Hello Jenneth,

Thank you for bringing your question here. During a fertile woman's monthly menstrual cycle, there are observable symptoms that the woman will feel. One of the noticeable symptoms are headaches, and even migraine-like headaches. During menstrual cycle there is a change in estrogen and progesterone levels. When estrogen levels drop very low, Jenneth, this will manifest in headaches. The lower the estrogen level, the more profound the headache.

You may try some remedies to help reduce your symptoms:

1) Try putting ice pad on the area of pain (usually around the head or the neck)
2) Exercise; this helps in the reduction of pain
3) If the pain is strong, it is recommended you take NSAIDS (non steroidal anti inflammatory drugs); ibuprofen, naproxen, celebrex

Jenneth, if you have a history of severe migraine-like headaches during your menstrual cycle, you should take NSAIDS some 3 days before the start of your menstrual cycle (this will take some monthly planning on your part!)

Hahaha! Make sure you go out once and a while, and have people to talk to there, Mohandz! In moderation lang gud, Mohandz.

On a serious note, it is not healthy to isolate yourself from people. Don't do that because there is a risk of developing depression. Develop healthy hobbies pod ba. 8)

You're most welcome, Jenneth. You know what also helps? he he he! When my gf had her menstrual periods, she also developed very debilitating headaches! To help reduce her pain ( i don't like it when she takes too much drugs ) , I will buy her chocolates. Incidentally, chocolates stimulates the release of a neuroendocrinal hormone called Serotonin, which is responsible for the "feel good" sensation. It reduced her pain considerably.

Another was massage. I usually massage her body and her feet to reduce her pain.

Mo joke around ni sija nako, "Oh, you're making me fat!"

ha ha ha ha! ;D

Mohandz,

It is normal to do it 3-5 times a week. It is perfectly and physiologically normal. Sexual activity is important for proper hormonal balance.

Hi Doc Lorenz,
Found this thread interesting, basahon ni nako nga thread sa day off, basahon nako from page 1, gusto ko aning discussion nimo about hormones. PCOS patient man gud ko unya ingon akoa OB dili ko tig ovulate.

Thanks.

Hi Joy,

PCOS - polycystic ovarian syndrome - is one of the most common endocrine disorders in reproductive women, with a prevalence of 6-10% among women in the US of reproductive age.

Women with PCOS are also at an increased risk for the following:
1. type 2 DM (diabetes mellitus; insulin dependent)
2. insulin resistance
3. infertility
4. metabolic syndrome - insulin resistance, obesity, atherogenic dyslipidemia, and hypertension

TREATMENT
1. Women are not attempting to conceive: treat with a combination of OCPs, progestin, and metformin (or other insulin-sensitizing agents)

2. Women who are attempting to conceive: Clomiphene + / - metformin is first line treatment for ovulatory stimulation

3. Symptom-specific treatment:
    a. Hirsutism: combination OCPs are first line; antiandrogens (spironolactone, finasteride) and metformin may also be used

    b. Cardiovascular risk factors and lipid levels: diet, weight loss, and exercise plus potentially lipid-controlling medications (e.g., statins).

If you have any other specific questions relating to PCOS, please ask it here, Joy.

Sincerely,
Dr. Lorenz

re lolla's problem during mentrual periods... i once suffered the same, terribly, coupled with sinusitis attacks.  during those times i felt murderous.  di gyod ko angay biklon.  my solution?  naayo ko sa acupuncture, done on me by a bonafide medical doctor who also practiced eastern medicine.  i hope doc lorenz will do the same, one day.

Pediatric Mental Health Disorders

by: Dr. Lorenzo L.

