Important Reminders for New Applicants of Philippine Passport * Applicant (18 yrs. old & above) should be physically present on the selected pick-up date for the signature & thumbmarks on application form.
* Mother or Travelling companion (if mother is abroad ) of minor applicants (17 y.o. and below) should be present during pick-up of requirements for signature & thumbmark on application form.
* Verify first all the information on application form before signing. Name (unless with change of status), birthday, birthplace, and parents' name on application form should exactly be the same with the Birth Certificate from NSO in SECPA.
* If application is rejected by the DFA due to reasons like wrong/incomplete requirements, under the look-out-list, has a pending case/memo, double passport, discrepancies, et al, documents will be returned with the partial refund. An amount of P400 is deducted from the payment as courier & handling fee.
* Upon delivery, any authorized representative may receive the passport provided the applicant's authorization letter is presented with the applicant's ID and the representative's ID (both IDs must be valid, w/ picture & signature). If applicant is a minor, any of the parents or guardian can authorize.
* Applicant (18 yrs. old & above) should be physically present on the selected pick-up date. A "print out" of the accomplished on-line application form will be brought during the pick-up date where the applicant will affix his signature and thumbprints.
NOTE: DFA has the right to request for additional requirement/s deemed necessary to process the passport application.
IMPORTANT: Only applicants are allowed to enter the DFA. A companion is allowed if the applicant is a senior citizen, a minor or disabled.
