I dont understand why the attending gave that one patient Nifedipine to treat his Heart arrythmia. Yes, Nifedipine is a CCB (calcium channel blocker), but its a weak heart affector and focuses more on vascuar vessels.

A far more effective Cardiac affector would have been Verapamil and Diltiezam.

And when we told him about the lower affinity for dihydropyrodine in treating cardiac defects--he didn't even listen. Nicedipine was effective back in the early 90s, but now we have newer drugs--cardiac specific.

We are 2nd Year medical students. I don't understand why some students have to cheat in exams. Or show notes during exams. During my exam someone tried to copy of my answers in the scan tron sheet. This had me reeling. I had to report this individual after the exam.


Preposterous! How can you be a medical student and try to copy from another medical student?

You passed Medical Legal Ethics last year, but you are copying ?
Where is the ethical notion? Where is your morals?

How can you be be a Physican-in-Training and have the audacity to copy and cheat in exams?


I have no sympathy for you. Absolutely absurd!

How can you even ask that kind of question? First of all ACH is standard for acetylcholine--it is the basis for cholimimetics and cholinergic drugs. Its basic biochemistry and pharmacology. You would know this had you not cheated and copied on exams.

Pathetic.

What is HTN? You asked me this--are you serious?
HTN: Standard acronym for Hypertension.

This is all material from medical physiology and surgical neuroanatomy.

Taking a break from that long, long, long pathology lecture.
Jeez, thanks alot Doc, but 120 slides just to explain nutritional deficiency was quite overwhelming!

So at 3:00pm we'll have Psychiatric Behavioral Science lecture.

Im pretty excited to try out these new psychoanalysis.
Since reading into psychoanalysis, ive been psychoanalyzing people as of late. The way they talk, the way they walk--their mannerisms. Its wierd because it becomes almost natural and one gets en sync to the procedure.

And remember, Rupa, don't forget that when we are treating a patient with only hypertension, if that patient is below the age of 55 and not suffering any other ailments aside from hypertension,

The first action will inlcude treating that individual with ACE-inhibitors.
If that does not produce wanted outcome, we will add treatment with Calcium Channel Blockers.

And if that does not help, we will utilize a combined treatment of:
ACE-Inhibitors, Calcium channel blockers and Thiazide Diuretics.

Good to see an old friend, and a future 1st year matriculatee.
Sharing some good memories of our NYLF/MED experiences back in 2003.

The irony of the situation:

We both attended the National Youth Leadership Conference on Medicine, held at the prestigious Georgetown University in Washington D.C back in July 2003.

Now, its ironic and befitting, that we meet again, in July 2009.
In Medical School. lol!

Our paths meet again. God Is Good.

Collegiate understanding and undergraduate works even master's work mean nothing, when it comes to medicine.

First, let that be made known.

In undergraduate college, I took 2 semesters worth of Microbiology and Virology, but the course Im taking right now in 2nd year Med : Medical Microbiology and Integrative Immunology is totally different from undergraduate schooling.
The focus is totally different. Undergraduate focused more on the genetic and molecular aspects, in medicine, we focus more on the clinical aspects, reading data and lab analysis, diagnostic testing, preventatives and treatment (if any).

My advice to all pre-med students and or students considering medicine is this:
--It's never too late or too early to start reading. Read journals, articles, read medical papers, read micro, read lab works, etc. Just get your hands on something and READ. Believe me, when you start medicine, you Won't even have time to do anything else but read assignments, or to finish a 500-600 paged textbook within 2-3 weeks time, and at the same time finish preqs in other classes. So Read, Read, Read.

There is no such thing as procrastination here. If you do, don't expect to do well. I'm telling you that right now. To your face, direct, honest, truth.

So start early. Be proactive.
But as the saying goes, "You do what you can with what you have, and with the time you have alotted to you. So work."

Dear Blog Diary,

Today was an especially hard day at work and at school. Dealing with all this stress has been definately multifaceted and two-edged. 1) Understanding the perplexities of work and the infinite possibilities given to us in the medical profession. Stress incarnate. :)
2) Accepting and totally getting used to the beautiful struggles of life.