In the field of mental health medicine, the goal of the health care providers is to care for those who are inundated with mental health disorders such as learning disabilities, cognitive disorders, and hormonal imbalances, behavioral disorders (Sadock & Sadock, 2007).  Mental health is defined as a state in “being in balance and harmony with one’s inner self with one’s friends, family and colleagues; with one’s environment; and with one’s spirituality, “ (Seligman & Reichenberg, 2006). This is why therapists emphasize the fundamental importance of patients to experience a sense of unity with one’s self and the world. There is a sense of continuity in regards to children’s mental health and adult mental health because the presence of a positive mental health in youth populations will carry on to their adult years. A presence of negative mental health in children will ultimately carry on to their adult years, naturally (Chuan-Yu, Lawlor, Duggan, Hardy ad Eaton, 2006).

Mental health disorder is a psychological problem that is observed in a change in behavioral that manifests as either distress, disability and outside the cultural norm. Mental disorders are defined as a combination of how an individual perceives reality, the individual’s thinking processes and mannerisms. In regards to mental disorders in children, this is a bit more sensitive than adult mental disorder because the disorders in cognition, mental activity and behavior that are seen in children can affect and shape their social life, which then influences the future of the child (Cohen and Hesselbart, 1993).

There are almost five million children in the United States of America that are suffering from some form of mental illness and many more will be diagnosed every year. It is said that some 20% of American children will be diagnosed with a mental illness (Busch and Barry, 2007). Mental disorders can be quite difficult to identify for health care providers that is why it is essential they are effectively trained in mental health care compliance protocols, ethical situations. In addition they should be licensed to practice in the clinical setting and knowledgeable in the different therapeutic modules in the field of psychology including , but not limited to, classical psychotherapy, cognitive behavioral therapy, psychodynamic psychotherapy, existential therapy (Seligman and Reichenberg, 2006).

The sensitivity of childhood mental disorders is founded on the fact that children differ from adults in that youth experience a varied amount of mental, emotional as well as physical changes as they progress towards adulthood, which is part of normal growth and development. As an individual ages, he or she undergoes personality development, which was espoused by Dr. Erik Erikson, who proposed that psychosocial development continues over the entire life span and results from the interaction of our inner instincts and drives with outer cultural and social demands (Bjorklund and Bee, 2008). Personality development is integral for adult development and for the an individual to develop a stable personality, the personal must move through and successful resolve eight crises or dilemmas over the course of a lifetime. Children with mental disorders such as personality disorders, cognitive disorders, learning disabilities, anxiety disorders, and even eating disorders are unable to progress through the eight stages of psychosocial development (Bjorklund and Bee, 2008). Children mature at their own pace and there is a wide range of behavior and abilities for what are considered normal (Chartier, Walker and Naimark, 2009). The common mental disorders that are observed in children include Attention Deficit Hyperactivity Disorder and Eating Disorders.

Attention Deficit Hyperactivity Disorder is a disorder that is characterized by a pattern of diminished sustained attention and higher levels of impulsivity in a child or adolescent than expected for someone of that age and developmental level (Sadock and Sadock, 2007).  ADHD is related to the complex interactions of the neuroanatomical and neurochemical systems. There is an evidence for a genetic contribution for the emergence of ADHD. Children are diagnosed with ADHD of they have six or more of the following symptoms of inattention that has persisted for more than 6 moths: 1) failing to give close attention to details or makes careless mistakes in schoolwork, work, or other activities; 2) often has difficulty sustaining attention in tasks or play activities; 3) often does not seem to listen when spoken to directly; 4) often does not follow through on instructions and fails to finish schoolwork, chores , or duties and fails to finish schoolwork, chores, or duties in the workplace; 5)often has difficulty organizing tasks and activities; 6)often avoids, dislikes, or is reluctant to engage in tasks that require sustained mental effort; 7) often loses things necessary for tasks or activities; 8 ) is often easily distracted by extraneous stimuli; 9) is often forgetful in daily activities (Sadock and Sadock, 2007).

The treatment for Attention Deficit Disorder is provided for in various community organizations that centers on mental health disorders (Clemans-Hope, 2004).

