MOBILE PASSPORT PROCESSING AND OTHER SERVICES – NOVEMBER 9-10,2007,TAGBILARAN CITY (BEPO)

The Provincial Government of Bohol (headed by Governor Erico Boyles Aumentado) through the Bohol Employment and Placement Office (BEPO) will conduct the mobile passport processing and other services here in the City of Tagbilaran on November 9 and 10, 2007, 8:00 am to 5:00 pm

* November 9 – Services of the Professional Regulations Commission (PRC), Land Transporation Franchising and Regulatory Board (LTFRB), Social Security System (SSS) and PhilHealth at the Provincial Capitol Building.

* November 10 – Mobile Passporting Services at the Island City Mall Activity Center

Requirements for New Passport Application: (Fee: P750.00)
1) Authenticated birth certificate – from the National Statistics Office
2) For married women, authenticated marriage certificate from the National Statistics Office
3) Three (3) passport-sized pictures
4) Baptismal certificate
5) NBI clearance or transcript of records (except for children)
6) Photocopy of any valid ID
7) For children, photocopy of parents’ passports

Requirements for Renewal of Passport: (Fee: P750.00)
1)   Photocopies of the 1st, 2nd, last pages of old passport and record of arrival
2)   Three (3) passport-sized photos