One thing, blog, that I have come to terms and acceptance throughout the duration of my medical studies is this:
1. YOU CAN'T LEARN EVERYTHING.
    a) as much as I have put into the learning and understanding of the materials included: anatomy, physiology, histology, embryology, medical legal ethics, human genetics, clonal morphism, pharmacology, pathology, pathoimmunology, micro-immunology, pathophysiology, Psychiatric Medical Science, Neurology, Clinical Neurosurgery, etc.----there can never be a way to know everything.
I accept that now. There are times when earning a 85% on an examination or even a 90%+ is acceptable. There is no way we can always achieve that 100%.
Perhaps in college--yes, but in medical school---where I am amongst very learned and accomplished colleagues, I must admit that I pale in comparison. Normal amongst these learned individuals.

   b) GOD IS EVERYTHING.
      b1) Studies and stress and the tests that have come to challenge my resolve and my understanding has ONLY strengthened my faith. To know that MY LORD JESUS CHRIST is above all things. The writer and creator of this study: MEDICINE.
He is the eternal physician. The healer of all things. Spiritual and bodily.

    c) I AM NOTHING but DUST.
     c1) I accept the fact that I am nothing; nothing before the site of MY GOD. I am and will always be dust. Dust like all of us. But am justified, tho I am dust and ash, by MY LORD JESUS CHRIST to do what must be done, to finish what must be finished, to study what must be studied, to heal what must be healed, to infer what must be inferred upon, to correct what must be corrected--in this life that HE has given me. To use the MIND AND BODY that HE has GIVEN me.

   D) GLORIFY THE LORD GOD
 and so I end this promt contemplation of these events in my life.
That so long as I have a breath in my body, so long as I am awakened every day by the rising of the sun and promted to sleep by the setting of the sun---I will give--and will always give GLORY and SHOUT OF THANKS to my LORD JESUS CHRIST.
Who is my COMPLETE and TOTAL justification in Life.
In pains, I give to Jesus.
In success, I thank Jesus,
In accomplishment, I sacrifice it all to Jesus.
In sadness, I seak HIS face
In testing times, I abide by HIS WORD
In times of greatness, I give everything, and all to JESUS.


I am nothing, but Dust.
But because my LORD loves me, I am a dust that recognizes my own sin and recognizes the POWER AND HONOR of my LORD JESUS CHRIST. GLORIFY HIM! LORD! AMEN be to YOUR GLORIOUS NAME!

How can I forget the TOTAL Sacrifice of Christ Jesus?
Which is LIFE.

Today and this entire week has been a difficult week in studies, in reviews.
Constant mode na.
Salamat lang naay ko blog post to unleash and unload my stress.

Tho difficult ahong road, and filled with so many demands,
I can rest assured that my pains IS NOTHING compared to Christ's Crucifixion on the Cross, and His Death. And His GLORIOUS RESURRECTION.


Christ is Salvation.
Christ is THE WORD MADE FLESH.
Unto Salvation Is His Name.

So I justify myself.
Tho I am filled with stress,
Am justified in Christ Jesus.

Christ Justifies.
JESUS IS JUSTIFICATION.

I am glad you are happy, Dong.  We are doing fine.  Garden produce is not as good as last year but we have enough for our own consumption.  Kamunggay, especially, is doing great.

Today I went to the medical collegium to try out my white coat.

The name "A. Lorenzo Lucino Jr, Medical Student"
was written on the right chest banner.
With the emblem of SJSOM on the left and right shoulder.

That white coat,
awakened me.

As I stood there looking at myself in the mirror,
as I was photographed with my stethoscope,
It hit me.
This is it.
This is the goal.
Chris Saves!

May I always have the fire of Christ in my heart.
I beg Eternal Providence to keep me in remembering the dying,
the needy, the poor, the impoverished.

As my colleagues were ecstatic in trying their white coats today,
I couldn't help but have a sense of urgency in praying.
Blessing God for the Grace he has given me to pass classes thus far,
and begging God to provide me the ability to complete the tasks required of me.

It was a humbling experience.
And I was honored, indeed.

Today I talked to my clinical instructor for PDI. It appears that we will not only be giving injections to each other (saline), but will also be injecting ourselves, and learning how to draw blood and basics of phlebotomy.

Today was so exciting. Officially started my 4th semester.
Gi select ko to present on a surgical procedure (of my choice, he he he) on September 17th, 2009. So excited ko ana. So far i am thinking of doing a presentation on something related to cardiothoracic surgery; we'll see.

Feeling of Teaching.


Today was our first full Anatomy Lab Day.
I am the Teacher's Assistant in Medical Gross Human Anatomy I, and am in charge in teaching the human anatomy lab to 18 some students; part of Group A. 2 other anatomy TAs will cover group B, C.