In an article by Clemans-Hope (2004), the treatment facilities for children with mental health disorders such as ADHD require continuous monitoring either in the form of therapy or psychopharmacology. The medical that is prescribed for children with this disorder include stimulants that stimulate the central nervous system such as Methylphenidate, Dexmethylphenidate and Dextromethamphetamines. Non-medication treatment includes psychosocial interventions such as social skill groups, behavioral intervention therapies (Sadock and Sadock, 2007).

Another very nefarious mental disorder that is quite common in school-bound adolescents is eating disorders. There are very two common types of eating disorders that continue to be a pervasive problem for the children in the United States; Anorexia Nervosa and Bulimia Nervosa.

Anorexia Nervosa is a self-induced starvation to a significant degree and is associated with a relentless drive for thinness or a morbid fear of fatness. This condition is associated with depression in 65 percent of cases, social phobia in 34 percent of cases and obsessive compulsive disorders in 26 percent of cases (Sadock and Sadock, 2007).

The treatment plan for Anorexia Nervosa includes hospitalization when necessary and both individual and family therapy. The best way to treat this condition is through behavioral, interpersonal and cognitive approaches more so than medication (Sadock and Sadock, 2007).

Bulimia Nervosa is a condition in which patients will binge eat and then throw up or take laxatives. The purpose of their actions is to evade in gaining wait. The treatment that is effective for this is behavioral modification therapy and psychoanalysis.

i think this is an issue that should be addressed by school officials and health professionals in academic settings. instructors should be educated on these issues as to identify students who are suffering said disorders and thus accommodate the proper resources for said intervention. it is a growing phenomena here in the united states of america, i'm sure it is also of pertinence in the philippines.

Dok,

kining guwang na ta, menos na man ug otog ang ots. okey ra ba mo gamit ug viagra. wa ba ni side effects

Nong Balong,

So long as you are not taking any nitrates, you should be fine. You have to consult your family M.D to make sure that you have no conflicting medications that can cause a severe hypotensive crisis.

Patients who are on nitrates (nitroglycerine, nitroprusside and other derivatives) must be careful not to take viagra, because viagra (otherwise known as SILDENAFIL) is a vasodilator and thus dilates the arteries and veins, which has the effect of reducing blood pressure. Nitrate drugs acts on the arteries in such a way that it causes dilation -- to reduce the oxygen demand on the heart. So, a patient who is taking nitrates and also takes sildenafil (viagra), might experience a severe hypotensive crisis.

This is why we warn patients about said drug to drug interaction and the side effect of -- severe hypotensive crisis.

If you want to work on your erection problem, I will suggest  you do keggel exercises. Keggel exercises increases the strength of your suspensory ligaments and muscles, which is responsible for maintaining and prolonging penile erection. Not only that, the said exercise can actually help the man delay ejaculation -- so you can experience a more profound and healthy sex life.

Jenneth, your BP is normal man na. It is neither in the threshold of hypotension or even pre-hypertension. Have you had any blood drawn recently and cholesterol markers mapped? A medically proven way to help reduce bad cholesterol (ldl) is by taking daily supplements of Omega 3&6 Fatty Acids.

As for your blood pressure, that is normal range, Jenneth. You are neither in the threshold of hypotension or pre-hypertension. Diet and exercise is good. I also would suggest you eat ampalaya as well, which has very noticeable Hmg Coa activity, which helps in reducing cholesterol.

:)

Hello and Good Afternoon Mr. Bolbuhol,

Thank you for addressing your question my way. To answer your question, there are a lot of vegetables that can help reduce cholesterol levels, specifically the LDL (low density lipoprotein), which is linked with a host of cardiovascular disorders. The regular intake of green leafy vegetables is associated with the reduction of LDL and TRIGLYCERIDES.

