Cancer (neoplasm) is an uncontrolled growth of cells due to a genetic defect of tumor suppressors, as well as a deficient cell-mediated lytic system, whose main goal is to destroy normally senescent cells (impaired ability). Lung cancer is but one type of cancer. Lung cancer is when the TNF (tumor necrosis factor) is deviated or destroyed due to a mutation of the P-53 gene in the chromosome, due to high / long term exposure of smoking. The continuous trauma to the lung epithelium by smoking and inhaling smoke changes the natural epithelium into stratified squamous epithelium, and continued irritation leads to deformity of the tissues and leads to pre-neoplastic conditions and then later neoplasia. And thus this growth spreads to the lung tissue and grows uncontrolled.
When this neoplastic growth enters the lymph node or the systemic circulation, it becomes malignant, meaning the uncontrolled growth of cells (cancer) will spread systemically. Lung cancer will spread to neighbouring organ systems that share the same blood route, thus it spreads to the adrenals, brain, liver, and bone.
Patients therefore who do not smoke can get cancer if they are exposed to the same effectors as those who smoke. In this case smoke, particulates, per chronic duration.
This is why there is such a large percentage of cancer deaths due to second hand smoking.
As far as i know, there is no debate about this anymore in the medical community. It is widely accepted truth. In fact, treatment regimen depends on this truth.

what we are doing now (new treatment regimen) is called gene therapy. We are now working on synthetic gene therapy to produce excess P-53 gene products so as to combat the neoplastic growth. This has been quite effective on animal experiments; if it continues , it will proceed to higher stages in FDA experiments and if it passes regulations, will be used in human treatment.
This treatment regimen is based on our accepted medical understanding of the processes that leads to cancer and its spread; and most of all, the mechanisms of action that facilitates it.