So I want to let things out:

It feels different. I've TAd for undergraduate courses some 2 years ago, but this is totally different. You have total control in teaching the anatomy material; the ones i am covering tho is Upper Limb and Thoracic Anatomy.

Helping the students cut the cadaver, removing the superficial tissues, the skin, and the underlying adipose (fat) tissues. The feeling is good, knowing that you are helping your fellow student and professional colleagues. This is really what it means to be a doctor in training. To help and train your brother-in-arms. Its very humbling, this feeling.

I want to increase the confidence of my students. Many of them are worried about the material; too much material they say. I cannot make exceptions. I must cover everything. For their own benefit, and that of mine.

Every muscle, every vein, artery, bone, ligament, will be taught. I want them to know and retain the structure's name, function, innervation, blood supply, and clinical pathology. I want the best because I expect the best.

The feeling of being saved in Christ,
is indescribable!

I talked to my friend Mercedes yesterday at church,
she is an old lady that has helped our church choir in organizing our song booklets,
and she saw me as I walked towards the Blessed Eucharist,

she whispered, "Lorenzo! Lorenzo!"

I turned around, and smiled!

It is as if the Holy Spirit was there waiting for me, through my friend Mercedes, GOD BLESS HER HEART!

Her words of encouragement to me, "Lorenzo, may the Lord bless you, will pray for you." really did vindicate me.

It was a feeling of personal re-affirmation.

That my prayer was heard.

LORD, is so GREAT!

It is very humbling.
Today, my classmate Harvey referred to me as , "Dr. Lucino".
I responded, "Harvey, just Al, please."

It struck me. He is right. In 2 months, we will receive our white coats.
In 2 months, I shall go inside the prestigious Auditorium Medicinae to stand with my cohorts,
and be called each by name.
To receive our honored White Coat of the Physician.
To pave the way to our next 2 years as a Medical Intern.

It has come to me, that I have survived 2 exhausting, trying, demanding years of Medicine.
It was not my own ability that has provided for my success thus far, nay, it was GRACE OF GOD.
TO GOD BE THE GLORY
TO GOD BE THE HONOR
TO GOD BE THE MAJESTY
TO GOD BE THE PRAISE of PRAISES.

For HE has made this average student able to do what I was able to do.

Dear Blog,

So I am relieved to know that my mother and my sister got back home safely. My mother and my sister came to stay with me for a week from last Saturday to this past Saturday, staying with me in my apartment, though I had classes all week. I took them to the beaches, to the medical campus, shopping districts, to church, as well as saw the sights in the local atmosphere. My friends were introduced to my family, thanks to mommy's home-made pancit and adobo that we all ate with delectable appreciation.

Thanks so much Lord for allowing my mom to take a week vacation to visit me and check up on me, as I was homesick for a bit. But that is life, and the journey in this road of Medicine is not an easy road, but it does help to see, feel, and talk to one's family and loved ones through the journey. Thanks be to God.

It feels good to know that family is good back home, Thanks be to God.

Thanks be to God, who is wondrous in His Compassion.
Praise be to God!

Dear Blog,

My day has been quite multi-faceted today. Work has not changed, the requirements of the day have increased, due to the culmination of patient care. I had a meeting with Ericka and Renee and Dr. Ash on our research presentation, which is a publication material for hyperbaric medicine regarding air embolism. We have finally collected medical data from the local hospital that stems from 2000 to 2008. Basic concept that was addressed with be the use of ANOVA in the statistical aspect of the project; as well as the use of regression analysis etc.

I am also counting the days till we receive our white coat and end this semester of 2nd year medicine. Countdown is 38 days.

The Lord is Kind. The Lord is Good.

Dearest Blog,

How are you? What is going on?
Life, as always, moves on. Let that be known.

My friend and our medical college's librarian assistant, Andalita K, passed away this Saturday.
I found out about this this morning from my clinical instructor. To my total astonishment.
I just saw her and talked to her on Friday, and I am so very sad to hear of her demise.
She was healthy, to my visual inspection from our last conversation, which was outside of the library's halls.
She was talking about visiting her family in Holland, and I, sharing my experiences and familial nostalgia.

She did ask me on Thursday Last, if I was going home for winter break, and I replied, "Yes, Andalita, I am."
With a smile on her face she said, "Thats good, Al. Family is always good."