Some green leafy veggies that you should be eating include:

1. oats
2. barley and other whole grains
3. eggplant and okra
4. nuts
5. vegetable oils
6. apples, grapes, strawberries, citrus fruits,
7. food that is fortified with sterols and stanols
8. soy
9. fish
10. fiber supplements
11. ampalaya

I will also produce a lecture review and post it on youtube and direct the link here to this thread later this week, Mr. Bolbuhol. I will find time in between my hectic schedule to produce a thorough and educational lecture.

Until then, if you have any other questions, feel free to ask it here. If i have time, i will answer to the best of my ability.

Sincerely I remain,
Dr. Lorenzo

Let's talk about the mature defense mechanisms!

1. Voluntarily choosing not to think of bad news -- > suppression
2. Indian Jones using comedy to express feelings of discomfort --> Humor
3. Arsonist donates money to the fire department --> Altruism
4. Using ones aggression to succeed in business ventures --> Sublimation
5. Realistically planning for future conflict --> Anticipation
6. Consciously postponing an inner conflict after a big project is completed --> Suppression
7. Redirecting impulses to a socially favorable subject --> Sublimation

What are some immature defense mechanisms ?

1. Not acknowledging a piece of bad news , as though it was never said --> Denial
2. Involuntary witholding a feeling from conscience awareness --> Repression
3. A veteran that can describe horrific war details without any emotion --> Isolation of affect
4. A child abuser who was also abused as a child --> Identificaion
5. A man yells at his family after a bad day at work --> Displacement
6. Belief that people are all good or all bad --> Splitting (borderline personality trait)
7. Expressing aggression through passivity, masochism, and turning against self --> Passive aggressive
8. Changing ones character or personal identity to avoid emotional distress --> Dissociation
9. Returning to an earlier level of maturation to avoid the conflict at the current maturational level --> Regression

10. A thought that is avoided is replaced by an unconscious emphasis on the opposite -- Reaction formation
11. Converting mental conflicts into bodily symptoms --> Somatization
12. Chronically giving into an impulse to avoid tension from an unexpressed unconscious wish --> Acting out

Bran-kun, could please tell me more about Iterax? Nagsugod ko ug tumar (pero not all the time) kadtong pagsugod nako ug allergy sa chicken.. 10mg lang nuon, nawala baya ang itchiness ato, tapos I feel sleepy and relax pod kada mo-take ko ato..

Hi Jenneth,

Did your MEDICAL DOCTOR prescribe that to you or did you buy that yourself? ITERAX is also known here in the United States via ATARAX, the drug name is Hydroxyzine Hydrochloride. This drug is used primarily here in the United States as an indication for GAD (generalized anxiety disorder) , psychosis, atypical depression and severe forms of obsessive compuslive disorder. Basically, its a neuroleptic. It has a mechanism of action that can effectively lower NOREPINEPHRINE, increase GABA (this is an inhibitory neurotransmitter), and can increase SEROTONIN (neurotransmitter associated with feelings of satisfaction, happiness, feel-good effect).

Sometimes, ATARAX  can be used for allergic reactions, however, the mechanism of action is still being researched, and quite frankly, i don't think you should be taking this drug , Jenneth, unless you are diagnosed with generalized anxiety disorder, have depression, have psychosis, or have ocd. this drug, jenneth is a neuroleptic.

In addition, due to the pluripotency of this drugs (meaning has the ability to affect many receptors in the body), it has also a wide side effect lists including vision problems, seizures, cardiotoxicity, respiratory depression, and addictive qualities.

In fact, we don't even give ATARAX is not even the drug of choice for anxiety disorders, for those we give them: benzodiazepines (clonazapam etc), buspirone or SSRI (serotonin selective reuptake inhibitors such as citalopram). We do this because these drugs a less severe side effect.

Again, to reiterate to you that ATARAX is primarily an neuroleptic drug that is given to reduce symptoms of anxiety, depression, psychosis, and obsessive compulsive disorders.

