on the other side of the world, but we don't know where our salads come from.  we've got to do away for a while with our daily vegetable salad.  :(

What is E. coli?

E. coli is short for Escherichia coli. It is a type of bacterium present in the gut of humans and other animals.

Most strains are harmless but some are able to produce toxins that can cause symptoms in humans.

The severity of the illness people can get varies considerably, but some types of E. coli lead to severe cramps and diarrhoea.

Previous outbreaks have been linked to the 0157 strain, however, this seems to be something different.

Symptoms such as diarrhea with blood in it, severe cramps, and fevers, can take up to eight days to develop.

It is hoped that the number of cases will begin to fall following public health advice issued in Germany last week.

But the outbreak is causing severe infections and in a number of cases, affecting the blood and kidneys.

Haemolytic uraemic syndrome, an unusual complication of some types of E. coli as well as other infections, has been seen in hundreds of current cases.

This can be mild, however, in some cases it can lead to epileptic fits, permanent kidney failure and even death.

Large numbers of people between the age of 16 and 60 have developed haemolytic uraemic syndrome (HUS).

What is the health advice?

The exact source of the contamination has not yet been determined.

Until the cause is known, German officials are telling people to avoid eating raw tomatoes, cucumbers and leafy salad including lettuce, especially in the north of the country, until further notice.

The UK's Health Protection Agency recommends travellers follow the same advice.

It says anyone returning from Germany with illness including bloody diarrhea should seek urgent medical attention and mention their travel history.

Treatment will probably include fluids and pain killers. There is evidence that some common antibiotics might not work and some experts believe antibiotics could make the illness worse.  For patients with severe HUS, dialysis might be needed to support failing kidneys.

The Food Standards Agency in the UK has issued general advice on the need to wash fruit and vegetables.

The agency says: "It's a good idea to wash fruit and vegetables before you eat them to ensure that they are clean, and to help remove germs that might be on the outside.

"Peeling or cooking fruit and vegetables can also remove these germs."

However, a Scottish expert said new research suggests washing alone may not be enough, as the bacteria may be inside the food.

Dr Nicola Holden of The James Hutton Institute said: "The bacteria are able to get from animal sources on to crops through different routes, most likely in irrigation water or sometimes from slurry spraying, while some contamination can also occur during processing and packaging."

She said the bacteria can colonize plant roots, moving up to the edible foliage or fruits.

"The threat to human health occurs because these bacteria are not simply sitting on the surface of the plant and are particularly difficult to remove post-harvest," she added.

E. coli outbreak blamed on German veggie sprouts
By KIRSTEN GRIESHABER and TOMISLAV SKARO, Associated Press – 31 mins ago

HAMBURG, Germany – The terrifying E. coli outbreak in Europe appears to have been caused by vegetable sprouts grown on an organic farm in Germany, an agriculture official Sunday as the toll climbed to at least 22 dead and more than 2,200 sickened.

Preliminary tests found that bean sprouts and other sprout varieties from the farm in the Uelzen area, between the northern cities of Hamburg and Hannover, could be traced to infections in five German states, Lower Saxony Agriculture Minister Gert Lindemann said.

"There were more and more indications in the last few hours that put the focus on this farm," Lindemann said.

Many restaurants involved in what is now the deadliest known E. coli outbreak in modern history had received deliveries of the sprouts, which are often used in salads, Lindemann spokesman Gert Hahne told The Associated Press.

Definitive test results should be available Monday, Lindemann said.

In recent days, as health officials tried to pinpoint the source of the unusually lethal outbreak, suspicion fell on lettuce, cucumbers and tomatoes, perhaps from Spain. Spanish farmers complained that the accusations were having a devastating financial effect.

"First it's the `evil' Spaniards, and then you hear, very surprised, that it is our neighbor," said Dietrich Benni, who lives near the German farm. "It's a bit scary all of this, especially that it is coming from an organic place."

He added: "No more organic food for me for now."

The farm was shut down Sunday and all its produce recalled, including fresh herbs, fruits, flowers and potatoes. Two of its employees were also infected with E. coli, Lindemann said. He said 18 different sprout mixtures from the farm were under suspicion, including sprouts of mung beans, broccoli, peas, chickpeas, garlic lentils and radishes.

As for how the sprouts became contaminated, Lindemann noted that they are grown with steam in barrels at 38 degrees Celsius (100 degrees Fahrenheit) — an ideal environment for bacteria to multiply.

He said it is possible that the water was contaminated with E. coli or that the sprout seeds — purchased in Germany and other countries — contained the germ. He said the farmers had not used any manure, which is commonly spread on organic farms and has been known to cause E. coli outbreaks.

Lindemann urged Germans not to eat sprouts until further notice. He said authorities could not yet rule out other possible sources and warned Germans to continue avoiding tomatoes, cucumbers and lettuce for now.

AP journalists went to the farm on Sunday night, but nobody was available to talk. Telephone messages left at the farm's office were also immediately returned.

The outbreak has been blamed on a highly aggressive, "super-toxic" strain of E. coli, perhaps one that scientists have never seen before.

E. coli can be found in the feces of humans and livestock and can spread to produce through sloppy bathroom habits among farmworkers and through animal waste in fields and in irrigation water. Organic farms tend to use more manure than other producers do.

Sprouts have been implicated in previous E. coli outbreaks, particularly one in 1996 in Japan, where tainted radish sprouts killed 12 people and reportedly sickened more than 12,000 others.

The head of Germany's national disease control center raised the death toll to 22 Sunday — 21 in Germany and one in Sweden — and said an additional 2,153 people in Germany have been sickened. That figure included 627 people who have developed a rare, serious complication of the disease that can cause kidney failure. Ten other European nations and the U.S. have reported a total of 90 other victims.

German E.coli outbreak claims first child victim
Tue Jun 14, 2:37 pm ET

BERLIN (Reuters) – Germany's E.coli outbreak claimed its first child fatality on Tuesday when a two-year-old died from the bacteria in the northern city of Hanover, taking the death toll to 37.

German health authorities have linked the epidemic, the deadliest of its kind in modern history, to contaminated bean sprouts and shoots from a German organic farm sold to consumers and restaurants across the country for eating in salads.

Germany's main disease control center, the Robert Koch Institute, said the number of new infections appeared to be on the wane.

More than 3,200 people have so far been taken ill with the E.coli strain, about a quarter of them developing a complication called hemolytic uremic syndrome, which affects the blood, kidneys and nervous system.

While cases have been reported in other countries, all have been traced back to visits to northern Germany and all but one of the fatalities have been in Germany.

The two-year-old child victim, who was not named, had been undergoing treatment for complications caused by the deadly new strain of E.coli at a Hannover hospital, along with the child's father and two siblings. There was no word on their condition. -- http://news.yahoo.com/s/nm/20110614/ts_nm/us_ecoli
