I woke up this morning to find out that there were mosquitoes in my room. I guess a big gush of wind opened the windows in my bedroom last night. Spent most of the morning cleaning and killing those darn mosquitoes.

I think I killed like 8 so far.

Was at St. Bernard's Hospital today. From 2pm till 3:30pm. My time.
Had to help the nurses stitch an elderly woman in her late 60s. Poor lady fell down the stairs and cut her foot. Luuy intawn because she was showing signs of dementia already. Advance stage.

Please pray for her.

Today was such a long day, def. Didn't come back home till 10pm my time. Been awak since 8am nonstop. Thank goodness its a Friday. Today was pretty awesome, we got to see --well i personally got to see drug interaction.

The attending gave me a patient situation--we had one patient whoshowing signs and symptoms of Tuberculosis at the same time he was also taking Melformin as treatment for diabetic mellitus. The attending physician asked us, what should we do-??

He asked me and my partner what mode of action should we do?

Well it was very obvious. Looking at his charts, clearly he had TB, so we opted to advice he be treated with rifampin as treatment for possible Tuberculosis. What dose? What effect? He asked us.

We were aloof for a good 5 minutes--until I remembered that Rifampin is also an inducer of cytocrhome P450 isoenzyme; so if we give him rifampin, his cytochrome p450 enzymes will increase, and hence metabolize any melformin drug he's taking to treat his already diabetic condition.

What should we adjust?

Obvious na.

Patient suffering from diabetese mellitus, taking melformin as treatment; now pt is showing signs of tuberculosis, must be treated with rifampin, but dose of melformin has to be increased or doubled due to metabolism of melformin via increase of c-p450 isoenzymes.

Correct. Pharmacology for the day. I was happy, naka answer mi adto.

To see the attending writing our prescription recomendation and dosage was rewarding for us. :)

Its amazing how much scientists/medical researchers go just to preserve their name / immortalize their contributions to science. Even if it means just impressing their names to a medical condition.

For example, there is a condition that involves the bacterium. Neisseria Gonoccus. An infection of the liver--leads to inflammation of the liver capsule, this condition leads to the formation of abnormal tissue connecting the outer band of the liver to the abdominal tissue.

Physicians refer to this condition as : Fitz-Hugh-Curtis Syndrome.

or FHCS. Such a long term when in all reality we could just call it PH (perihepatitis). Because, in all reality, that is what it is. ;D

Thats what life is all about, friend.

Its all about struggles. And overcoming those struggles, By Grace of God.

There is nothing we cannot do that God has not provided us with the means to overcome those struggles/obstacles.

The meaning in this--put inasmuch faith in your own abilities as well as have faith in God's abounding GRACE--that wills you to overcome whatever obstacle it is that separates you from the Love of GOD.

Nothing in this life is without struggles. Christ, OUR LORD, HIMSELF, suffered and died on the cross for us.

Its up to the individual to find beauty and grace in his or her struggles. In Jesus, it is called Sanctifying Grace.

I dont understand why the attending gave that one patient Nifedipine to treat his Heart arrythmia. Yes, Nifedipine is a CCB (calcium channel blocker), but its a weak heart affector and focuses more on vascuar vessels.

A far more effective Cardiac affector would have been Verapamil and Diltiezam.

And when we told him about the lower affinity for dihydropyrodine in treating cardiac defects--he didn't even listen. Nicedipine was effective back in the early 90s, but now we have newer drugs--cardiac specific.

We are 2nd Year medical students. I don't understand why some students have to cheat in exams. Or show notes during exams. During my exam someone tried to copy of my answers in the scan tron sheet. This had me reeling. I had to report this individual after the exam.


Preposterous! How can you be a medical student and try to copy from another medical student?

You passed Medical Legal Ethics last year, but you are copying ?
Where is the ethical notion? Where is your morals?

How can you be be a Physican-in-Training and have the audacity to copy and cheat in exams?


I have no sympathy for you. Absolutely absurd!

How can you even ask that kind of question? First of all ACH is standard for acetylcholine--it is the basis for cholimimetics and cholinergic drugs. Its basic biochemistry and pharmacology. You would know this had you not cheated and copied on exams.

HTN: Standard acronym for Hypertension.

Jeez, thanks alot Doc, but 120 slides just to explain nutritional deficiency was quite overwhelming!

So at 3:00pm we'll have Psychiatric Behavioral Science lecture.

Im pretty excited to try out these new psychoanalysis.
Since reading into psychoanalysis, ive been psychoanalyzing people as of late. The way they talk, the way they walk--their mannerisms. Its wierd because it becomes almost natural and one gets en sync to the procedure.

And remember, Rupa, don't forget that when we are treating a patient with only hypertension, if that patient is below the age of 55 and not suffering any other ailments aside from hypertension,

The first action will inlcude treating that individual with ACE-inhibitors.
If that does not produce wanted outcome, we will add treatment with Calcium Channel Blockers.

And if that does not help, we will utilize a combined treatment of:
ACE-Inhibitors, Calcium channel blockers and Thiazide Diuretics.

My friend Albert arrived on Sunday 4am.
Good to see an old friend, and a future 1st year matriculatee.
Sharing some good memories of our NYLF/MED experiences back in 2003.

The irony of the situation:

We both attended the National Youth Leadership Conference on Medicine, held at the prestigious Georgetown University in Washington D.C back in July 2003.

Now, its ironic and befitting, that we meet again, in July 2009.
In Medical School. lol!

Our paths meet again. God Is Good.

God is Indeed Good.

It was rather dusty outside today.
I wish it wasn't so dusty and hot this time of the year.

One can't help but to 'blow some steam' due to the heat outside.

Well, what can I say? Today was just another typical day, I guess. Work, and more work. Its 2:08am, still im not finished with microbio. Almost done notes for non-envoloped viruses tho. Lounge is silent and quiet. I can hear the ac blowing hard.

Should head out for home soon.

On the 11th of July, I attended the IMSCS {Indian Medical Student's Cultural Show}
Had alot of spicy dances, spicy drinks, good people, relaxing pood. hehehe.

Collegiate understanding and undergraduate works even master's work mean nothing, when it comes to medicine.

First, let that be made known.

In undergraduate college, I took 2 semesters worth of Microbiology and Virology, but the course Im taking right now in 2nd year Med : Medical Microbiology and Integrative Immunology is totally different from undergraduate schooling.
The focus is totally different. Undergraduate focused more on the genetic and molecular aspects, in medicine, we focus more on the clinical aspects, reading data and lab analysis, diagnostic testing, preventatives and treatment (if any).

My advice to all pre-med students and or students considering medicine is this:
--It's never too late or too early to start reading. Read journals, articles, read medical papers, read micro, read lab works, etc. Just get your hands on something and READ. Believe me, when you start medicine, you Won't even have time to do anything else but read assignments, or to finish a 500-600 paged textbook within 2-3 weeks time, and at the same time finish preqs in other classes. So Read, Read, Read.

There is no such thing as procrastination here. If you do, don't expect to do well. I'm telling you that right now. To your face, direct, honest, truth.

So start early. Be proactive.
But as the saying goes, "You do what you can with what you have, and with the time you have alotted to you. So work."

Today is Saturday, USA eastern standard time~ 11:51 AM

Im in my cubicle, reviewing, typing up err, writing down notes for mycology.
Frech vanilla flavored coffee w/ evaporated milk. one sugar cube. Oh so good, so great, so inspiring, on a sweet Saturday afternoon.

Work, work, 38 more days till the end of this semester. then our 3 week summer break. :P
