Need Your Opinion: Refusal in terminating patient's pregnancy
« on: February 26, 2012, 02:35:32 AM »
Hi guys, i need your opinion on something.

Today i came in early in the morning from 5 am to 11 am to fill in for someone, we had exchanged our rotation schedule for this weekend (i'm filling in for him this weekend, and he's filling in for me next weekend). I was filling in for his OB/GYN rotation, so today was a general day, doing patient charts and scheduling patient pre-natal checkups, general check ups and all that.

Well, i was also given a surprise by the attending OB/GYN , he was scheduled to complete some 6 pregterms today (pregterm= pregnancy termination; surgical abortion).

I was prepped about this just and that i found out that that same OBGN wanted me to observe the first 2 pregterms because he wanted me to finish the remaining 4.

I really felt sick about it during preparation. As he was explaining the procedure, the curatives, the complications , etc, i was really feeling nauseous. I went to the locker room and got suited up for surgery, and observed my attending perform the first pregterm, the second one was quick. As he finished, he asked me if i was , "Ready" for the remaining 4 patients.

After the 2nd surgery, i asked to talk to him in the physician's office. We went there and i told him of my personal feelings, and my stance on the matter and since the pregnancy terminations were elective surgeries (meaning, i was not required to do them, but only observe), i told him that i will opt not to do the surgeries, but rather, observe only. He understood the situation and agreed to my request.

I felt so sick during the operations. My true feelings and the disgust i felt being there, at witnessing these surgeries cannot be explained in words. I also felt a sense of anger towards my friend who asked me to fill in for him this weekend. He did not tell me that he was scheduled to perform 6 elective surgical abortions.

I will need to talk to him this week about this.

The 5th patient we had was a mother who was pregnant with a 14 week old fetus. She was too late into term to take Mifeprestone (taken with misoprostol to induce abortion), so she wanted to get surgical abortion. As the medical intern, i asked her why she wanted to get it. This girl , who was in her adolescent age. She replied how she was scared to tell her parents about the pregnancy and her almost 4 month old baby inside her.

We monitored the patient's HR as well as the fetal HR. What made me more disgusted was the ending of the fetal HR when my attending OB/GYN ligated the fetus' forming umbilical vein and artery from the patient's uterine lining.

The patient was crying after the surgery. Later in the morning, i went in to check up on her and reassured her of the outcome. She kept on crying and asked me, "Do you think i did the right thing?". I just smiled at her and said, "Just take care of yourself. And feel better."

I felt so very sick after my rounds.

It is a feeling that pulls your stomach really low, and when you want to cry, you cannot cry because you are in a surgical setting. Even tho a preg-term surgery takes only some 20-30 minutes, it feels like an eternity in that place. Why on hell anyone would want to specialize as an Abortionist escapes me. Even if one can make a lot of money through it, does not their conscious take the toll?