It is in sad and grieving posture that I offer a hymn of praise to God in the Highest for His love,
and acceptance of what has happened as being the Will of the Most High.
He who knows each one's hour and demise, the time of death, and the manner of death.
To which all are destined to, as is written in the Book of Life.

God, Eternal Father, we pray for the life and the soul of the late Ms. Andalita K,
That thou woud'st grant her mercy and justice in the hour of her judgement.
Remember, O Lord, that mercy was never alien from you,
and that Grace and Pardon is filled in the heart of your Son, Jesus Christ.
Merciful Savior, judge the soul of Andalita, my friend, in accordance to your precepts and Law,
that despite her weaknesses, she did live a faithful and Good Life.
Vouchsafe for her, Lord, the acceptance into your Kingdom,
A Kingdom that shall never perish, and shall stand in unison before Your Glory.
In Glorious Praise we ask this in the name of Jesus Christ, Our Lord.

Today I took my Clinical Medical PDI Exam, Block 3.
50 Clinical questions,
then a strict 30 minute live patient examination.
Covering Abdominal: spleen, liver, kidney, pancreas, appendix, stomach, bowel.
It was a hard exam. It was our midterm examination.

By Grace of God, I scored a 98%.

In the Year of Our Lord~ 2009.
In the month of November, on the 28th day of the month.

I am in silent contemplation right now.
The reason for this is that I am reviewing material as well as getting ready for tomorrow's Health Fair.
I am one of the group leaders of the Vitals Group, and am feeling nervous, anxious, excited. I must be in the medical campus by 7am tomorrow, as the Health Fair starts on 8am and shall end around 3-4pm. I will be there for 8 hours straight. We are estimating 600-1000 patients. So I will be seeing about that much, There are 15 of us in the Vitals group. and are expected to see at least 100 or so patients each.

I pray to God that He calms me, and make this smoothe.
That is how I am feeling right now.

I will leave for Church at 5pm my time, I pray that Church Mass will strengthen me and that the Eucharist will continue to uplift me. In His name, I pray.

It snowed hardcore in South Jersey yesterday. I drove home for break and realized that even with 4L drive, my suv had to slow down during the snow bound roads of Southern New Jersey.

I LOVED it.

When I arrived, I could not help but play in the snow for a bit.
Memories of my childhood when i used to play in the snow with my sister and my old school friends replayed in my mind.

Alas where can the mind go if sleep is deprived? How can a body operate if sleep has been deprived?
I have taken a new policy of sleeping at least 6-7 hours a day. I refuse to let my immune system be compromised due to lack of sleep.

That said, I cannot wait till the 3rd of April. :)

Yesterday was my first time of seeing a patient with the rare  HA (hereditary angioedema). Patient cam eiwth stridor due to the obstruction of the larynx via laryngeal edema and also had  colic. The reason for this was because of a defieciency of C1 esterase inhibitors. The attending came. Asked what the MOA (mechanism of action) was; we dictated. He agreed.

Treatment was rather simple.  We ordered this patient be given  epsiolon aminocaproate. Which inhibits plasma activation of C1; and thus reduces the symptomology of frequent episodes.

In a couple of hours, patient's edema subsided.

EFFING AMAZING.

She is exceptionally kind to me and our class.

Today we had lunch after our psychiatric medical review. One thing I notice was that most of us in the medical field like to use technical terms ; a rationalization technique.

As I ate with my colleagues, could not help but refer to the terms that Dr. Fadem, Professor of Psychiatric Medicine in UMDNJ's Newark Campus, dictated in today's lecture. And as we walked back to the lecture hall, was amazed how I began to psychoanalyze individuals as we walked back.

An effect that not only I, but several of my colleagues, started doing. Fritz joked, "Lets psychoanalze each other."

This is really interesting how we can use indomethacin to not only inhibit COX1 and COX2 but can also be used to treat acute gout and has effective regimen as compared to say, colchicine. For chronic gout: best to use allopurinol (moa: inhibitos xanthine oxidase; thus prevents uric acid from being formed) and probenicid (also used as adjuvant to antimicrobial therapy.

One thing i realize is that there are so many treatment indications for such drugs. But toxicity is already such a concern. Worrying about hematologic effects and then GI disorders too.

No wonder why residents and attending physicians are so strict; sharp memorization is necessary.

accounting. why are they even teaching us accounting principles in epidemiology? Assets and liabilities and equity is fine and dandy, but is this really necessary when treating a patient and etiology of disease?  ::)
When did treatment protocol require the knowledge of subsidiary and general ledgers? ha ha ha.