There are better forms of anti-allergy medication to treat your allergic reaction to chicken. In fact, you don't even need to recourse for prescription medicine like ATARAX for that. If you are allergic to something, particularly a food source, the best way to treat this is behavioral modification. Stop eating the chicken. Your body has developed antibodies to it and thus it attacks it everytime you eat it. that is why you hare having systemic symptoms. If , however, you do eat chicken accidentally and you get an allergic reaction, consider benedryll.

You are tired and sleepy because ATARAX is an anxiolytic and neuroleptic drug. If you are not diagnosed with GAD , Atypical Depression, Psychosis, Mania, or Obsessive Compulsive Disorder, then I highly recommend you stop taking this drug at once.

If you have symptoms of cardiotoxicity, seizures, visual problems, please stop taking this drug and consult your attending M.D.

Sincerely,
Bran

Jenneth,

If your food allergy is that discomforting for you, as in itching, it may be due in part to your body's sensitivity to chicken. The reason why you get itchy when you eat chicken -- that is because your immune system, particularly your body's b-cells have been sensitized to it. It considers the chicken as an antigen. Your b-cells have developed specific antibodies to chicken, and thus when you eat it, the b-cell created antibodies which bind to the chicken. this then attracts the migration of specific immune cells such as your macrophages, neutrophils, basophils, and mast cells. The mast cells and basophils release a chemical called HISTAMINE, which is responsible for allergic reaction activation. It contributes to bradykinin release, which is the cause of coughing. Histamine causes vasodilation of the blood vessels, increased body temperature, and pruritic itching.

Ang kaso ani, Jenneth, is the fact that your body is naturally allergic to chicken. It was sensitized to it.

To effectively alleviate this symptom is to stop eating chicken period.

However, if you like to eat chicken, then you need antihistamine drugs such as benadryll or chlorpheniramine. These are drugs specifically targeting the release of histamine and does not deal with neurological consequences such as ATARAX.

I hope you are more informed. And please, update me if you are having symptoms of the side effects i listed in my previous post. If you do, go to your attending M.D.

Be safe, my friend !

What is Arrhythmia ? How can we treat Arrhythmia ? Can you tell me more about the different types of drugs for Arrhythmia ?

1. Arrhythmia is a type of pathology that affects the heart. The heart, like a machine, is powered through electrical system, which is called the cardiac conduction system. If there is an anomaly in the electrical system, there will be abnormal contraction of the cardiac muscles, which in turn leads to the symptoms of rapid heart beat (tachycardia), skipped beats, feelings of palpitations. All of these are a result of abnormal heart rhythm due to the abnormal cardiac conduction system. The word ARRHYTHMIA , if we break it down, means ABNORMAL HEART RHYTHM.

2. Arrhythmia, which is divided into different types, can be treated through different types of anti-arrhythmic medication. Let's talk about the different types. Anti-arrhythmics are local anesthetics that are able to slow or block the conduction (especially in depolarized cells). These drugs decrease the slope of phase 4 depolarization and increases the threshold for firing in abnormal pacemaker cells. There are 4 classes of anti-arrhythmic medication: 1) Class I, Class II, Class III, and Class IV.

Class I anti-arrhythmics are Sodium-Channel blockers; these drugs block the sodium channels on the myocardial cells. Some popularly prescribed drugs of this class include; Quinidine, Procainamide, Disopyramide, Lidocaine, Mexiletine, Tocainamide, Flecainide.

Class II anti-arrhythmics are Beta-Blockers that block the beta-receptors of the myocardial cells. Some prescribed drugs of this class include: Propranolol, Esmolol, Metoprolol, Atenolol, Timolol.

Class III anti-arrhythmics are Potassium-channel blockers. Some drugs of this class are Sotalol, Bretylium, Dofetilide and Amiodarone. Note: we usually administer this when all other classes are ineffective.

Class IV anti-arrhythmics are Calcium-channel blockers: Verapamil and Diltiazem are commonly prescribed.
